The room they put my mom in law is awful. The window doesn't close all the way bringing in cold air. The door is falling apart. They took forever to take her IV out. Her dinner was supposed to come at 5pm and she didn't get it until 5:40pm. Then the chicken was mush, the rice was pink, the ice cream was completely melted. She has a rash on her bottom and I really hope someone takes care of this. She's here because of recovering from the hospital and needing 24 care. This place is supposed to specialize in dementia. I wanted my other choices didn't have space. I pray they do better or some phone calls need to be made.

I gave this Facility five stars in every area. My mom have been to many Nursing home for rehab. Crestwood Rehabitation Center cares!! I feel like they are here to support me in this difficult time and to take care of my mom. My mind is at ease knowing she is in good care. From the social worker to doctor, nurse practitioner, RN, cna , Admission Terrai is the best. and supporting staff all work together for my mom to reach her goal. Activity Director has actively really daily and they are very entertaining. This place do not smell bad! Food is good! Rooms are big. I am so grateful that my mom is happy being here. Thank you Crestwood Rehabilitation Center

THE STAFF IS VERY PROFESSIONAL AND VERY KIND WHEN YOU ASK QUESTIONS THEY ANSWER THEM VERY PROFESSIONALLY AND VERY CLEARLY SO YOU CAN UNDERSTAND THEM MY MOTHER IS IN THE FACILITY AND THEY TAKE VERY GOOD CARE OF HER AND I KNOW THAT SHE'S IN GOOD HANDS WHEN I LEAVE HERE AFTER VISITING.

Our experience with Crestwood Rehabilitation Center has been very good. The people have been excellent. They are attentive and caring. It's a clean facility and they're responsive to any concerns that I may bring up. They listen to my husband when he talks. Staffing is very good. All the meals are also good. My husband has a nice, large double room with a roommate, and it's big enough for two people to have space. He's by the window so he gets a lot of sunshine. It's a nice clean facility. It's not old, but it's about mid range, and it's worth the money of having him stay there.

My mother in law was transferred there December 3, from munster community hospital. The plan was for her to be somewhere she can have therapy to get her back up at least able to get in a wheelchair. ( with in house dialysis) My family called for three days . I was told someone would contact me with a care plan. Nothing on the forth day I asked to speak to the director. That's when the person who answered the phone wanted to be of assistance. They wasn't calling us. They claimed they called her husband who passed away in 2017 phone. And other contacts from a contact list from a visit in 2016 mind you her contacts was updated at the hospital. It's been 14 days she has been in the bed no therapy we still can't get a real care plan. I was told by the nurse director she has been in the bed because she was on the covid unit. I understand that but she has been off the unit 3 days. I went to go pick up her dirty laundry. Her clothes are still packed and cleaned from the day my niece dropped them off. They never even touched the bag. I called and asked what's the procedure and was told by the person that answered the phone it wasn't their job to check or count clothes. We was told the activity director would be calling so we can have a zoom. Haven't spoken to her yet but I'm sure she has a few voice-mail. I called to speak with a dialysis nurse. She couldn't tell me anything about her dialysis said I had to talk to someone from day shift but day shift told me to contact her. At this point I am at my wits end. They are using covid as an excuse to not do their job the social worker wants to know her care plan for home but it seems like they have no plan solid plan or solution their. They can't tell me a food plan. I don't know what the rooms look like because we are not allowed in because of covid. I am really upset I think I'm going to get my lawyer involved because this is really got me baffled.

I have been there and it is a nice facility. The therapy department got my aunt back walking. The staff was great!
If you love your family member run from this place as fast as you can.

This is one of the worst places I have seen in a long time my parent is currently a resident and it was a terrible decision to place them there. The facility is always under staffed, the food (which looks bad) is always cold and the smell is terrible. I had to request a room change due to the mold over the entire wall. Please if you care about your family member do not bring them there. The staff is not helpful useless you are there everyday and a side from 1 or 2 Nurses and CNA's your family member is on their own.

The food isn't nice, it's always cold. Don't request to transfer unless your prepared to call the other facility and your case worker for your insurance and set it up yourself. They drag their heels and don't know what they are doing. The staff aside from a few RNs are not helpful and deffinetly have no compassion.

The worst one to me was Symphony of Crestwood (formerly Crestwood Care Centre). The odor, the lack of cleanliness -- it was horrible. I would not recommend anyone to go in there. In fact, I know someone who transferred their mother out of there. I wouldn’t put anybody there. It was just bad, and they claim they’re going through some rehab. They even were going through rehab on one floor, and the other floors were disgusting. We came after work, and the beds didn’t look like they have been touched. I wouldn’t put anyone in there, and I wouldn’t recommend it to anyone.

My mother was at Crestwood Care Centre. She had Alzheimer's and they did have a special floor for just Alzheimer's patients where they have a little extra staff and everything. The facility seemed very clean. There were two or three beds in every room, but my mother only had to share the room with one other person. The staff was wonderful. They were so gentle with my mother and very caring. If she had a slightest fall or anything, they called me immediately. If I wasn't there to feed her, they always had an aide to help her. Food smelled good. Regarding activities, they did not do too much. They did have different activities and they would try to get her to participate in some. My mother would do okay with that. The dining area looked nice and there was plenty of room. One thing I was impressed about it is that there was plenty of room in between the tables for people like if they have a wheelchair, you can maneuver a wheelchair through there if you had to. I would recommend this facility to anyone. When my mom turned 95, they had a few people come in and they had a special room over there where they had a little birthday party for her. They supplied the coffee and stuff for us. They made you feel they actually care about their patients. The morning that they called me when my mother had taken the turn for the worst, they thought that it was the end of her. So they put her in a room by herself where we could all come in. We had the privacy of that room at that time and every one of the staff, they kept coming every little while to tell me how it was a pleasure to have taken care of my mother and they just made me feel really good. They were giving me a hug at that time and were so sorry about that. They were just so wonderful. Also several of them came to my mother's wake.

I like Symphony of Crestwood. It seemed to be clean. It was not the cleanest place, but it was far cleaner than many I've been in. It seemed to have been very well administered. It was a friendly community. There was a lack of privacy on one hand but on the other hand, it was far better than some I have been into. For this facility, it was a room situation where two people would be in the same room and that I did not really go for too much. The dining room was nice. It was very pleasant. I like the working staff. The personnel were friendly. The admissions director was wonderful. She was very pleasant and very helpful. She made me feel comfortable talking about bringing my brother there. I like the activities. They have indoor activities but not for the winter.

I didn't like Crestwood Care Center at all. This is the first one I toured. The staff was too busy to bother with you, and the place smells. I didn't see too much activity going on. They just showed me a couple of rooms and that was it. The rooms were small and they didn't seem that clean.

For some reason, my father wasn't being given his pills on the 2nd shift. His cancer medication cost over $10,000 per month and although it was covered by insurance, my family kept a careful inventory of his pills every week or two to make sure the right amount was left. They kept finding that there were too many left over and it was determined that his second dosage which was to be administered in the evening wasn't being given. He also was perpetually covered in bruises and sores for some reason, and constantly expressed his unhappiness and loneliness living there. Additionally much of the staff came across to me as rather unprofessional in their behavior. For instance they would ignore you for a while as you waited at the help desk to discuss or request something, while they leisurely engaged in their own conversations with each other which were not related to their work or the nursing home. I would not recommend this particular place to anybody.

In my opinion this is a pretty typical kind of nursing home in that it seems to provide adequate care, but nothing extraordinary and certainly not any bells and whistles. Once you pass the lobby which is nice and has a chandelier, the halls are much plainer and more institutionalized. I will say that what I think is special about Crestwood was the staff who seemed to genuinely care about the patients and knew them quite well. The rooms that I saw were shared by two and three beds and again, they were rather stodgy and could certainly use some redecorating.
