Pacific Heights Transitional Care Facility must be one of the worst “skilled” nursing facilities in the city. I have been a patient here for multiple years. For my entire stay, I have been in multiple rooms with extremely loud, screaming dementia patients. Try sleeping wearing expensive noise cancelling headphones that can’t keep out the screeching. Now, imagine trying to recover to get out of here while unable to get a decent night’s sleep. Food is disgusting & even looks & smells awful. I was nauseous everyday until I started using various outside & expensive food options. They don’t care about the patients here. I constantly hear how bad the quality of the food is. For the 1st 4 months my hair wasn’t brushed or washed not were my teeth brushed or was I even offered a toothbrush. Because of more negligence, I have paid out of pocket thousands of dollars to have teeth extracted. The vile food is included so don’t expect a discount for not eating it. Complaints are rarely taken seriously. If you or a family member or friend needs a SNF, please do your research & avoid this place like the plague. They are constantly understaffed & working as a CNA or nurse are hard work. Staff suffers and so do patients.

Pacific Heights Transitional Care Center is clean and the staff seems very professional. My sister moved there and it's very close to my house, which is one of the reasons why I like it. It has its drawbacks, but I am beginning to think that this is probably what most facilities and most reputable facilities are.

Overall stay exceeded expectations. My wife got excellent personal care every day which was very consistent in personal hygiene, medications and therapy. My wife's stay was very comfortable and accommodating and although she has a major femur fracture the overall atmosphere made it possible for a very relaxing stay and progress to an extent where she could be comfortable to go home feeling confident that she can recover in the nearest future. There are particular staff who from the get go provided support and helped us make the right decisions these staff members include Rich and Rena from Social Services and special thanks to a charge nurse on the 2nd floor-TJ and CNA-Merlita. The entire management and staff should receive recognition for my wife's stay. I would recommend Pacific Heights to family and friends because the personalized care and attention I have seen is one of the best in the industry. I wish Pacific Heights all the best with their nursing and care for the future. Thank you Barbara and Dominic.

My father was admitted after a stroke and he was unable to speak and his right side was paralyzed. He received great care from the rehab department and nursing staff. He had 3 specialists coming in daily from OT, PT and ST to help him rehabilitate. They are all very professional and caring people. He is now getting ready to return home and will be able to live in the community with a helper. Social workers and nurses were immediately available to assist when needed. Thank you very much to the Pacific Heights team.

Earlier this year my loved one was a patient here. I was so impressed by the caring and attentiveness of the staff. The aides (helper people) answered call lights quickly, and the nurses were able to answer all my questions about the medication my mom was taking. They never seemed bothered or rushed, even when I had multiple questions. The discharge planning people- specifically the case worker and the social worker- made a confusing and stressful time more simple, I liked how they explained things in an easy to understand way. I wish there was better parking and the food wasn’t very good (according to mom) but those things don’t matter when the staff was so good. All in all I couldn’t have asked for a better place to care for my mom as she recovered.

Everyone on my floor that helped in my recovery was like a person that I already knew. All were friendly and professional. I'm glad that I chose Kindred. My recovery time was less than I expected.

Our 100 year old grandmother is well taken care of at the facility and they always call us if they notice any changes. We are well informed of her care and we we know she is safe there, and it gives us a lot of peace.

They took care of my grandfather very well and treated him with dignity. He was a fall risk and ensured his safety at all times.

we transferred our dad from another facility and it was the best decision we ever made. Everybody from this facility's very helpful, from the director, the nurses and the CNA's. They are very flexible with regards to our request and they make sure all communication lines are open. I would definitely recommend this facility because of their excellent service

My Dad was in this facility for 8 months and am pleased to say that the staff was incredible! They were thorough, professional and sensitive to his needs. Praise goes out to Abbey, Roberto, every staff member on his floor and Alissa, who went above and beyond to make sure his transition home was seamless! Thank You!

My clients are placed here for long term and they are very happy in this facility. Staff has a good and caring manner about them. There are activities that keep my clients entertained. I would like to recognize nursing staff for their leadership and their impeccably high standard of care, delivered with warmth in such a caring environment. The nursing staff is very attentive, caring and professional. I like to mention one CNA, she went above and beyond the call of duty. She treats my 94 yrs old client exactly like her own mother. My client is very happy and doing much better because of her care. I would especially like to recognize all the social workers. Especially Social worker Director, she is truly a good hearted person and so are the many staff members that work at this facility. If you want your loved one to be cared for like a family member then this is the right place for them.
