I am a friend or relative of a current/past resident

Some great staff members, but most are useless. Never cavailable, meds not being dispensed on time or not at all, people screaming for help, awful food and not very clean. Always smells in the corridors and rooms are extremely dated. No joy or happiness exists in this place. No happy interactions. Very ugly and bare. Sadly, not the worst of nursing homes around, but needs serious improvements and kinder staff.

I have my brother here and he has been here several years. The place is fine and a lot of nice people. I used to call and no one would ever return my calls. They went through a lot of head nurses but there has been a new lady Denice who I have been speaking with about my brother. She always returns my calls and is very helpful. Finally there is a person who cares about the families as well as the patients. I feel so blessed to know I can now call and get the information I need.

My father in law is end stage Parkinson’s. He needed skilled care nursing. He was a handful when it comes to care- wheelchair bound that quickly graduated to reclining wheelchair bound. Needed to be fed, dressed, turned, oral care, depends, occasional meds so he didn’t hurt himself or others...he was a handful and I get that. Family visited daily. We saw that he worsened in the two and a half weeks he lived there. I advocated for him and his needs more than should have been needed regarding dehydration, physical therapy, Ted hose, and medication tweaking so he’s isn’t so sleepy all the time. I feel like they should have known to do oral care and turning Q2hours. Some staff did. Some staff is good. But there just isn’t enough good staff for patients like him. I’m not going to sit here and say this location and all the staff are terrible. I don’t feel it is. They are all very nice. Most residents who can walk and get around and communicate will do great here! it doesn’t smell bad. The rooms are nice. It is clean. (The laundry is annoying, though-we didn’t realize we should wash it ourselves before some clothing was lost. He frequently was wearing others random clothes before we said we would buy a hamper, wash it and bring it in ourselves). I cannot stress enough that the staff is nice! I will say, though, that if your loved one is more work like mine, this may not be the best option. After 2 1/2 weeks, my father in law was severely dehydrated, had several bedsores— one very bad, and his tongue looked supper bumpy and red like he never drank or received oral care. He ended up in the ICU and soon hospice care. like I said he is end stage Parkinson’s. Elderly people have thin skin and bruise easily and he didn’t always make it easy. Don’t see this as a bad review, but a be diligent and visit often and speak up if you see issues review.

The staff at South Suburban Rehab Center was nice, but they're understaffed. The appearance was so sterile and almost prison like. My father wasn't that happy there. We were doing his laundry, but his clothes always came up missing. The rooms were decent sized, but they didn't have closets. All they had was a little armor cabinet, which looked like it came from the 1950s, and it was only 20 inches wide. They didn't have activities. They had an area where they had a TV, and a room where they ate their meals. It was industrial looking. The people that I was in contact with were very helpful, polite, and informative (to the best of their ability). When I went to visit him, the bathroom had dirty soiled diapers on the floor. The staff who took care of the residents were good, but the management part of it wasn't necessarily good.

South Suburban was not very clean, and my dad said they don’t take care of them. My dad was in urine in a diaper on the floor for I think 4 hours; he fell, and they didn’t pick him up. The residents lack patience and fight with each other, they just let them fight at it. My dad has been punched in the face. He was malnourished and dehydrated. It was just terrible.

I visited this facility

It was much better than I last remember. It used to be horrible. It smelled really bad now it's so clean and the staff is friendly. They take good care of the old people there. They look like they enjoy living there. I visited with my friend and she was going to see her mom.

Stay far away from this facility. Not only is it filthy but the staff is incompetent and the director is clueless. My mother's meds were discontinued before her discharge. These were psych and pain meds. Why this was done I still don't know why. It caused her to go into a relapse when she was moved to a new facility. She said it was the worst experience of her life.

My actual rating is zero stars. I am a friend of a resident here. He had his hip removed and needs to get rid of a MRSA infection he contracted at another hospital. Since he has been here he has only had one decent meal. I have had to come in and empty his urinals. I have to bathe him because nobody assists him. Today they came in his room and told him that they were going to put him out because he refused medication. In actuality, one of the nurses brought the medicine and hung it. Through her incompetence she punctured the bag. Instead of admitting to that she went back and told them that he refused the medication. That was not his fault and she didn't bring him another bag. The dosage is administered every 12 hours and should not be administered any sooner than 11 hours. He received the dosage at 4pm and they came back to give it to him again at 6pm. He explained that he had just had a dose at 4pm and the side effects are such that his liver could be damaged. The nurse became indignant and left and told the administration that he refused his medication. This is not true. He was correct in not accepting the dosage. My friend is being put out of the facility because he would not sit still and let the staff mistreat him and further endanger his life. He has no hip, he cannot walk and cannot support his own weight yet they feel justified in putting him out. The facility is not up to standards so he is not disgruntled. He really never wanted to be here but was resigned to it because he needed a place to be in the interim before he is readmitted into the hospital to receive a new hip. He is upset however; because they are banding together and lying on him to disguise their laziness and incompetence as well as their unwillingness to do their jobs.

My dad is in South Suburban Rehab Center. It is a very good facility. It is very warm and inviting. It is very secure. They are very personable and friendly. The dementia units are phenomenal. They are amazing. They manage a very challenging population, but they are very attentive. They are very good, very compassionate and caring. They are friendly to the family. Every time I am there, it feels like a very good experience. The dementia units or rooms are shared rooms. They are very clean and well maintained.

I am a friend or relative of resident

By far this is one of the worst facilities in the south suburbs. Mbr father has been a resident here for about 5 years. Many of the other area nursing homes are filled and having a waiting list so we were unable to have him moved. I could go on about their lack of care for hours, however the facts are. My dad has been in the hospital for dehydration 3 times, and he has fallen many times. This last example of dehydration was fatal and he is now in hospice with only days to live. We were told at the hospital that he had not been fed or properly hydrated for a while. My dad has dementia so he is unable to express himself and he no longer knows us. My siblings and I visited him weekly but that was not enough. The facility is almost always understaffed, and by appearance many of the staff members just don't care. It seems as though the residents that are not able to express themselves are often left to fend for themselves and not very well taken care of. In addition to the lack of care every single piece of my fathers clothing that we bought for him ended up missing/stolen and the false teeth that we purchased for him were stolen within 3 days of purchase despite his name being etched into the teeth and his name was also written in sharpie markers inside his clothing. If you are considering this facility do yourself or your loved ones a favor and keep looking. It may save their lives.

We chose South Suburban Rehab Center because of the proximity to family members. It seemed decent at first glance compared to the other places we went to. The staff seemed attentive in the beginning but became negligent latter on. They did not take her to the activities because she was on a wheelchair with oxygen attached, so they had to move those machines with her. Sometimes they moved everybody but she was left in front of the television. They also did not change her every 2-3 hours but ususally once a day. She was in a private room before but they switched her to a double room, which looked like a hospital room. The dining area was fine but the food was undesirable. I think they need to improve on the staff because basically they have staff shortage.
