The rooms are old and need to be remodeled. The food looked unappetizing and tasted horrible! There are some staff that are very caring and attentive. Most are there just for a paycheck, to smoke and socialize! My sister would push her call light, the CNA would walk to the room and turn it off and might not come back for hours. I know because I would talk to my sister on the phone for hours. She was bed bound and totally dependent on staff for everything. They would not put the call bell where she could reach it. They would leave her in soiled diapers for hours. She developed a horrific decubitus or bedsore because they didn’t keep her off her butt and didn’t put her on a specialty mattress until too late. She has had to had multiple surgeries on the decubitus and that decubitus is ultimately what will kill her. They allowed her to fall and dislocate her hip. They claimed they never saw her fall. Yet she told me about it right after it happened. Many nurses would leave meds on the bedside table and she would drop them! As a nurse, I know that is NOT allowed! The nurses did not appear to do V/S or nursing assessments. My sister always asked me why they never checked her V/S! The administration kept saying they would make things better, but they never did. She has been in the hospital for months now. She will NOT be going back to that NON nursing home.

This place is one of the WORST nursing home ! My great uncle died here when i walked in it spelled like the place was not taken care of . They treated my uncle so disrespectful.

I would want anyone to know the subpar care my husband has received and he has complained of the food to me. The facility has cigarette butts on the cover area at the main entrance where we have our 30 min visits. On weekend the side of the home is strewn with garbage. This should not be how to keep a home for visiting family members. My husband is there for skilled one care. Well I noticed two small places on his right thumb knuckle and for days I asked for someone to attend to this before it became infected. On Thursday the 22nd myself and his home care can, Charlotte, went for a visit and she went over to the glass by the front door and came back and said it was infected. The corporate head nurse was waiting to be let in and I ask if I could have a word with her and I explained to her how my husband’s hand had not been doctored as I had asked. She step inside and called nurse over and immediately someone came and put a bandage on his hand. In a couple of days I was told they had put him on an antibiotic for the infection. It never should’ve gone this long unattended. I visited my husband over the weekend and he looked dirty and unshaven and his hair was stand all over his head. On Monday I could see scabs in his hair. I called the administrator outside and should this to him and I went to the drug store and purchased a shampoo for seborrheic dermatitis and brought it back to the home and was assured they would clean him up. This condition would have never if they had been easing his hair and rinsing the soap out. I could add more, but hopefully you get the picture. Put your loved one in another facility. .

too many of the cna's do not check on the residents they are in charge of care. cna's have been seen asleep in recliners in a residents room. a resident may not see their cna for a complete shift. weekend nurse does not pass meds on a timely schedule. 7-9am meds not passed until 11am so the noon time meds are not given to residents. wrong meds have been passed to residents. food is subpar even for a nursing home. complaints to management are dismissed as unimportant.

A zero rating would be more appropriate. The rooms are in serious need of updating and cleanliness is surface not only with the rooms but patients as well. Very Poor supervision of patients needs. The Lack of personal attention is shameful. Our family deeply regret-using their services for our loved one who suffered severely there from lack of being fed food and water along with unsanitary care for the 90 days we used them. The response from management was superficial and useless, full of empty words and broken promises. Regular State monitoring is seriously needed at this facility.

So, within 24 hours of my relative being in the facility he had fallen twice due to lack of care from employees. We expressed our concerns after the fact, and my relative continued to be neglected. Since the first incidents we have walked into my relatives room with him completely naked with no sheets. The facilities excuse? "We ran out of linens". They have told my relative to no longer ask to go to the restroom, to just go in his diaper. My relative has fallen four times during the two months he has been at the facility. A nurse, by the name of Karen, told my relative (when I expressed concerns because he was in bed with no clothing) that if I had a problem with the facility we could take him out and leave! This facility is filled with rude, neglecting, arrogant staff. I can only imagine what harm is being done to the residents who's family members don't visit daily!

My family member has had multiple falls at this facility and the last one resulted in a broken bone that was unchecked for a least 2 weeks. Family was not notified when they finally sent this resident to the hospital and their negligence resulted in an amputation having to be done for an infection. I'm at a loss for words.

My dad was at Jefferson Manor for about three weeks. The staff responded to his needs; if there was a concern, they responded to the concern. It was very clean. We were satisfied with them.

I am in Jefferson Manor. They provide pretty good services. The rooms and the surroundings were nice. The facility is clean.

I have actually visted this place twice as I am looking for a home for my mother with AZ. I was very impressed with how clean the place is and how the staff is very attune to the guests that live there. Three times guests have asked whomever I was speaking too questions and each time the staff assisted the guest before me. I was impressed because it shows how much they pay attention to their guests.
