My sister is at Fountain Manor Health & Rehabilitation Center. She is in the skilled nursing. She has been there for two weeks now. I've been there at least three times to visit her. It's more than acceptable. The personnel there were polite. It looked like they are always paying attention to what's going on with the residents there. My sister's room has been clean the times that I have been there. Most of the time, she is in bed. There was a moment that she was able to express feelings and tell me that she was OK and felt good there. Once I was there at 9:00 a.m. My sister still had a breakfast tray on the side table, and it was a more-than-acceptable breakfast. I haven't witnessed any activities, but the person there told me that they take my sister to activities, and she seems to be content with what's going on there.

This facility was a horrible experience for my mother and for us. My mother had 2 heart attacks and a triple bypass so she went to this facility for rehab. The facility smelled like urine and feces all the time. My mother was left in her urine and feces for hours. The microwave was covered in roaches I notices it when I went to warm some food we had brought for my mother because the food provided was horrible. My mother was only in this facility for 3 weeks because she had a stroke while in their care and they didn't even notice. It was my brother that was there for a visit that noticed and immediately instructed a nurse to take her vitals and call rescue. Please do not send your love ones here

Rudeness, manipulative, deceitful is all I can say for the social worker at this facility. My stepmother has been kept there against her and our will. They did not provide the information required to transfer her to other rehabilitation facility with an available bed near to our home. No matter how many times I requested it they kept giving excuses to not release her. I’m filing a complaint with AHCA or the appropriate state agency supervising this facility. The lack of empathy, indifference and rudeness are common among this administrative staff. I can’t comment about other services inside the facility because I have not been able to see my family even once in almost four weeks. The clothes I bring were received in the parking lot. That is inadmissible, this facility should not be allowed to operate.

My father stayed at Fountainhead Care Center for around 35 days. It was clean, nicely manicured, nicely painted, and appealing. The staff was very nice, and the food was timely. He ate and got better. He went to the Christmas event, where they gave him gifts and had a pizza party.

My sister-in-law is in Fountain Manor (Fountainhead Care Center). She seems happy there, getting physical therapy, and being taken care of well. The meals and everything seem to be good. They have a doctor that goes there, nurses on staff and a lot of people. It's calm and nice. There's a big patio. When it's lunchtime, they're all in the cafeteria and when it's not, there are about 20 or 30 people out on the patio just sitting and talking. My sister-in-law has a roommate. Their room is small and doesn't have a TV; she has to go somewhere to watch TV. She could have a TV if she's going to live there, but right now she doesn't which is fine. She knows how to use an iPad, so she's using their Wi-Fi, and that's how we communicate because the facility is not really close to our house, so it's a little inconvenient. My sister-in-law seems pretty happy with the place and the physical therapy that she's getting. She's improving every day. It just needs to be updated since it's an old building.

Fountainhead is fairly clean. The staff is personable, understanding and aware. The rooms are small, and the dining area is big enough for its population.
