My husband is at San Pablo Healthcare And Wellness Center. The people I have spoken with seemed very helpful. I can't go in, so I don't know what anything looks like inside. The outside is not good. The part facing the street is fine, but the sides need work. There is a garden in the front. They have activities, but he is in the red zone with Covid, so he can't mingle with anyone yet. With his dementia, he does not join in activities. He does not last long, he only lasts a matter of minutes, and he walks away and onto something else. His focus is just not there. He seems to be progressing, so they must be doing something right even though I cannot physically see it. The staff has been attentive, helpful, and they call me back if I can't speak to them on the first call.

My mom moved into San Pablo Healthcare & Wellness Center. The staff is great. The facility is just older and rundown. It's three people in one room. They serve three meals a day. The same stuff that was on the floor by Monday is still on the floor, so it's not good. Right now, it's being paid for by insurance.

My father was a resident at this facility for several months, and I absolutely would not recommend this facility to anyone. Care: Most of the CNAs at this facility were nice people. However, there were a few bad apples, and I would never want anyone to be subject to them. In fact, one of the other residents told me, "Watch out for (name protected), she's nice to your face, but she's mean." When my father was under the care of this particular night shift CNA, I would come to visit in the morning and I found my father drenched in urine. She would simply shrug that he didn't want her to change him. Really? Every single night for weeks on end? The other night shift CNAs were able to take care of my father just fine. Additionally, these CNAs appeared to do the minimum required amount of care. I would show up in the morning to find that my father had slept all night in a wheelchair, out in the hallway, instead of in his bed. And covered in drool. From the amount of drool on his shirt, it was obvious that he had been sitting there all night, and no one even bothered to put a towel under his chin, or wipe the drool away. My father also got a got on the side of his arm because the CNAs would leave him in his wheelchair for hours unattended, and didn't properly protect his body from arm on the wheelchair. Cleanliness: This facility usually smelled awful, which is a sure sign that the facility is not being cleaned properly. The bathrooms in the residents' rooms were filthy. They often lacked soap, gloves, and paper towels. I understand that it is difficult for the maintenance staff to clean every single bathroom every single day, but that's a poor excuse. The facility should be able to provide clean bathrooms for their residents. Billing: This facility will try to squeeze every penny they can from residents and their families. If improper care and lack of cleanliness wasn't enough to make you run away from this facility, inaccurate, scammy billing should convince you. My father was discharged over a year ago, and we just received a bill for $800 from the facility. The bill says that the insurance company refused to pay the bill, and when we inquired with the insurance company, it turned out that the bill was never even filed. Additionally, the bill lists a "specialty bed" as one of the items that my father supposedly received. He never received a special bed--he was given the same crappy bed that all the other residents received, and when we inquired about the item listed as "specialty bed", we were told that they list other equipment under "specialty bed" and could not tell us specifically what the charge was for. I am considering this fraudulent activity. After receiving the bill, we immediately called the facility and asked for a more detailed bill. They agreed to send it over, but after two weeks, they still have not sent it. However, they did send us a notice saying that they want the money within 7 days. Do not send your loved ones to this facility. It is run very poorly, their billing tactics are fraudulent, and your loved ones will not be taken care of.

They're really good over at San Pablo Healthcare Center. Everything is good. I have no complaints with them. They do good with the care and the food. The rooms are nice; they're spacious. Sanitation was good also. I like the friendly staff.

I currently reside here. After being here for a few months I can honestly say that the overall care here is good. Most of the staff are not Anglo but the care more than makes up for the small language barrier.Thyey are kind and attentive. When I came in I had bedsores, they new what to do and I healed up. I am long-term and will give updates. I like it here.

This is not a skilled nursing facility. After open heart surgery my mother went for Rehabilitation. She was to be walked every day and to shower everyday. I came 3 times a day at different times. They did not recognize my mom was going into congestive heart failure. Something they were supposed to be looking for, especially in an Open Heart patient. I brought it to their attention, but nothing was done. Until my mom was so sick drowning in her fluid, sodium level dangerously low and she slipped and fell. It took them 2 hours to let me know. I went there as soon as I got the call. I had to get them to call the ambulance and the Charge Nurse that was on duty at the time of the fall. Was gone. I almost lost her. Horrible care.

I went to visit my Aunty after her openheart surgery. Her care there was awful . The response time from the staff was slow. She had a bad fall and the staff took to long to let her daughter know what and why she fell. The noise level was bad. I would never stay or put anyone at this center.

For a loved one trying to support a resident long distance, my ease of access to the resident, (a medical necessity for him) has been extremely frustrating for the last 4 weeks.
