I am a friend or relative of a current/past resident

My dad moved into Lakewood Nrsg & Rehab Center in February. It's good for short-term. But as far as long-term, they have a few amenities there. It's not always consistent as far as games and activities for the people to do. The staff is very knowledgeable. It's pretty mediocre standard rehab and long-term care. For activities, they have a Tuesday and Thursday ice cream social. And then during the week, they'll have like a movie night or some game night.

I am/was a resident of this facility

I'm in Lakewood Nursing & Rehab Center for rehab. I was here previously in hospice, but I didn't die because they took such good care of me, and I believe that to this day. I think they're great because obviously they saved my life. I'm not going to brag about the food, but it's passable. They have activities every day. They do bingo and board games and they break into groups. I have not been very active in the activities myself. Every time I go past the cafeteria there's something different going on and they do charades and bingo. The food is adequate. The staff is very professional and they are very strict in terms of the requirements, and there are people that were here years ago. They remembered me. I was impressed with that. They keep everything up-to-date. The fact that I'm here for the second time after them saving my life, and the fact that there's so many staff that were still here, I think that's a very good sign.

My mother has been in Lakewood Nursing & Rehab Center for six years now, but I had wanted to relocate her a few times. She was recently hospitalized for a severe UTI infection and bedsores, but she's doing well now. They have very bad administration problems. They can't keep an administrator. Over the years, it runs hot and cold. The staff has been good and Mom has a couple of very good nurses, but the staff changes like the wind. They have a beauty shop and a lady comes in once a week, but you pay for it. The rooms are suitable, some better than others. The atmosphere is fine. The receptionists are very helpful. The rotation of the staff makes a big difference in the care. They need to get it together. I visited my mom on Friday, and they didn't have the dining facility open because they had no staff. The food is awful. I wouldn't feed it to my dog. They do not keep good records of their medications. They've overmedicated her.

My mother is at Lakewood Nursing & Rehab Center. The place is very clean. I was impressed because the person in charge administering my mom actually reached out to me before we even knew whether my mom was going to go there or not. Then yesterday, my mom had physical therapy, and the physical therapist even called me to give me an update. I like that they're keeping me in the loop. I've not been there while she's eating, but she said the food was good. The staff seemed to be very attentive. I had a conversation with the nurse the other day because my mom is hard of hearing, so she can't even hear with her hearing aids anymore. The nurse came in with a white board, and that's how they're communicating with her. Rather than not communicating at all, they write stuff down, and then my mom verbally answers them. I've been impressed with them so far.

The room my Dad is in is way too small for 2 people. Make sure if you visit that you make them show you the room your loved one will be in. The staff seem nice but it’s hard to overcome being packed into this tiny room. The food is pretty bad but we didn’t expect much there. We are just praying he won’t be here long.

My husband was placed at the Lakewood Nursing Rehabilitation Facility on December 20, 2018. He was admitted at Lakewood because he did not have a supplemental insurance. He came from another nursing facility, which was high standard. His wait was 198 pounds at the other quality nursing facility. Within two weeks, he went from 198 pounds to 217 pounds while at Lakewood. Lakewood let him eat anything he wanted knowingly he has congested heart failure, some of the nurses were slow to not calling back, and when coming in Lakewood I would smell urine and deffication. I was warned by agency members, which worked there as CNA to keep an watchful eye because of there lack of care for their patients.

Lakewood Nrsg and Rehab Center was excellent. They have good staffing. They kept my mom clean and they kept her fed as much as they could. The atmosphere was very good. Its location was in good spot too. It was 25 minutes from where I lived. My mom shared a room with a roommate. They kept her room clean. She went to watch a couple of movies, but she wasn't very active at that point. They have movies and bingo. The staff members were awesome.

Lakewood Nursing and Rehab are very helpful in getting my father in law situated. The physical therapy and the nurses have been fantastic, and the staff is all good.

I would just like to comment about one of your employees, her name is Jamie and works in activities and actually I see her running around doing so many things. She goes above and beyond to make residents smile. She really makes my mom happy and that's not an easy task, but Jamie always goes that extra mile. I'm so grateful for her. Too bad there aren't more Jamie's in the world. It would be such a better place as she truly knows and cares for the residents. She is a true asset to your facolity!!!A big thank you to Jamie and her hesrtfelt, hard work! I will be writing an overall review soon, but Jamie really had my attention today.

My mom is 60 years old and here with cancer to get rehab so she can walk again. So far they've forgotten to feed her, they've made her wait 45 minutes to use the bathroom and have let dementia patients wander into her room and scream at her. Shes deathly allergic to peas and they have given her peas. She cant have milk and they send her milk everyday. One of the worst I have been to in a long time. It looks nice and has nice stuff at it but the care is awful!

My mother was in Lakewood for rehab. The appearance was OK, the sanitation was good, and the rooms had two beds and a bathroom. They had ice cream days, and they had an indoor birdcage where they can watch birds. The nurses were very good, but the admin staff was not very helpful on following up on issues. There was poor communication among the management, the nurses, and the rehab specialists.

I visited this facility

I've visited this facility many times and am always impressed with the care and attention of the staff. The facility is beautifully decorated and always clean. The food can be hit or miss but I can tell they have tried to improve this area greatly. I really appreciate how pleasant and friendly everyone is in the halls. The therapy staff has done an excellent job with my family member. They are so approachable and knowledgeable.

I am/was a resident of this facility

I was a patient there for 3 months and the staff treated me like family. I have even gone back to visit since I went back home and the staff welcome me with open arms. I went there from St Joseph Medical Center and my same therapy doctor followed me there. The building is better than the last place I have been to. This building has an ice cream parlor and is much more modern as well. I highly recommend this place! Best Thanksgiving party ever!! My family still talking about it

My father was at Lakewood after he broke his hip, he also had dementia. The place was clean but I was not happy with the car he had. Many times we went in a he was sitting is his room with his food tray just sitting in the room and no one was helping him. Many times we went and they had not even gotten him out of bed. He was suppose to be there for therapy so I asked if I could watch a therapy session. It lasted 5 minutes. Had him lay in bed and stand up, sit down. I was not impressed at all. I never really knew who his nurse was, as they never really talked to us. Most of the workers put their heads down when they walked by you as to not have to talk.. As soon as we thought he would have to go on Medicaid all they wanted to do was kick him out of the nursing home. They were very cold about this. We moved him to Sunny Hill nursing home and could not have been happier with the care he was given. I would not recommend Lakewood nursing home.

I am/was a resident of this facility

I was a rehab patient at this facility and can tell you firsthand this place is a dump. The furniture in the patient rooms are older than me and I am 61. Just rundown and falling apart. The staff here was useless, on many occasions I would push my call button only to wait 30-45 minutes for a response. On some occasions noone came at all. The only redeeming value in this experience was the rehab PT and OT group did teir job and gotme back on my feet. If you are thinking of placing a loved one in this facility check it out before you do and I'm sure you'll go elsewhere.

My dad was in Lakewood. They have a lot of staff, which was good. It's very clean. My mom said the food was good. They're pretty good with the people. It's just, I think, sometimes with the medications, they give out too many narcotics to some of the patients. I would not give the residents as many narcotics as they do because they're at more of a fall risk, and my dad did fall five times there. So luckily he didn't fracture anything. That's the only thing I didn't like about the place. But they had lots of activities, like exercises, ice cream socials, bingo, and they also take them out to movies, or to Walmart to shop. They do have services to take them out and about. They actually have a barbershop and a beauty shop where the ladies can get their hair done and get their nails painted. They have birds they can bird watch. When you first walk in, they have all these different birds that they can admire.

Am not very happy with this place. My relative has had to wait over 30 minutes at times to get a bedpan. One CNA even told me she never put a patient on a bedpan in the two years she has been a CNA. Patients call lights are on and noone can be found to help. Patients in the hall are grabbing my arm and pleading "help me". This is not how the elderly should be treated.

Excellent care. Lakewood is a great place. The staff is exceptionally good at what they do. My mother was there for 18 months until the end of her terminal illness. The Lakewood staff took great care of her. I would highly recommend this facility. The staff were kind and friendly. Everyone made you feel at home. Great attention was paid to every need she had while there.

I visited this facility

My whole experience at the nursing home was wonderful. The staff was all very knowledgeable, and i was impressed with how well they worked with the residents. Some were not very cooperative with the staff. I visited around christmas and it was nice to see they had the place decorated with a tree and everything. The kitchen had a nice variety of items for residents with different diets or tastes, not just one menu for everyone. They also had a dietitian. There was a recreation room where residents could play games like bingo or watch a movie. They were playing "it's a wonderful life" when I visited. They had a piano for residents to sit down and play and sing christmas carols which i played with a woman. After talking with the staff, I learned that they had several planned outings for the month, as well as a bunch of in house activity nights with things like crafting and trivia nights.

I visited this facility

Lakewood Nursing & Rehabilitation Center is a very visually appealing place. Upon entrance to the facility it feels a bit less institutional then other nursing homes I have been to. They have a nice courtyard area for residents and family to visit. The dining area is ample but, a bit like a cafeteria. Lakewood also features an inhouse beauty parlor with the look and feel of going to your local salon. For a little fun and a nominal fee residents and their families can enjoy a scoop of ice cream in the ice cream parlor. The rehabilitation center has many tools and exercise equipment to aide in rehabilitation. The resident rooms are cozy, but lack the feeling of home you feel in the lobby. I was always greeted kindly when I arrived at Lakewood however, the rest of the staff lacked kindness and personality. I rarely had an easy time finding the correct nurse or cna taking care of my family member if I had questions. When I could speak to the person I needed I felt rushed; my concerns were not being addressed. Worse yet, the administrative staff seemed to have a very quick turnover rate. I always felt I was speaking to someone new when I would go up the ladder with my concerns about my family member. If you can get past the lack of staffing and their personality defects, Lakewood is a very nice facility. They have a wide variety of amenities and tools to make you or your loved one feel at home during your stay no matter the length of stay.
