After 17 days in this hideous facility, 2.5 years ago, I am just reading of others who had similar experiences. They said it as well as I could. This place should definitely be shut down. The staff for the most part are unpleasant and neglectful, and the powers to be are just as bad. I could not get out of there fast enough, because my own health was in increasing danger. Wrong meds, no one bathing me, talking to the patient very rudely and in general, just the opposite of a caring and well run facility. I do not know how they stay open, but their negligence has put most of us in danger.

I was in Page Rehab because i broke my right ankle and left foot. I was non weightbearing on both feet and in a wheelchair. This is just some of what I experienced. If I put everything in here it would be to long! Needless to say, I do not recommend sending anyone here. 12/31/22. Asked for pain meds at 12 30pm and again 2:00pm. It is now 3 30pm and I still don't have them. Used commode at midnight. The commode is next to bed so I can slide onto it. Pressed button. Nurse came in at 2am. I told her I used the commode. She said mam I'm just an agency nurse and she left. Commode was not emptied. After having used the commode a 2nd time at 2:45 I used the wheelchair and brought it in the bathroom myself to empty and clean it. 1/3/23. dinner shift. Afternoon Cna, is so rude and this is the one I have most of my time here. Doesn't speak to you...doesn't look at you. Throws the food tray on the table. I say thank you and no acknowledgement. 1/5/23 asked for tylenol at 2 pm. Finally got it at 4 15. This morning when my morning meds were brought in I got my pills and what appeared to be nose drops. I said this isn't my inhaler. She said yes it is. I said nope, not mine. She walked out, was able to find mine and brought it in. The CNA's don't come when you press the button and when they do come they act like your bothering them. 1/12/23 nurse comes in with meds. No inhaler. I asked where my inhaler is. She says oh I don't know. Goes out to desk, comes back and says they must have run out.? I'll have to order it. I have to use this inhaler every day (copd). Why was it not ordered when it got close to running out? The numbers are on the inhaler. 3 15 pm supervising nurse came in with my inhaler in the box saying she found it. It was in wrong drawer. She further stated she will make sure the nurses are aware. 1/13. 8;05. Nurse comes in with my meds. She can't find my inhaler? She is going to talk to the supervising nurse because they did find it yesterday. Not only did the nurse never come back, but neither did her supervisor. I had my husband sneak mine in from home yesterday and used it again today, but the nurse doesnt know that! just What is going on in this rehab? There are elderly here who would not know if they are getting their meds. What if they aren't?? The 4pm nurse just came in. I asked about the inhaler. She looked and it was there? She had no idea that I didn't receive my complete meds and a supervisor was never notified This nurse just said to me that the morning nurse doesn't work here. She is from an agency. My thoughts -- shouldn't she know how to find meds? The supervising nurse was told by the 4pm nurse about the incident. I believe she was not happy about coming to my room and told me she was not notified by the nurse about the inhaler. At one point my daughter called the director about some of these concerns(he recently took over) he came to my room to discuss the concerns and then asked if I prefer to be discharged? I'm here for rehab - I can't walk. It's also important to note in the weeks I spent here there is an elderly woman 2 doors up who would scream HELP! HELP! HELP ME! Constantly, all day and night. There were a few quiet hours early evening where she would fall asleep but the screaming was endless. She was obviously sick, I'm guessing dementia, but this was handled by staff by just shutting her door. You could still hear her just not as loud, but this probably wasn't where she should be. Friends and family that visited could not believe we were forced to listen to this. It was constant! 1/14/23. 8pm. Tonight's cna is the same one I had last Saturday for the afternoon shift. She is constantly walking up & down the hall either on her phone or she has her music playing. This would be ok but she talks loud and has her music very loud 1/15/23. At 3:30 am I was woken by a male nurse, he knocked loudly on the door, which is what woke me. Then he opened the door to see if I want Ice water? I am now listening to my neighbor scream for help. I am up for the night! 1/19/23 i just asked for a pain med. I just started these this week. I was just told by the nurse that my prescription was cut off. I know for a fact my doctor didn't do that because he just prescribed. Who cut me off? Nobody knows On a good note: I can say I had 2 awesome nurses- Pamela and Mrs P. 2 Great CNA'S - Pierre and Altida PT- Allison, Brian and Laura - the best OT- Shelby and Roslyn - awesome It's to bad the entire staff can't be as caring as the above! I have so much appreciation for the above named. Without them I don't think I could have tolerated my time here.

Long story short….the minute I stepped in I could see how rundown and filthy this place was …the smell was unbelievable,i continued down to find my mother,and I find her in a room where a lady next to her was begging for help and see my mother laying in sheets and a blanket I wouldn’t let my dog lay on…the lady next to my mom was begging me get get help ,so I went to nursing station and told them she needs help and they said she’s also screaming for something,that’s just her…then I went back to my mom in shock ( told the lady I tried but there mean,she said I know)…I look at my mom and thought she cannot stay here and cried…they can in and said give it a chance ,somehow when I finally said ok cried all the way home for her …got up early next am there by 7;45 was told dr was going in am,so I didn’t want to miss him …waited till 3:30 ish finally got there very nice older gentleman said let’s do her therapy for today and come get her tomorrow am and I will sigh for her release…next morning early am go to get her and they said dr said no she has to stay here two weeks ,i said there lying… and I’m taking her now ,he told me a different story…now they will not give me a simple from to get my mom in a ALF …if that is greed I don’t know what is… oh and by the way they lady screaming next to my mom, sister came the next day could see how dirty and soiled her sister was and had dr look at her and they realized the screaming lady begging for help had a stroke….emergency ambulance came and got her… This place ,I don’t know how they are legally open,the food was cold and looked like dog food….god help r seniors,they deserve so much better,and that’s all I was trying to do for my mom and I’m still waiting for a 1823 form and the place I’m trying to get her into was hung up on four times being transferred…I wouldn’t even rate them a star.though the rehabilitation people were very nice the one day they helped my mom…they get five stars

If you have the misfortune to end up in this facility, try your best to get out and go to another. This place should be condemned because of the negligence of the staff, the dirty condition of the room and the lack of personal attention. Why it has not been closed before this is a mystery.

My wife is safe and cared for. Rooms are clean. Bathrooms and showers in need of upgrade. Staff is professional and attendants are caring, patient people. Plenty of activities available. She is able to roam inside and out with help.

I am stunned that this facility continues to take "victims" in. These people are mean, do not understand nor care about the patient, do not bathe the residents and simply do not care for the vulnerable people who have to come into this abyss. The staff is incompetent. The management is worse. The medications that are dispensed are often the wrong ones, the rooms are filthy, and NO ONE CARES! Do not go there and do not allow your loved ones to go there!

This facility should be closed. It is full of employees who do not care, do not answer calls for help and are mean. The place is very dirty and full of vermin. I should be grateful I came out of there without any deep harm. DO NOT PUT A LOVED ONE HERE!

I wouldn't take a plant there. My significant other was a patient there. They did not know to do wound care and ended up giving him such an infection it required 3 drains and 3 different antibiotics to clear it up when he was transferred back to the hospital. The place is rated by Medicare as a 1 and now I know why. He was there 2 years ago and they were wonderful but now it is horrible. They don't answer call bells. The PT director was good but the rest was terrible.

This facility was shockingly filthy. Closet doors falling off, A/C coming off the wall and not working. There were cockroaches in the room, and the staff was belligerent and unhelpful. The rehab services were minimal. The meds that were delivered were always wrong and needed to be re-done. Do not stay in this facility!

We've chosen Page Rehabilitation and Healthcare Center. So far it's been fine. We just appreciate what they're doing with my husband, and can still have many happy years there. The staff is very good, because of the COVID problem I haven't been able to see his room, the whole thing, but just from the window, and it seems very nice. It seems very clean.

This Facility is Horrible. Please do not let your love ones go there. My Father almost lost his life. Terrible Service all around. I don't know how they continue to operate. They do things behind closed doors. Don't answer phone calls. Just a Horrible experience

Father caught Covid19 while staying in this center due to lack of employee screening and not having residents wear masks or practice social distancing My father passed Center never followed up with us and was not willing to say if other cases were reported. Staff was not particularly helpful to family while my father was there. He passed from Covid sad to say

Rooms are cluttered and very small, messy. Staff doesn’t know or care. My family member almost passed out in a wheelchair because he was left there unattended and for too long. Aides don’t answer call buttons in a timely manner, especially the night staff. Food was terrible, pinto beans over rice for lunch one time. I would never recommend this place.

For the price point, this is a well run, caring nursing home. The facility and rooms are clean, food is good. In general, staff is professional. The social and physical activities are minimal - often residents can be seen in one large recreation room 'watching tv'. (There should be more structured activities available.) The nursing staff is competent and promptly communicates all medical issues. Aides are mostly caring but, again, more oversight would be helpful - they are often the only visible staff. The laundry center is not well run, personal clothing is regularly misplaced. There are some standout staff members and volunteers that definitely help energize the atmosphere for residents and visiting family members.

Page Rehab is good. I am satisfied with it. The staff members are OK. They have sitters who watch the residents. Since my husband is on pureed food, it doesn't always look good. However, it smells good, and tastes good, too. The dining area is always clean. In memory care, they have crafts and toss a ball. They also have a salon where they could get haircuts. I didn't see an exercise room, but my husband got physical therapy.

The rehabilitation side is pretty good for such a facility, but the skilled nursing was a big step down in staffing and care. Both sides tend to be sloppy with resident's belongings. Clothes were often left on floor of bathroom in a wad. Belongings were lost or carelessly tossed around. Once moved to skilled nursing, the staff were disengaged and rough in handling frail patients. Patients were left unattended on bed pans for long periods of time. Staff often disappeared from floor area and unavailable when needed. We had family there for hours each day. I can only imagine how poor the care would have been if no one checking in. We have suspicions of our family member being restrained without our knowledge. Not suitable interaction between staff and Hope Hospice care. Billing records are like something from years ago and are designed to allow for easy errors in over payment. Staff is non-responsive. Bright spot would be good service. Not so much great food, but they have an amazing gal that works hard to try and find a way to try and make something appealing to seniors that are losing their taste and desires to eat. Additionally, they have a few outstanding CNA and nurse or two on the rehab side. Unfortunately, their level of compassion and caring was totally absent on the skilled nursing side.

Page Rehabilitation and Healthcare Center is not a very good place. They are not very thorough. My mom had her money stolen, so I am not very happy with them. Their care is below average. The food is OK. They have different activities like bingo, singing, church, and a reading group.

My mother was a resident at Page. I think Page is a good facility if your loved one is semi-sufficient, able to speak out when things are wrong and dementia does not have a firm grip. My mother was 92, very frail and deeply in the final stages of dementia. Therefore my experience with Page was not great. The staff for the most part are baby sitters and people movers, not terribly skilled in working with patients or actively engaging with patients. They are careless with the residents personal belongings, my mother's clothing, glasses, shoes, blankets, etc were constantly lost and it was up to me to try to find them. The family meeting with the administrative staff and the family is really nothing more than paper work to ensure they are being paid, no real information on progress, no advice on how to make the experience better, etc. I toured many facilities and selected Page based on the number of staff members I saw working and what appeared to be a caring community, in hindsight I think I could have done much better with other options in the area for my mothers situation. During her 2 month stay, I never had the opportunity to speak with a Provider that could explain why my mother still had pneumonia after 2 months at their facility. I was always told "she is receiving the care she needs". I also never had a discussion with any clinical staff regarding medication changes, only that they were changing her medications. My mother passed away after 2 months at Page, I still have no real information about what happened on her final day.

The staff at Page Rehabilitation has been fine with me so far. Every time there has been any kind of just a pain or little accident, they will call me right away and tell me of what happened and what they are doing about it. The apartments are up to date and clean. The dining room is nice. They have been good with me, and I am happy with them for now.

My mother was at Page Rehabilitation and Healthcare Center, and it was a good experience for us because we were with her the whole time. And they knew that. They were very well aware that somebody would be there by 7 A.M and that we left late in the evening. We would recommend it, but there were also things that went on in there. But we were with my mom, and we did not let her out of our sight, and everything has worked out really well. There were a lot of people who were very good, and there was a lot of lazy employees there as well. All in all, if my mom would have to stay in a place long-term, she said that she would go back there. And then, she was in a room with a lady who'd been in there long term, so that made us feel better because we had that woman watch after my mom. So as far as my mom's standard of care and what she had experienced in there, it was good. They assured us that if she had to come back, she could come back and to just give them a call. Maria -- she was the cleaning girl -- lifted your spirits no matter where you were. She came in and talked to mom, cleaned her room, and was just very delightful. A man named Rick -- the case worker who's on call 24 hours a day in case of elderly abuse -- was wonderful. He even told us aside, that if we had a problem with any one in there to call him and he would personally take care of them. He even gave us his number personally, and I believed him. The head personnel was -- at first -- unfriendly, and then she was very good. There were some nurses that took care of my mother, and they were just very good.

The staff at Page Rehabilitation & Healthcare Center were professional, they took good care of the residents and had a lot of patience. They helped the family and supported the family as well. I liked the staff above all and their work schedule was different and they gave reports to each other.
