Do not send a loved one here unless you want them drugged into a soporific state for the benefit of Country Villa management. We found my brother in a wheel chair slumped and drooling. His regular medications supplemented with sedatives, antihistamines and extra antipsychotics with contraindications. They denied changing his regime, while refusing the advice of his regular medical providers. He was in a wheel chair because he was dizzy and had fallen several times most likely from over medication. Within days of removal he could walk and converse at his normal level. The daily care staff are fine. The management, administration and doctors are managing for their own needs, unethical at best and I believe criminally negligent

I have personally stayed at Country Villa San Rafael, and throughout my stay I have been treated like a "king." Everything I asked for or needed was taken care of very quickly and effeciently. I have been to other facilities before, and nothing can compare to the quality at Country Villa. This facility has excellent regarding nursing and CNA care and is remarkably clean and well taken care of. If I ever knew of anyone that needed or required temporary or permanent care I would highly recommend this facility.

I had my great aunt reside here in this facility after she had a very serious and stroke. She ended up being a hospice patient and expired in the facility. I feel that the staff was outstanding during that time giving my aunt the great care that she deserved and needed. I am very thankful that I chose Country Villa San Rafael and strongly recommend this facility to others that want exceptional and compassionate care.

My 94 year old mother was transferred to Country Villa, San Rafael after a two week stay in the hospital primarily for rehab. This facility is very well managed. It is clean and cheerful. Both the Physical and Occupational therapy departments practice a very aggressive yet caring program. The "TEAM" therapist are very well organized and communicative among themselves, the residents and family members. Many thanks to Patrick (PT) and Judy (OT) for your "TLC" extended to my Mom and for your support to me! The interactive and all inclusive activities program would get TEN stars if that were possible. Special kudos to both Maggie & Alberta for the wonderful work that you do on a daily basis. Your passion and affection you display is so apparent in the eyes of the residents. Keep up the great job that you do! My further observations revealed just a few short comings and could use some improvement. Communication among some of the administrative & supervisory staff to family members is a bit remiss. There were couple of scheduling of appointments that were "mis-handled", but later resolved. It wasn't until the day of my mother's discharge that I learned that the Director of Nursing was even remotely involved with her care. (Mom was being followed by her own Primary Care Doctor). It also appeared to me that the primary role of the nurses was to administer meds to the residents. There were several occasions where a resident would ask a nurse for assistance and rather than take a minute to wheel them to their destination, he or she would either call for an aide or refer them to one. The RNA's and the CNA's are the ones who actually do the majority of the physical tasks and should ALL be commended for their diligence and professionalism in which they perform their duties. Believe me, without them, there wouldn't be a "Country Villa San Rafael" to review. Based on my experience, it is my intention to refer anyone in need of a Skilled Nursing Facility here in Marin to CVSR! My mother has been a resident at Country Villa for over 15 months. There were multiple issues in the beginning of my mother's stay, but through constant diligence of the medical and support staff she has become a lot calmer and appears very comfortable. I am very greatful for all the effort that the wonderful staff at Country Villa provides daily for my mother. Country Villa is located in a beautiful setting and the facility is clean,airy, and the food is nutritionally balanced. All together this is a fabulous facility.

I have my Uncle here at the facility and has been here for almost 4 years. One honestly great thing I can say about the facility is the communication between the staff and myself. Also this facility is located very close to my home. The facility is very clean and smells good. I also think that the level of care my uncle recieves is very very good. Especially I really like the CNA's that take care of my uncle.

I get a good feeling when walking into the facility. Staff very professional and very caring in taking care of my husband. Is nice that the administrator is around and checking in on things. DON, and assistant and activities have been very good to me and my husband. I feel comfortable with the facility. One quick suggestion is occasionally by the time the food tray gets to my husband it is at times cold. Sometimes communication could be better between nurses and Cnas, however for the most part the level of care has been excellent. Communication is a big part of care as well. My wishlist would be to have automatic handicap doors especially by back patio and to be careful with the slope.

A good friend of mine has been a patient at Country Villa San Rafael for over 13 years. He has a degenerative muscle disease and can only control several fingers on his left hand. Last night at midnight he used those fingers on a cell phone to speed dial my number. He was having serious trouble breathing, was in great pain. He apologized for waking me but pleaded for my help, asking me to call the facility's front desk and tell them to send a nurse to his room. He had turned on his call light at 11:15 PM and was no one had responded for more than 45 minutes. The woman who answered the phone hung up on me. I called back and read her the riot act demanding medical help for my friend. My wife called several minutes later and she was hung up on too. She then called 911 and and when the emergency techs arrived, they took him to the nearest emergency room where he was evaluated and admitted with suspected pneumonia and possibly an intestinal blockage . What makes this worse is a similar incident took place just 3 weeks ago, the first time my friend had to call me for help because no one responded to his crisis. After I filed a complaint with the county ombudsman, the facility director called me to apologize profusely and promise it would never happen again. The promise lasted 20 days. If you love someone... keep them out of this place if you have any other reasonable options. If you don't have options... you cannot trust this place and need to be constantly vigilant. My friend could have died from this incident. I'm now looking into the laws regarding criminal negligence.
