My room was ample for me, with all necessary amenities. I had plenty of heat and light. It was kept clean and fresh. Toilet was easily accessible - as was the shower. The bed was typical hospital bed. I'd have preferred my bed at home, more softness, smoother sheets. But it was hospital style and kept clean and fresh daily. Most important was the staff; the care I got was A+ It could not have been better. I was super impressed with therapy and nursing. They were all - without exception - kind and caring and competent. They formed a truly healing space.

My room was ample for me, with all necessary amenities. I had plenty of heat and light. It was kept clean and fresh. Toilet was easily accessible - as was the shower. The bed was typical hospital bed. I'd have preferred my bed at home, more softness, smoother sheets. But it was hospital style and kept clean and fresh daily. Most important was the staff; the care I got was A+ It could not have been better. I was super impressed with therapy and nursing. They were all - without exception - kind and caring and competent. They formed a truly healing space.

My mom is 83. The first time she fell, she was 81 and she choose Liberty Commons for her rehab care. She had a great experience with the staff, PT and her accommodations. They gave her the care she needed for 3 weeks. Being elderly, she fell again at home and insisted on going back to this facility. This is now her 4th time falling and the family is going to check out the nursing care unit. My mom is a very independent woman and will only go somewhere that she feels comfortable. I will consider the low ratings but I am not sure why they are rating Liberty so low. I would recommend this location for rehab care. I do not know enough about advanced care but from what I have already discovered, I am hopeful.

Our family's experience overall experience with Liberty Commons Rehabilitation was terrible. It is beautiful on the outside, but I would advise against choosing this for a loved one. My father was there for rehab after surgery at 91. The PT team was not helpful and in fact after improving at the hospital, he declined because of incompetence at Liberty Commons. When he arrived at home, he had blisters on his heels of his feet and multiple bed sores. Our initial social worker was very rude and did not follow up. She inappropriately laughed during our call concerning my father's care. We had to request a new social worker to follow up with us. It was a nightmare for our family.

My grandpa had acute here few years ago and they were excellent we Took him home to care for him at home after a stroke finally dementia kicked in and it became increasingly difficult as we have children and work Liberty Commons told our grandpa that he always had a place there to call home so after having a terrible experience at another place we had him transferred immediately he has been cared for in such a way that I can honestly sleep at night no place is perfect but I will say they treat him like he is family he is cared for every day ! At some facilities you see confused demented patients sitting around the nurses station desk sitting in their own feces or crawling out of the recliner’s : here they are in an entertainment room and they have not only the nursing assistant staff but they have activity aids to stimulate them and intervene when they become confused And need attention ! Right now during a virus we are not able to visit we talk to him all the time on the phone the staff is so helpful with everything we need to keep us close to him! Liberty Commons is top notch I work at the hospital in town myself I would not send my family member anywhere else!

My friend had a terrible experience with this facility. Admission was horrible and staff was rude when questioned on run down amenities. She left to another facility asap.

We did find Liberty Commons for my loved one. It's very clean. They have a special unit just for dementia and Alzheimer's. The staff is very professional, and they keep my loved one very nice. They spend a lot of personal time with him. He likes them. He gets along with the crafts people and social workers. It has been a good fit for him. The food has been OK. It is fresh food. It has vegetables, protein, and carbohydrates. Their main meal is at lunch. It seems to be balanced. He likes his breakfast.

The place where my husband is, is called Liberty Commons Nursing Facility. It has a rehab facility and a long-term care facility. My husband came in as a rehab patient, but he didn't improve, so he is now in long-term. I think the nurses and the aides are wonderful. I am very fond of them. They are all so friendly and pleasant. You hardly hear nasty words from anybody. They are all very nice and attentive to my husband, so I am very grateful. They have a variety of rooms. They have a semi-private, which is two people to a room. They have some single rooms. My husband is in a semi-private with another person. His room happens to be very small, but it looks out onto a patio, and it's kind of pretty. So, I have not asked them to change it. His roommate has an even smaller space. Some of the two-bedroom rooms are bigger and look nicer because they are not too congested.

My husband has been a temporary resident for rehab here and also as an outpatient. He has received rehab at other facility but here it is the best! As an inpatient the staff is kind an attentive and as a wife I feel secure when he is there.

My grandmother, who died there recently, was admitted to this home back in 2007 after it became clear that she would be unable to care for herself anymore. The staff there were wonderful. We could not have asked for better care for her from the staff. The made extra visits to her all the time and treated her like she was a member of their own family. Most of the time she could not fill out the menu for her daily meals but the staff rathering then letting the default meal be given to her filled out the menu according to her likes and dislikes and made sure that she got ice cream with her meals every so often even through she wasn't supposed to have it. When she finally died there a large amount of the staff came by with tears in their eyes to give their condolences and share their stories of her with us, and even through we were supposed to be responsible for packing up here stuff and removing it by the time we got there the next day most of her stuff had been neatly packed into boxes and some helped pack the remaining stuff with us. Several of them tried to make it to the funeral which was several hours away from the home.
