Grace Nursing Home Upon entering the building, someone can answer any question about your loved one. The building is very clean. My loved one enjoyed living there because she had no responsible Her clothes wash the food was good three times a day. being around other people who were friendly. Staff, visitors, and other residents. The buildings were very clean. The staff are very knowledgeable about their job. it was a pleasure to have my loved one as a resident at Grace nursing home.

My mother stayed in this facility after Ida. It was horrible. They do not communicate with one another and never give you the same information. When I finally moved my mom the nurse literally tried to refuse letting my mother leave. Then when we packed her things we left 3 small bags of trash. We asked the staff on hall 100 where to throw it away they said they’d take care of it to leave it in the room. Well they did they sent it in the van to be moved with my mother. Not only that but they kept a custom made blanket with my mothers name and the name of her grandchildren. They lied and told me it was in one of the bags that she had 3 bags of personal belongings m. Which turned out to be the trash we offered to take out for them. They are rude and inconsiderate. They are extremely rough my mom has bruises all over her from them being so ruff. She was much better off on the hall 3 better staff. Moving her to 1 was a huge mistake on their part. Then to top it off for weeks I have spoken to Hayden about moving my mother completed all proper steps with the new facility and Grace. Hayden was supposed to take care of everything as far as letting HIS STAFF KNOW SHE WAS MOVING. He did nothing they knew NOTHING about her moving. I want my mothers blanket it’s sentimental. Why send my mother away with trash. Rooms are out dated, dirty, it smells horrible, they ignore patients who don’t speak. They do not communicate properly at all. The staff may nice but it doesn’t matter when you provided poor care. All my mother did was lay in bed uncomfortable EVERYDAY. If you’re forced to put your family there make sure you visit everyday randomly. Pray to God you live close. I didn’t choose this place she was brought there after evacuating for IDA. At first it was ok there. I can’t say I was a fan of the place it’s outdated but bc it was ok temporarily. Once they moved her from one hall to another there everything changed. Her health and quality of life all declined when she moved to hall 1.

This definitely is not a friendly , caring place. They want you to make appointments but while I was there I couldn’t even find a nurse to answer my questions. One lady folding clothes in the hall and that was it. So when I did she took my information to have them call me back and no call back so how the heck are you supposed to visit and follow there rules if you can’t even make the appointment. I could go on and on.

My grandmother was at Grace Health and Rehab not once but twice. The care was exceptional. The nurses were caring and understanding, especially since my grandmother was pretty demanding. The CNAs spoiled her rotten. They knew she always wanted her coffee first thing in the am and made sure she got it. The place was always clean and never smelled of urine. She didn't acquire a wound while she was there. I've never seen another place that was better.

I am the daughter of a previous resident, who was there for rehab. He left there in much worse health than when he arrived. He had bed sores, one of which was to the bone! It is awful...they are under staffed (although they will say they are staffed according to the state standards) and the staff they do have are over worked. Most of the aides are rude and do not care at all about the residents/patients. The smell in the hallway was of pure urine, the entire time my dad was there. I made numerous complaints and that only made the staff bitter and resentful. If you care at all about your loved one...PLEASE DO NOT admit them to this facility. Even the doctors at OLOL told us not to leave our dad there. So, we have since brought him home and hired caregivers to be there when family can't. He is now being treated by a Wound Care Specialist. We had the worse experience with Grace Health & Rehab Center. They definitely DID NOT rehabilitate our dad!

Grace Health & Rehab Center was clean. The nurses were very helpful and the food was good. This is a very new facility. The only thing mom didn’t like is she had to share her room with another resident. They gave her physical therapy twice a day so she had her strength back up. It was a good thing we put her there. The staff was very friendly. There was a lady there who would braid her hair every week. They were all sweet, helpful, and nice people.
