Out Mom lived at Fountain West for over 2 years. She did not want to go there but then she loved it there. The staff was wonderful! They fixed her hair, manicured her nails, and treated her with great respect. She was just as happy as possible living there. On her 90th birthday, we presented the nursing home with a rolling popcorn machine in her honor with a plaque on it stating that she donated it to Fountain West. The Director, wearing a straw boating hat, unveiled it! We had a bowling party and popcorn with purple and lavendar (Mom's favorites) M&Ms. Great fun!

The primary thing we liked about Pine Acres Rehabilitation And Care Center was that they have Medicaid beds immediately available. Secondly, just walking in and seeing the seniors that were there, they all looked happy and were enjoying an activity. There's a large open space for eating, playing bingo, watching TV, and doing that stuff as a community. The rooms were bigger than what I had seen in other places as far as Medicaid beds where they have to share a room, and the rooms were brighter and seemed more homey. I met with a social worker who gave me the tour and she was very helpful. She was able to show me a couple of rooms that would be very similar to what my mom would be in. The nursing staff that I saw all looked happy, engaged, and energetic. They had PT and OT in-house, a hairdresser who had a studio there and came in once a week to do haircuts for the residents, a dentist and a podiatrist that came in once a month to see residents who needed that, and transportation to and from appointments. They also had an activity room and a separate eating area for residents to have gatherings with their families. The only thing was I had to initiate most of the contact with the staff.

My mother was in Fountain West Health Center for respite care. She seemed to like it really well, she liked the room service, and she liked being waited on. However, they're short staffed, but they did the best they can. She was there for only 20 days.

My mother was a resident at Fountain West for almost 4 years before passing this past June. I would strongly recommend this facility for your family members. This is a tough line of work to be and please everyone, but they did a great job in all facets with my mother--wonderful nurses, great food and meal program, and administrative staff was always jumping in to help at lunch, with activities,etc. I actually have just finished placing my brother in this facility--they have been very helpful and respective--especially Holly and Michelle in administrative. I am very hands on and feel they do a great job with any concerns I may have.

Activities are only bingo, showers are rarely completed unless you are there in the facility seeing your family member. It smells of urine terribly. The simple cares were not even done for my mother while she was here, there was reports to state officials to investigate things. It seems like employees hate their job. Reconsider when considering Fountain West nursing home. Our families deserve better.

My mother was placed at Fountain West November of 2016. While the staff was at times short handed , the full time staff we always found to be caring and compassionate from the kitchen, laundry, nursing staff to the administration. We are grateful for the love our Mother was shown during the time she was there.

This is the worst facility ever. The staff are constantly short (the residents need more caring staff to help take care of them especially on the weekends and at nights during the week), I wouldn't feed the food to a dog, the building should be torn down and reconstructed, the case managers lack knowledge, the activities that the residents participate repeat themselves, it is just an overall negative place. I would NEVER put a family member here. HORRIBLE!!!!

The Skilled and Rehabilitation Hall has very old a poor quality mattresses which with hard plastic covers. My family member who was living independently had a large bloody coxyx sore on his rump that had to be treated at a hospital with infection. Also back issues from the mattress from hell cause further pain and debilitation. For the cost be in this facility patients should have an adequate bed mattress. My family member was able to walk. When looking for assistance on several visitations there were no staff to be found, yet I saw a group in the break room. The daytime staff does well, but the evening and night staff seem to not have enough help, but also slack. Also those not able to feed themselves in the "special" dining area are give little bites of food and most is taken to the garbage. When I asked about this, I was told that they "the staff" does not have all evening to sit and feed patients as they barely eat and are slow." Some of the plates did not look as if anything was attempted.

This , Fountain West Nursing Facility, is the absolute worst Nursing Facility on this planet. The disrespect and disregard, they express to certain patients, whom they decide are acting independently of their wishes for the proper zombie like behavior they require, is unbelievably and totally disrespectful towards their clients. Their attendants are aggressively rough, when handling fragile women with physical injuries. They neglect the people for hours on end as they sit in their restrooms and wait for help. The cold slop, they serve for food,is not fit for man nor beast. They leave people in their beds, in rooms with no heat, and one thin blanket, and no socks on their feet. People go in healthy for their age, and within two months those same people will contract multiple disgusting health situations that no one should ever have to endure.The place should have a complete staff firing and a new staff of caring professionals should replace the incompetents.

Horrible place! Patients are ignored for up to two hours! My mother got scabies from one of the workers and even though the doctors at the hospital found the mites in a piece of skin, the nurses and aides denied her diagnosis. There are so many horrible things at this facility, I can't begin to list all of them. My mother was taken to the hospital with severe dehydration and urinary tract infection.
