Management is the worst at this facility. They have made up lies about my stay. I went to go get treatments at another facility and the manager of social services refused me back at their facility. I had no place to go. Then they would not take my phone calls to address the situation at hand. Their management team is full of liars and unacceptable characters to manage a facility in charge of people’s lives and family members.

I couldn’t imagine just how cold and cruel people could be towards the elderly and sick until my experience at this place with my mother. The entire staff should be changed. U have no idea who is who because no one wears their I’d! Not even management if they do, it’s turned backwards. They’re rude and nasty when you ask them questions as if how dare you. They don’t answer the call lights for hours if they decide to. One staff member told my mother’s roommate NOT to turn her call light on for my mother, left my mother wet two nights in a row. So for two nights my mother cried wet, feeling like she couldn’t speak up for herself due to possible retaliation. They gave her 19 pills to take at one time, couldn’t tell her what she was taking, after asking the person who portrayed being her nurse, because again no name tag to identify who this person was for a copy of her Med list, he replied that has to come from the social worker. To have 3 people in one room was horrific, no way they had the proper amount of space required especially in these times. The water ran for the entire four days my mother was in this particular room, you couldn’t turn it off, you asked why of course no one knows if maintenance was notified, it’s probably still on. The resident personal items were left sitting on the sink, basin, soap, lotion, towels, etc. You had no idea what belonged to who! No names on anything, I guess they don’t believe in infection control! I overheard a staff member talking to her disrespectful coworker about a resident being evil! They showed not one ounce of respect for the residents nor visitors. The state surveyors were there and when asked to speak with them, they said they wasn’t there even though for two additional days the notice was on the entrance door! The customer service is shameful, the care is nonexistent! I know during this time a lot of facilities are dealing with agency staff! However these people deserve better! I never seen anything like this, I’ve worked in the nursing home setting for 11 years and never saw this! My heart is broken for the treatment my mother received and for the people that are still there! I could actually continue telling things I personally observed but I’m sure people there are comfortable with what they do and how they care for others. This place is sickening to know lives are at total risk for Deterioration and only because of the care from the staff!

This place is a hot mess . Staff is rude and do not care . My mom arrived on Saturday (Feb2021) no one even gave us a call to let us know she made it safely . Then when I call to check on her they were rude as if it wasn’t my right to do so . I’m my is alert and able to express herself with no trouble . They forced her to wear a diaper. Did not check on her which resulted in her falling and hurting her face .

My mother is in The Villa at Evergreen Park (Evergreen Health Care Center). It's a pretty nice place, but some of the staff members aren't very good, are unprofessional, and they don't come to work on the weekend. Somebody's always taking off and they have a high turnover of CNA workers. They also don't know very much about dementia. When I asked the nurses questions, some of them get offended when I take the time to see how my mother is doing, and what kind of medication she's still taking. I don't think my mother drinks enough water so I go there every day. I'm kind of tired, but I do it for my mother. The food is pretty good, but sometimes my mother can't eat like she used to, so most of her food is chopped. Some days they don't read the diet and they send her food that she might choke on, so I had to go to the kitchen to tell them that my mom can't eat it. Sometimes I don't want to argue with them so I go out to buy my mom something to eat. On the weekends, it's pretty bad, like they'd give her a sandwich with some meat in it, but sometimes they don't put condiments or ketchup or seasoning on it. The activities are pretty nice, like Bingo and going to movies. They take my mom to exercise, therapy, and social hours, so they have pretty decent activities. I just don't like that they keep my mother by the nurse's station when I'm not there, and it makes her antsy. I tell them that I don't like it and they're supposed to have enough staff members to keep their eyes on her. They're supposed to check her every other hour in her room, but they don't do it. A couple of times, my mom has been soaking wet, so I raised a lot hell about it. The CNAs don't communicate and just come in with a frown on their faces, and some of them aren't friendly. My mother doesn't like people coming in to her room all frowned up. They don't say, "Good morning," or tell her, "This is your medicine." My mom asks questions because she wants to know what they're giving her, and they think you're supposed to take whatever they give you without question, and I don't like that.

Evergreen Healthcare Center (The Villa at Evergreen Park) was all on one level, and it was a good place. They had an area where they can rehabilitate people. The people were friendly, and they were OK. It had nice grounds where you could go out, but you do a lot of walking because it's all in one level. It's such a big place.

My mom is in a rehab facility at The Villa at Evergreen Park. The facility is nice, but I am not happy with the treatment and the care Mom is getting. I think they could be better. Some people are good, though. They are understaffed, so they need to improve on their service. The facility is very nice and up to date. The amenities are really nice. They have activities for the patients, like group activities. I didn’t see all the patients participate in them, but they did have them. The rooms are nice size and updated. In the rehab facility, the equipment that they have is OK but could be updated a bit.

On our inquiry of Evergreen, they made wonderful promises with regards to my father's needs that's why we placed him there for a rehab. The facility was nice and clean. It also smelled good. It seemed that they have an attentive staff. However, when I go and visit him, he was just there lying in his bed. He got tired of lying and watching TV that he told me, "I'm not moving, I'm not doing nothing." I asked them why weren't they teaching him how to walk with a walker and why wasn't he in therapy? They said, "Well, he's at risk for falling." I thought if he was going to fall somewhere, it might as well be there, not at home. That's why he was there in the first place - for therapy. I checked this rehab center out on the internet and it seemed like a wonderful place; however, that wasn't the case with my dad.
