I am a resident of Heritage Park Nursing Center. I love the staff; they are very professional. It's an excellent facility and they have excellent food. They did a great job taking care of me. If I need to be in skilled nursing, if I could, I would just stay here. I love it so much. The building is two stories, and I think there are 75 patients in the building. We have nice rooms, and just great staff, great nurses, CNAs, LVNs, and RNs. There's nothing I didn't like about it, it's just that I don't need to be in skilled nursing anymore. There's no pool here, but they do have a hair salon and a rehab center which is fairly large, for exercise and rehabilitation. Depending on your dietary needs they have nourishment or snacks twice a day. Not every patient has the same doctor, but doctors come regularly. They have a very nice courtyard for those that are capable to go in their wheelchair and go outside. I'm very picky about food, and they provide a lot of alternatives if you don't want what they're going to serve. I have Social Security, and they pay my share of the costs, and the state takes care of the rest. But as far as I'm concerned, there is no question that I got the value for the money.

My mother was admitted after a minor surgery on her foot and being very deconditioned. The doctor told me she could walk on her foot in a few days. When I found out they were not letting her do full PT, I asked why. She cannot stand on her foot. Yes she can. They needed an order from the doctor. 3 weeks delay and then another 2 weeks delay for a special shoe she needed and now she can do PT. Well, that would be nice, but it was time for her to go. WHAT?? No therapy with actually walking on her foot now?? Appealed with Medicare and won, but it only got her another couple of days. Now, I am being charged for the "extra" days past what Medicare covers. If the doctor's approval would have come sooner, she would have been out of there before any charges were incurred. I understand no visitors, I understand needing the doctor's approval, but 3 weeks delay in getting it and now charge me for the extra days?? Staff were ok. Food this time was a bit better than before when she was there. The place is old and the equipment really needs replacing. Tiny TVs so far away that the elderly cannot see them and the sound competes with the neighbor's TV. The PT is pretty decent. The therapists were very nice and I think if she would have been able to use both legs early on, she would have improved. Although, you do have to tell them to have the patient ready for PT when it is time or the therapist getting them ready takes the time away from their PT session. My mom was getting 20 min of therapy because they were helping her get dressed and ready. You have to be an advocate for your loved one. Also, the social worker needs some training on how to work with people and how to care for Medicare patients. He gave me wrong information about appealing and almost didn't go through because I missed the deadline. Luckily Livanta was very willing to listen to my story and agreed with me when they called me with the results of winning the appeal. Other than good PT therapists, not a fan of the place. The 2 stars are for the therapists.

The care that my grandmother got at Heritage Park Nursing Center was good. She had therapy there which was what she needed after a stroke. They got her back a little bit mobile and walking, so she had a good experience. Her room was kept clean, they were fine, though it's not an upgraded place. It's not a new place, it's an old facility, but they kept it together. The staff did what they had to do; some days were probably better than others. They had like an outdoor patio and a dining room for some people that could walk there, and they also had a common area with a TV. They have a gym down there where they would do the rehab. With regards to the meals, it depended, and there were some things she didn't like so we would take her meals. She had pretty attentive people.

My father-in-law was admitted to Heritage Park (HP) on March 10, 2020. He was discharged from San Antonio Hospital in Upland after undergoing a partial hip replacement after falling and breaking his upper femur. 3 days later visitors were banned due to Covid-19. Two weeks after he was admitted, my sister-in-law met with his care team, including a doctor. He had not even started physical therapy! His care team was non-existent. He was not given any medication for pain until we inquired about this. They then started him on Oxycontin, and he was drugged up for the next 30 days! He was incoherent, could not carry on a conversation, and in fact would fall asleep in the middle of a phone call. The staff is a joke, and is constantly redirecting to someone else who is never available to answer questions. They discharged him when Medicare ran out, not letting us know that he was eligible for a medical transport to another facility. He could not stand on his own, could not transfer to a toilet, could not do anything for himself, and yet they threw him out, with half a tank of oxygen for his COPD. Do not use this place if you have other options. Terrible treatment of patients. By the way, now that he is off the Oxycontin, he is regaining his wits and can carry on a conversation.

My mom is living in Heritage Park Nursing Center. The service has a couple of lapses but it's fine. The staff is friendly. Sometimes the food on the weekends is not as good as during the week. For activities, they bring animals over, so thats good.

I went to go visit my Uncle this afternoon at the Heritage Assisted Living. Really nice, my uncles room was a pretty good Size and had a refrigerator microwave. Nice bed, tv , dresser and desk. He looked really good . He looked like he is well Taken of. I felt pretty comfortable there. He said the staff is treating him well.

My husband was recently sent there from the hospital to get stronger through PT and to have a severe bedsore that developed while in the hospital taken care of. He also is a heart patient with a defibrillator/pacemaker. He was supposed to get meds twice daily along with having his wound kept clean and changed daily along with being moved from side to side several times daily. I visit him from 1-4 every day, and there is never anyone who comes in to check on him. He is given a pitcher of water that is warm by the middle of the day, food is left on his stand whether he's awake or asleep, and I empty his urinal while there or it would be overflowing. He has missed his meds the last 2 days because quote "the pharmacy hasn't delivered them". Does one wait till they run out of medicine before ordering more? I am extremely disappointed in this facility and the staff and would never give them any kind of recommendation.

My father was at this facility and he got the best care from one of the CNAs she was always smiling and willing to go above and beyond to make sure he was comfortable thank you Ilse R.

We’re moving my mom this Saturday to Heritage Park. It seemed very nice, good, and clean. My mom really liked the visibility of doctors and nurses. They actually have a rehab center right on the premises, so if they ever fell, they have people that take care of them right then and there. We had a nice experience having lunch there, and the food was good. They have a salad bar every day during lunch, then convert it into a sandwich bar at dinner. They had a wide calendar of activities, transportation, games, puzzles and people that read from the newspapers in the morning. It’s a fairly large room with a little kitchenette and nice view.

We had to move my father to skilled nursing right now. He is at Heritage Park. I’ve been there twice at separate times and it’s been OK. They do a good job. It's very clean and well run. My dad complains a lot but nothing out of the normal. It’s a small community, probably 80's or 90's architecture. Staffing as far as my sister said, is better than the last place he’s been in. So there’s enough staff there since it’s a small place. They don’t have to run around as much for a lot of people. Their ratio of staff to patients is higher. It feels traditionally like a skilled nursing home, not too homey but its OK. Overall it’s a good facility.

I had a friend who used to live at Heritage Court long time and since then it has been remodeled and everything is really nice. When I went there for dinner before for my friend, their rooms are double rooms, but now they are all single rooms and each of them has their own microwave and refrigerator which is really nice and it's fairly spacious. My mother-in law is there right now and she has a nice view out of the window where she sees some trees and grass. She also likes to be outside and they have a really nice patio with flowers and a fountain. They have a really huge living room with a huge TV and they do activities there and they have another activity room. They have a lot of activities. She likes their Bible study and they also have church activities on Sunday. They also have trivia, contest or games. The food is good and she likes the food. The people that worked there seemed competent and nice. They are very welcoming to visitors. We think that the care she is getting is good. The residents are also friendly and they were pretty much mentally alert. The nice thing about the facility also is that they have a nursing facility next door. One time she broke her hip and she went into their nursing facility and after that she went back to their assisted living facility so that was an easy transition. Their cost was also reasonable compared with the other facilities that we've looked at. The place is very clean and nicely decorated. They have a good air conditioning system. Overall, we've been very pleased with it.

My grandmother lived here for about fifteen months with a varied experience. The facilities are very nice with a fair amount of activities planned in order to encourage the active involvement of all residents. However, my grandmother was stand-offish, though never rude, which limited her involvement in many of these events. Most of the staff were fantastic to interact with though there were a few receptionists and nurses that could be very cold to my family and I, along with other visitors. In the grand scheme of things a few rude individuals should not matter much but with how much you are spending for a loved one to live here some basic courtesy should be expected. Ultimately, my grandmother developed serious health issues that took her in and out of the hospital, during which the staff and other residents were very sympathetic of. After enough time we had to relocate her to another facility that offered greater personalized medical care: in moving her out we faced some more rudeness from staff, nothing mean but they were not helpful in any substantial way, so our final impression was not positive. Beautiful location with some truly great people, living and working there, but for my family the few negative individuals really left a bad taste in our mouths.
