My mother is at Life Care Center of Orange Park. The staff members are extremely caring and kind. The food is very good. They have choices for every single meal. The facility is well cared for. They have a lot of activities. I see people in several different large rooms for different activities. There is always a schedule of activities posted, and it looks very good.

This facility and staff cares nothing about your loved one. Unattentive. Had fl staff yell and scream at our relatives and myself when calling. Has told the family member she was a bother and didn’t want to be in her area. Does not come when called for over an hr. Just don’t place your family there.

DON'T! A few staff were wonderful, but majority were uninterested and didn't care. Food is brought to the rooms, temperature is cool to luke warm at best. Setting up trays ie cutting food or opening condiments just don't happen. Call bell are not answered promptly, taking 20ish minutes or more to respond. Dementia training is not done, nor do they even attempt to provide care. Staff turn over is a problem. Finally, changes in medication is NOT conveyed to family. Care is only as good as, you the family, can provide.

-- What little communication that is provided is not always truthful. -- The staff makes promises and then does not follow through. CNAs are rude and impatient at times. -- Men are sent in to change the clothing of female patients. -- The nursing staff is fair, except for the evening staff that ignores the patient's needs making them wait 15-25 mins for requested help. -- Therapists are kind as long as a family member is present. -- Therapists do not follow the doctor's orders they do whatever they think is best regardless of doctors' orders. -- Therapist are not attentive during sessions they chit-chat among themselves instead of encouraging patient. -- The food is very well presented although in most cases it is lukewarm to cold by the time they get it to the patient. -- Life Care will not allow a family doctor to come and visit. LC attending doctor Never visits thus the only medical care you'll receive is that of a nurse. -- The buildings are very clean and pleasant. They have several places for patients to gather. -- Security is pretty good. They have a signin system at the door and the doors are locked in the evenings. Never saw a security person. -- Management is difficult to contact.

Very angry about this place. My mother went in there able to stand up on her own but had problems walking. They were supposed to help her walk but now She can't walk or stand at all. And on top of it I found And on top of it I found encrusted encrusted dark poo on her behind and between her legs this morning when I tried to clean her. She just came home Saturday and did not poo until this morning. The dark stuff was the old stuff that they didn't clean off of her. I am so angry I olspit!!!!! I will never allow her to go there again! And

My experience has been varied. The rehab services are excellent with very caring workers. However, the nursing staff is very up and down. There are some very good nurses but there are some others who were rude and very uncaring. I waited over two-and-a-half hours to get something for a headache and for nausea. The same complaint holds true with some of the nursing aides.

No communication with the family at all! We were never told our family member was transferred to hospital till days later when we were supposed to pick them up, they lost their clothes, their phone charger, the left hand was swollen & looks like it's been broke. Not a single person from that place would call me back on these matters. Would not recommend to anyone!

Nursing home looks great to visitors, but is not what it seems. Food is decent. Activities for residents are okay if they are taken to them. UNDERSTAFFED, especially at nite when people aren’t visiting. The care is terrible for residents who are bedridden. My mother was there, and I went bf work and afterwards. Mixups with medicine and my mom almost died twice, bc she was coughing up blood. Nurses said it was chocolate she had eaten. No one would take her to bathroom. Never put your relative here. Unbelievably bad care. They let you see what you want to see if you visit once a week, but I was there everyday. It’s not good,scary place to leave a helpless old person!!!!

Short staffed. Very good PT and OT staffs. CNAs are overworked!! Food disgusting. CNA made my friend/relative wait in his/her own excrement for 1/2 hour to up to over an hour between meals because are they are also the kitchen bussing staff and priority is getting all dishes and slop back as a priority over answering the patient call button. Repeatedly gave my friend/relative unnecessary stool softeners and laxatives resulting in constant diarrhea. Not a great combo with aforementioned note on dishes vs. patient care. Also misdiagnosis resulting in hospital stat mid-way thru rehab. Would never recommend.

I liked Life Care Center of Orange Park very much, but they won’t take my dad because of his diagnosis. The staff was very friendly. It looked clean, and they had a lot of activities.

They have very good physical therapists at Life Care Center of Orange Park. My mother was in there in November and couldn’t walk at all, and they got her walking and moving. They listen to me. I am over there every day, so they know me well. If I have any concerns, I can meet with the caregivers in her room and the therapists. The lady in the admin office and I are on a first-name basis. They have been very responsive to me and listened to what the hospital told them to do with her food. The response and help I got from the admin office in getting her in there was great. Melissa in the administration and the admitting office has been wonderful. I'm changing doctors from Dr Luis, who was assigned to my mother. He has sent her to the hospital next door twice, by ambulance, which is hugely expensive. For no reason. He tells me she has pneumonia --this was oinn Dec, and the hospital kept her overnight and sent her back on Christmas weekend, asking me why she was there. There is no pneumonia. Heart is steady for her age. No reason to be there. Today, he sent her again to the hospital, stating pneumonia and heart. Again, the doctor at the hospital said No Pneumonia. No change in her heart. Why was she there??? Dr Luis also told me he asked her who I was this morning and she had no idea. When I told her what he said, her jaw dropped. He wrote dementia on his send out sheet to the hospital -- which is far from the truth. I have no idea why he told me that, and my mother was furious with him. This doctor is irresponsible and apparently can't read the hospital reports or the X-Ray --he told me her X-Ray showed pneumonia all week. Which was proved false by the doctors at the hospital. So two times pulled out and ambulanced to the hospital on his whim. I've had it with this one. My mother's glasses have been missing from her room since Jan 2; and it has taken two weeks for their 'food police' to decide she can actually have regular food instead of the pureed mystery meal they've pushed on her. I keep taking her cheese burgers for lunch --she eats regular food just fine. So they changed her food back to regular two days ago, but the kitchen apparently can't read instructions either. Hopefully this has finally been corrected today --I'll find out when she calls me about what she gets for dinner and whether she sends it back again! My mother tells me the night staff are loud and noisy, so I told her to have them shut her door. I'm there every day, so the day people know me now. I get very good responses from them, as I don't hesitate to ask them or direct any concerns I have to them.

My family member was transported to Life Care Center from the hospital in the evening. Life Care Center knew days in advance my family member was coming, that they had special needs, and that they had meds that were needed and not provided until a day later. The CNA on duty took the "CARE" out of Caregiver.

I went to visit my mother-in-law at Life Care Center of Orange Park. The staff were marvelous and very attentive. The rooms are very nice and very clean. They were immaculate in fact.

The facilities at Life Care Center are nice. They're always clean and they don't smell, which is good. The staff is very effective and knowledgeable. The only issue that I still have is that nobody answers their telephone. Communications are terrible. So it's a good thing that I'm able to go and visit my mother three times a week or so, and handle communications that way.

A friend of mine, an RN, had his elderly mother in this facility after a stroke. The family and patient were pleased w/ the care.

was in the rehab part of this center in Orange Park, florida as a parient for 20 days after hip replacement surgery. the rehab people did a great job, helping me to learn to walk all over again. nurses were good and nursing attendants.
