I used to visit Boulder Creek Post Acute. The community was in different stages, like they have people in different phases, but the activities department there was outstanding. They are very good with the patients there and they are very engaged with the people they are working with, as well as the physical therapists there. There was this one gentleman, I don't really know his name, he was terrific. My cousin's mother-in-law, at one time, she was bedridden, and he would come in and feed her and things. I think that facility was very good with regards to that. In my experience they are very accommodating with the people. The place was clean. It seemed to be properly maintained. Of course these facilities would have a scent to it, because it is a place where they have a lot of sick people. But from my point of view it was pretty good for the type of facility it is. I know that these patients are sometimes very difficult and demanding if they don't get their medications on time. But as far as looking after the patients, they're always coming into the room at the right time, making sure they are fed and bathe. That was one of the things I noticed. Especially with my cousin's mother-in-law. She was not able to get up, but she was clean all the time, and every time she had a problem they always took care of it. I think the facility is very well put together.

I lived at Boulder Creek for 9 weeks in 2024. As for the nursing staff, a few of the nurses are wonderful. These few listen to patients, respect each individual and address each persons needs, but the majority of them do not. For three days I went to the nurses desk saying I did not feel well, so they gave me otc cough syrup. They did not advise doctor or investigate my complaints at all. Because I felt so awful my friend took me to Urgent Care where they sent me to ER. I had pneumonia. Upon my return to facility I had an armful of prescriptions and stopped at nurses desk. They were mad at me for going to ER saying I should have come to them first. Turns out I had an antibiotic resistant bacteria which was discovered after I left facility. This bacteria was a strain that also gave me UTI and is typically found in nursing home facilities and spread by the staff in unsanitary conditions. The doctor there is an embarassment to the medical profession. She took all of 2 seconds to listen to my chest after I had returned and said the ER diagnosis after reading xrays was wrong. She said I only had bronchitis then she left as if I was a bother. Another thing about this house doctor, she will only approve short term disability for two weeks tops, saying people at Boulder Creek have everything paid for and do not need to collect disability. This doctor is so wrong and should be fired. This sickness spread all through this facility affecting patients and staff. Before this illness was finished, another bad outbreak appeared causing severe vomiting and fevers throughout facility. This is far from acceptable. There is a high turnover rate of CNAs with the longer employed CNAs being disrespectful, mean and rude to patients. They intentionally corner patients and incite rage by pushing their buttons. They call out in the halls telling people to change diapers which humiliates them, or often they all disappear at the same time leaving nobody to respond or help the patients when they call. They ignore when call buttons are pushed for very, very long times so it feels like no help will ever come and this feeds patient depression. Some of them are just simply mean. They have a non caring attitude that echoes through the halls. I liked the Activity people as they try very hard at this facility. There are daily activities planned with great attitudes and even schedule time to do manicures and include all the patients. This facility will not improve care for its patients until authorities get involved. Nobody knows what goes on here and nothing will get better unless people who have stayed here say something.

I am very disappointed in this facility. My husband went there as a Post acute patient primarily for physical therapy. Some of the physical therapists are good. Some of them are not. Some of them are bad. He's had several that refused to get him out of bed even though he was out of bed the day before with a different therapist. They claim they don't know how. One therapist insisted that he sit on the side of his bed and do bed exercises. The problem is that he had one of those no pressure air mattresses on his bed and she didn't know how to turn it off. So, while he was sitting on the edge of the bed, it kept pushing him farther and farther. He kept telling her that he was sliding off the bed and she kept insisting that he keep doing his exercises. He ended up with nothing on the bed but his back. His legs and hips were completely off the bed. He managed to put himself back up with no help from the physical therapist! He could easily have fallen and injured himself! I don't believe that this person even was a physical therapist! She was completely incompetent and appeared to be untrained. He has frequently had staff talking in other languages in front of him so that he does not know what they are going to do. Are they having private conversations while they're supposed to be working with him? Or are they just being plain rude? Either way it's not okay! The food is terrible! I haven't had the misfortune of tasting it but based on the way it looks it is very very institutionalized, cheap, and not all that nutritious. His first night there he had a little bit of pasta with some Polska kielbasa in it. It didn't even have cheese! It looked nasty and it was the tiniest little serving - something I would expect to see in an early elementary School cafeteria. I have not seen a sign posting the information about the ombudsman which is required by law. It may be hidden somewhere but certainly not in the unit where my husband is. I sent a message to the administrator and I'm waiting to hear back. The facility does not smell bad, it is adequately air conditioned and heated, and appears clean. But, other than the front entrance, the place is dingy and very, very dated. For the last 2 weeks my husband has been in a bed that is too short for him and too high for him to get out of on his own - or even with the help of a PT. His chair is too low so he can't get out of it without having at least two strong people helping him. All he needs is a cushion and they can't even give him that! They keep telling him that he's going to get the cushion, a lower bed, and a trapeze so that he can pull himself up to the top of the bed and not get bed sores on his heels from the footboard. But they never carry through 7

My father is in Boulder Creek Post Acute. It's very clean, it's nice when you come in the hallway, and they have a nice courtyard outside. The room doesn't smell, but the furniture is a bit outdated, and could be updated. The staff seems relaxed and friendly, and the residents seem well taken care of. It seems like he's getting a good variety of food, his dietician works with us on trying to get him alternate choices, and that's good.

I chose Villa Monte Vista Nursing for my husband because my insurance would cover this nursing home and it was the closest to my home. I felt comfortable with this facility, and the people were very nice. The rooms were a little tight because there were walkers and wheelchairs, and the food was terrible, but they have something going on each day. They have a rehabilitation facility, and that is also the reason why he was there, to get some rehab.
