"Rehab" is outsourced to a third party Paragon. Brief rehab not sufficient to be helpful. No communication unless you initiate it. Never saw the physician assigned to my father. No returned phone calls from his office. Over 1 month. Lots of people on "vacation". Too bad for you. Father was neglected after he was cut from physical therapy. Did not put him in a wheelchair. Smelly, soiled. Had to plead to get his clothes changed. Staff said he refused. He does have some dementia but that does not mean he should not have been cared for properly. Sheets not changed. Management needs to wake up and "manage". Some medical attention would also be nice. Father now in hospice after his "rehab"

I am very disappointed by the experiences and interactions I have had with the staff at the Rehabilitation Facility this past month. I had heard from others that this was supposed to be the best place in NWI. In my opinion, this was not the case, with the only caring people I talked to during my visits being two receptionists at the front desk. The other CNAs (RNs are very hard to come by in this facility) appeared to me to have no idea what was going on. During my walk down the hallway, I would be asked by at least 2-3 patients to "help them" or "try and find a nurse". I will be sure to share my experiences with not only the board, but associates at this facility including the director of nursing, coworkers, family, and friends. It disheartens me to think this is one of the best rehab facilities in NWI. I encourage everyone to do their research prior to sending their family members to the Rehab facility or to any facility at all in that matter. This includes reviewing the the Investigations of Complaints that are available to the public via a quick search on the Web.

I would give no stars if that were possible. My husband was sent there to rehab his foot after surgery. He he also has Multiple Scelerosis. He was not to put any weight on his foot. They had him on a stationary bike and tried to have him walk at one point before he had to remind them about the doctors order. He had to use the bathroom and hit the call button and it went un answered. He tried to get out of bed and into the wheelchair on his own and nearly fell, it took him 15 minutes to get himself into the chair on his own to use the bathroom. Upon arrival he was put in a room and left there for over 45 minutes before he called me at home and informed me of this. When I got up there a CNA came in with a tray of food and a cup of juice had spilled and she was going to give him the tray in that condition. I asked if she had some paper towels and I would wipe it up. She went into the bathroom, grabbed toilet paper and mopped up some of the juice and threw the soaked toilet paper in the trash can splashing both me and my husband with some of the juice. The day before my husband was released, an administrator of some sort came in with documents to sign that should have been presented to him when he first got there. Thank god my husband called me and told me about this. I told him not to sign anything until I could look at it. It was literally a 40 page document that was about his rights as a patient/resident of the facility. A welcome packet was also included. I nearly laughed out loud. Also a word of caution and I mean this seriously. Whoever is in charge of handing your insurance claims, watch your bills and know your policy. These people are trying to say we owe them close to $4000 for my husbands one week stay. They are saying we had a deductible we didn't meet and WE DID. We have very good insurance and our company has been trying to contact them repeatedly to get this resolved and no one calls them back. We have now tried to contact them by mail and we are still getting bills for this insane amount of money. We have the backing of our insurance company and they are working with us and documenting every time I call and try to resolve the issue and every time Hartsfield ignores or doesn't comply with requests. So please beware and research facility's before you put your loved ones in the care of others. I personally will never trust another one. This whole experience makes me believe it's all about money. If that's true, remember money grubbers, you too will be old or hurt some day and pray you aren't treated this way.

My 85 year old mother almost died at Hartsfield Nursing Home. She was there for therapy and fell out of bed onto the floor. She had a brain bleed and broke her wrist and rushed to Community Hospital Er. She spent over 2 months in the hospital and like I said almost died. That place is awful. The rounding and care at night is bad, not very many CNAs and help at night, very light staffing at night. The food is salty and very dry. Half of the CNA's don't have a caring attitude toward the patients and many patients wait over 15 minutes to be assisted when there light goes on for assistance. I would never recommend that place to anyone. I visited several nursing homes in Northwest IN and its so sad, the facility is dated and old and the staff did not seem very attentive. We now have our mother living at home with a live in 24 hour care giver. At least she is watched and not left to die in a place like Hartsfield Nursing Home. Be afraid…very afraid to leave your love one there. Since this fall, I have heard several other patients from Hartsfield ended up at Community's ER from falls!

I had my husband in for a rehab stay and the staff were outstanding. From the moment we walked in and met with Admissions and the Director of Nursing, we felt at ease and welcome. A very professionally run home that was clean and friendly.

Had mom (87 yrs old) there 2 weeks for respite care. Clean, smells good, attentive to her needs, lots of activities. Will be going back again for respite care. Deb
