My very dear, very elderly husband was at Catalina for five weeks before he died. Not a fancy place, but I was deeply impressed with the care and attention he received, the respect, and the professional attention. All the staff were exceptionally polite as well as skilled and conscientious. He was wheeled to the therapy room and helped to work on some of the machines, which encouraged him to eat well and try to grow stronger. . .I asked to speak to the cook one day, as the food was surprisingly good. It certainly helped him that our dog could visit. Thank you, Catalina, I hold you in my heart with gratitude.

First of all if you want the best care for your loved ones, this is not the place! My loved one was there for 3 horrible weeks. From the very first night we arrived, the place had a very strong odor of urine. There was absolutely no staff in sight. The main entrance door doesn't even open correctly & we along with the transportation driver didn't even know where we were supposed to go? Then when a staff member finally came by, they were very unwelcoming. I say this because we were ignored. When we're finally placed in a room, it was unacceptable. It was a 2 person room, already there was someone in their but this poor resident had to maneuvered in every which way possible just so my loved one can get to their bed! The rooms are way too small. Then we waited 3hrs (9p.m.-11p.m.) for someone to come talk to us. When a staff member finally did they told us there's no administration & nothing would be done until tomorrow! The next day they only checked on us twice! No matter if the call light was pressed, we were told "regardless of the light being on, the patient gets checked every 2 hours." So my loved one had to lie in their mess?! They only showered her twice because everytime she was supposedly scheduled "something came up." The staff was very unprofessional & uncaring. They showed no concern when all this was addressed in our care conference. They were supposed to be doing physical, occupational & speech therapy Mon-Fri. This didn't happen daily. Whenever a question was asked, they never answered or would reply "I'm not too sure but let me find out." Never got any clarification on anything. The only phone number listed doesn't work. They give u their business cards with also non working numbers. They said "the issue is being worked on." We weren't the only families that had concerns about our loved ones being left their overnight. This all continues until about a week ago when my loved one ends up back in ICU. We will not be sending our loved one back to such an unpleasant place for any reason whatsoever.

Beware of this facility my Step Dad was taken there for nutrition and physical therapy the night he was taken there he was very agitated and they had no medication for him till early in the morning the place needs to be revamped the rooms are over crowded they need better trained staff my Dad kept falling out of bed getting open wounds only to be tossed back in bed without cleaning the wounds or dressing them meds were not given on time left in a dirty diaper for hours food trays shoved at them knowing they cant feed themself call lights not answered and the way they talk to residents is aweful to say the least! My Dad said the CNA hit him on his last night there because he fell out of bed again and he had marks on his back! I dont think that this facility is trained to take care and recognize people who need acute care! my Mother is so distraught about taking our Dad there because he passed away 2 days after leaving that place now we are concerned for her health after all this

My dad was a recent patient and I have never experienced such shotty, pathetic nursing in my life. It took hours for them to answer the call button, no one knew what was going on with him, he was sent for speech therapy But never received it. The food is disgusting , the staff are rude, management has only excuses for their staff. I had to continually call the nursing director to get any kind of help. The nurse manager is extremely rude, asking my father to give me a message because I was bothering them too much . I wonder why? Do not send your family member there!!!!

My brother has been at Catalina Post Acute and Rehabilitation. The staff was friendly, I don't think they did their job, but they were nice. He was there for more than 10 days. They kept him there however long his insurance allowed him to be there, and then they released him. I don't believe he ever got a shower that whole time or any type of bathing. They didn't change his bedding, even though they told me that they did, but I could still see urine on it. It was just disgusting. The meals were the least of our problems there, the food was acceptable. His room wasn't the cleanest. My brother wasn't getting any type of treatment, even though they were telling me he was, and his roommate vouched for that.

Please do NOT take your loved one there! My cousin was there and would be left sitting in the hallway in a wheelchair for over 2 hours after therapy, in his fecal matter, in so much pain from the wheelchair he would sit in! He would use the call button for the nurse when he couldn't stand the pain he was in and they wouldn't respond for over 2 hours. No bath, not even a sponge bath. This place is horrible. I would try for hours to contact someone but they never answer their phone!

Terrible. The case worker here does not know her job or responsibilities. My sister requested many times to keep me informed of her health and progress and the case worker refused, without a valid reason. She even failed to show up to meet with us twice. The Administrator is an expert at passing the buck. You won't get any help from him either. The front doors are just left open most of the time and anyone from the street could just walk into the facility. The reception desk is usually unoccupied. Some of the hallways smell of urine. They tell you all the rooms have windows, but they dont tell you that all the beds by the windows are taken and your loved one will be put in a small space between two cutains. This place is disorganized and dysfunctional. Terrible.

By far the worst place to send love ones for care. The amount of neglect my grandmother received from this facility is unreal. Majority of the techs/nurses made her feel like she was a burden and made her feel very uncomfortable. One of the nights there my grandmother had a small accident and the night tech told her she wasn’t going to change her since “ it’s always one thing after other.” They left her like that for 6+ hours which caused her to get bed sores/rash. It’s pretty heart breaking that my grandmother had to call crying to the family saying how uncomfortable she was there. When she had to go back to UMC for a evaluation one of the Drs even noticed how much worse she got since being moved into there. Also they made documentation on how poorly her wound was being taken care of. This rehab needs a serious evaluation!

My brother is at Catalina Post-Acute Care and Rehabilitation. He moved in there 2-1/2 weeks ago. They provide physical therapy while he's there. When I was able to actually talk to somebody, the information was outstanding and the attention I got was outstanding, and I'd get to talk to his attending physician. The problem is they don't return your phone calls most of the time. Communication has been pretty bad, but the care has been good. The location is great. The OT and PT services are good. He said the food is pretty good.

DO NOT SEND YOUR LOVED ONE TO THIS PLACE!!! YOUR LOVED ONE COULD END UP DEAD!!! THEY LEAVE PEOPLE IN THEIR FECAL MATTER, THEY DONT CALL ABULANCE WHEN YOUR LOVED ONE NEEDS IT, THEY GIVE YOUR LOVED ONE TOO MUCH OXYGEN, THEY LEAVE YOUR LOVED ONE AT THE DOCTORS FOR HOURS ON END WITHOUT TRANSPORTATION BACK. THIS PLACE NEEDS TO BE SHUT DOWN ASAP!!! May my loved one RIP 🙏

I wouldn’t even give a star if I could this place isn’t worth it I have my father here the rooms are disgusting you call the facility and their phones are down every single day and every single day of the time I would not recommend this place

Do NOT allow your loved one to be cared for here. Times are difficult so I wasn’t able to actually see my dad for the two and a half weeks he was in there. He had overcome double pneumonia but needed help to regain his strength. Come to find out that PT amounted to someone coming to his room each day and having him do a few ridiculous exercises. And then, all of a sudden, they said he was being discharged back to his independent living apartment. Because I hadn’t been able to actually see him, I had to trust that they knew best. Something seemed fishy because they wouldn’t even let me speak to dad at his window. During the time he was there, we rarely were able to even have a phone conversation with him. Either no one answered the phone numbers we’d been given anywhere in the facility or the phone in his room wasn’t working properly. When we did speak to a staff member who should have been an advocate for dad, we got very little satisfaction. We never knew who dad’s doctor was or exactly how much PT he was getting. Even dad could not get a straight answer out if anyone. It was so frustrating because I felt like our hands were tied. There was one receptionist who, if we were lucky enough to get her, was very nice and would go to dad’s room, dial my number, and make sure the phone’s volume was turned up before she put dad on. But that didn’t happen consistently. I could go on and on about how bad this place is but it would really be enough to say that if you want more than glorified babysitters for someone you care about that needs “skilled nursing” and physical therapy, this is not the place. Please find a place that knows what they’re doing and hope their insurance is onboard with your decision.

Right Hand doesn't know what the left hand is doing. Staff was nice but have no idea what they are doing. My wife had PT for 1/2 hr each day, then lay in bed till next day. She was making progress till they stopped PT for 3 days. She lost all she progress. After those 3days they gave her and IV. She asked what it was & why. I don't know! I asked the same ? Same answer. I picked her up, can't stand, feet swollen. WORSE THEN WHEN SEE WENT IN!

I was a patient at Catalina for almost a month. All of the staff from the nurses and CNAs to the therapy, kitchen , housekeeping and administration staff were extremely nice and accommodating to my needs. My stay was very pleasant and I would recommend this facility to others.

My stay at Catalina was very welcoming and felt like home. All the emploees were friendly and alert. They were on top of everything I needed. I enjoyed the therapy room which helped me get my strength where I need it to be to move on with my surgery. I recommend this facility to anyone who needs the therapy. Thank you again Catalina!

When we visited the establishment, it was in order to visit my new step-father's aging mother. We walked in and I remember being immediately surprised to how nice the center was. For starters, it did not have that smell that I heard of many nursing homes of having, so that was definitely appreciated and a good sign, and then secondly, I thought the furniture throughout was classic and great quality. We sat with her at a nice table that easily pulled up 4 additional chairs and space for her wheelchair and sat and talked. Prior to meeting with her, we spoke with the staff and they were friendly, smiled often, and very helpful to our needs. When the day comes that my mother needs to go to a home, if I can't help her at my own home, I would suggest this center as both the staff seemed friendly, his mother was happy (even though she is in dementia), and the center was clean.

Would have wanted to see the Doctor visit schedule and the nurses and aides rotations posted on the floor. So much anxious time wasted hoping to "catch" one of them to get proper information.
