When I first got to Avente I was in really bad shape, I was in a car accident and had major surgery as a result of the injuries. Within the first week I was progressing immensely in my recovery. Not being able to stand up or walk at the time was an obstacle that they taught me to overcome. Soon after I was able to get myself into a wheelchair unassisted, use the restroom on my own. When my surgeon gave me weight barring clearance the very next day they had me up and on my feet taking steps in the parallel bars. After that they put me in a rolling walker and everyday I was able to more and more walking with their help. I would like to thank my physical therapist Mendy and Leah, you are the best. Then Nurse Pepe and Aldrain who stayed on top of all the medication I was getting after my surgery. All the CNA's were great, Tula, Precious, Ms. Lee, Manuel and all the others that worked the west wing. Thank You

Terrible, horrible place! I was a resident there for six months! I qualified for Medicaid, but they NEVER asked me about it or billed Medicaid! On 2/1/23, I was rudely and roughly awakened by Shelley, who told me to get up and out!! They put me, in my wheelchair, on the curb, with NOTHING! When my grandson went to retrieve my belongings, they only gave him 1/3 of what I brought there & purchased while I was living there I had 3 iPhone 13 chargers stolen! Many purchases were returned by the front desk without my knowledge! When I asked about packages, I was told nothing came in! LIES!! I had 2 seizures while there! I don’t have seizures when properly medicated! Some CNAs refused to respond to my “call light”! One CNA would come from the Acute Care hall to see me and told me my lights had been on for 30 minutes and the CNAs were just sitting at the desk talking, so she took care of my needs on more than one occasion! DO NOT GO THERE UNLESS YOU ARE CLOSE TO DEATH AND UNCONSCIOUS AND OBLIVIOUS!!!

Very difficult to contact someone to get information on patient by telephone or email. Phone removed from room. Too few staff members to provide adequate care for patients (e.g., 2 baths a week). Phone calls to nurses station cut off. Calls not returned.

I would not recommend for a loved one with memory care/alzheimer’s needs. Where the facility seems ok for short-term rehab, our experience as a long term resident with memory care was not a pleasant one. The administration admitted to not being able to handle a memory care patient after having accepted. Change/moving can be a lot of someone with loss of memory. I would go straight to a facility who has trained memory care staff.

My brother is staying in Avante at Jacksonville Beach. We wanted really active physical therapy, and they have the personnel to do it. They get him to the exercise room and try to get him to work on the bicycle. The staff is very nice; I haven't run into anybody whom I can complain about. The facility is older and not the best in terms of the physical aspect of it, but it's the only place that offers what I needed for my brother. They have activities, like games, for the whole month. They have a library.

We chose Avante Villa for my mother because it is close to her doctor, and it is five minutes from the hospital that she will be going to. The staff is really nice. They have good people that are really caring and are doing a good job. They had some changes in administration which improved the facility. It is old, but it is clean. I was glad to see that they changed some of the staff. I think they also need to improve the facility itself. It needs a little updating and remodeling.

My father had been in Avante Villa for a few months. The hospital that he was in at that time sent him there because they accept patients with trachs. He was there for a while until he was feeling better. It's an old building. I've seen bugs and ants in his room. The staff were all different, some were kind and some not friendly. He had a shared room just enough for them, but not so spacious -- just enough for the wheelchair to roll in. I don't like the food, and my father was on trach. The first day, they gave him a salad and a taco, and he was supposed to eat soft food. I was really disappointed, and everyday I had to bring him his own food. He was getting therapy, and that's all he had been doing. Patients like my dad have to be watched. He had been falling there. They leave him alone on the toilet, and he cannot do that. Now, he needs their services, and they declined him for the second time. They didn't give any reason for that. It's not fair for treating patients who are in need of help.

Mom has been in Avante. It's nice, it's clean, and it's close to the house for me. The people who work there are very nice. It's not necessarily as fancy as a lot of the newer ones, but it has a nice little patio area. It has one bedroom and a little private room like a dining room. It's just that it feels more homey than other places. I feel like they take good care, and there are people there who have their mother and grandmother there, and they continue this. More than one generation has gone to this place, so they seem to keep up with it. It's a good place. I would recommend it.

DO NOT TAKE ANYONE YOU LOVE TO THIS PLACE NOT EVEN FOR REHAB IT IS NOT MANAGED WELL THEY DO NOT HAVE A SENSE OF URGENCY ABOUT ANYTHING INCLUDING MEDICAL NEEDS THE STAFF ARE RUNNING AROUND IN CIRCLES BECAUSE THE UNIT MANAGER HARDLY COMES OUT OF HER OFFICE AND DOES NOT STAY UP TO DATE ON THE PATIENTS WHEN SHE SPEAKS TO A FAMILY MEMBER SHE REALLY AT LEAST HAVE THE CHART HANDY TO BE SURE SHE IS AWARE OF WHATS GOING ON BEFORE SPEAKING THEY HAVE NOT PROVIDED EVEN FAIR MEDICAL CARE OR BEDSIDE CARE I WOULD NOT LEAVE MY DOG HERE I LOVE HER TO MUCH !!!!!!!!!! I CAN NOT SAY ENOUGH PLEASE DONT TAKE ANYONE YOU CARE ABOUT TO THIS AWFUL PLACE
