Assisted Living

A little assistance goes a long way. Avista offers Assisted Living residents a chance to maintain as much of their independence as possible, while accessing help in the areas they need. Caregivers are available 24/7 to help manage medications, assist with dress and grooming, and to provide companionship and friendship to our residents. We believe in the power and dignity of allowing our residents to do as much for themselves as possible and that is our goal. Our caregivers help to bridge the gaps that may arise with aging while providing in a home like setting. Just like home, our community offers activities and friends to help make each day productive and fun.

Memory Care

Avista Senior Living North Mountain has a secure, dedicated floor for our Memory Care residents. These wonderful friends who have lived very full lives now need some extra caring in a secured environment with specialized programming. Our beautifully remodeled Serenity Suites have lovely common areas, including a balcony adorned with fresh flowers that the residents help to care for. Meals are served family style and we have designed special spaces where families can come and spend time privately with their loved ones. You'll be amazed at all the detail that went into making this a safe, peaceful home for our residents. Shared or private accommodations are available.

Independent Living

Avista Senior Living offers peace of mind for seniors who are able to manage many of their own affairs, but enjoy a social life and the convenience of a caring community. Onsite laundry facilities and housekeeping services are available as well. Independent residents will find all of the comforts of home, as well as the opportunity to make lasting friendships with other residents and staff, when they choose to live at Avista.

I am a friend or relative of a current/past resident

I recently relocated my mom from another senior living facility in the Phoenix Valley, that was reprehensible. In fact, I lobbied to have there Director jailed for elderly neglect. Following this horrifying experience, I had little to no hope that I could ever find a place for my mom where she would be safe and respected. I was so wrong. The Avista Senior Living on S. Eva in Phoenix is a Godsend! The leader there, Amy, truly cares about every resident and knows them all by name. Her sidekick, Rob, has treated my mom with complete respect and honored her 90 years and all the experience that comes by living close to a century. In fact, the entire team there are people who care. From the housecleaning director to the front desk team members to the food service staff. I just spent three days there fixing up my mom's apartment and was able to meet so many staff members. I ate the same meals that the residents do and was very impressed with the quality of the food and the size of the portions. I have found, through my research, as an award-winning investigative journalist, that so many of the senior living facilities in the Valley are nothing more than a money-grabbing racket. It breaks my heart to see our seniors treated only as dollar signs by these big companies. Not so with Avista Senior Living! I thank God for all the team there and I can now rest assured that my mom will be treated with respect, honor, and as a loving human being, not a dollar sign. If you are unhappy where your loved one currently resides, I highly recommend you contact Rob at Avista. You will be thrilled you did, and your loved one will benefit I ways you didn't think were possible. My mom is now happy, content, joyful and calls Avista her "home." Thank you again, Amy, Rob and the entire team!

I visited this facility

My mom's going to be living at Avista Senior Living North Mountain. They have independent, assisted, and memory care. The best thing is that it's an apartment setup, so she still feels she has her own home. The least I like is that they encourage her to eat in the dining hall. I think that's a positive, but my mom is not that social. The staff was helpful and knowledgeable and they got back fairly quickly to you. We toured the one-bedroom apartment setup that they have. There's enough space for her to get around safely in a wheelchair. There is a mini kitchen. There's no stove, but she can easily get her snacks and things and we refrigerate them. The restroom is set up for safety with a walk-in closet and is also very spacious for the wheelchair to get around. It is homey like you're walking into an indoor apartment. There are visiting areas outside the apartment complex. If you want to have like a birthday cake or whatever, there's a special room for that as well. It has a coffee area. It's very welcoming when you walk in. There's music playing. There are people hanging out and walking around. They take weekly trips to Walgreens or Wal-Mart, so that's nearby. I saw their menu. They have items on their menu, but then they also have daily items that they always provide, like French fries are always going to be there if you don't want what's on the normal menu. The dining area is like a restaurant setup where there are tables and chairs. It's a big hall where everybody can sit and eat. They have a whole room for activities. They have a wall full of books. They have a room where they have activities, where you can sit for religious purposes, or they have bingo. Also, another benefit is that the family members can spend the night with their loved ones because it is an apartment setup and it's so personal. The staff is friendly and helpful. It's clean. It seems the residents feel very comfortable, like saying hi to you and greeting you and stuff like that, so I think that that in itself speaks volumes that they're comfortable in their setting.

We've chosen Avista Senior Living North Mountain for my mom. The apartment that we found for her was a good-sized apartment. It was very nice. The management there was very nice. The staff is amazing, according to my mother. And from what I've seen, they're amazing. According to my mom, they have excellent food. She hasn't been using very many amenities and facilities at this point other than using the laundry room. And that's been amazing. They do have a salon and an activities room. They do have activities that go out and stuff like that. So they have a lot of things and they have an activities calendar. The value itself is very expensive. It's hard to get something that's not expensive in value.

We chose Avista Senior Living North Mountain for my mom because they were set up like apartments, and she was more comfortable with it. It wasn't set up like a group home or a hospital ward, and it was also one of the cleanest places. She's only been there a couple of months now. I don't know all of them, but the staff that I communicated with was excellent. It's not the most beautiful facility, but the layout, like the apartment type concept, is what she loved.

I visited this facility

By far one of the nicest senior livings in the North Side. The marketing walked us through the while process she was so sweet and patient as I had many questions. Everyone seem friendly. The decorations in the room we were shown was so nice. As we walked by someone they would greet me. Their pricing was just a little bit above my budget. I am hoping to apply for altc and move in right after.

I am a friend or relative of a current/past resident

This place used to be nice. I had a friend who was there for over a year. But in the past year it has went down hill to unbelievably bad. My friend had a room by the back of the kitchen. They sprayed the kitchen for bugs and she got an unbelievable amount of roaches. They refused to spray her room and when she complained they evicted her. She used boric acid to kill them all. When she moved out she had over 500 roaches dead in a jar. It was so disgusting. How can they say they care when they would evict her rather than help her. Prior to this she also had a 5 week infestation of fruit flies. They did nothing to help on that too. Her case manager got her some fly strips. Over 1000 flies on her walls, ceiling, and furniture. This place should be shut down.

I visited this facility

I visited the Memory Care Unit of this facility and was not pleased with what I observed. It is understaffed, was not clean and smelled of urine and fecal matter. The residents did not appear to be happy and the clothing they wore either did not fit properly or was soiled. Patients are free to roam the bare hallways and enter any room they want so forget about privacy or protecting any personal items from theft. This could be because the names on the resident room doors is typed so small they can't read it. The family dining area was a mess and crowded with too many residents considering it wasn't even a meal time. I seeked out a staff person to ask questions but never found one. The security is poor especially at the elevators with not one person monitoring it. It won't take long before a resident watches a person type in the code on the keypad and remembers it so they can get out. I entered the building and went to the Memory Care Unit without ever signing in or being asked who I was and who I wanted to see. Any person could walk in from the street who does not have business being there which is a security problem you don't want happening to a loved one.

I visited this facility

I toured Avista facility. They are probably better on cost than all of the other ones, but it didn’t have the ambience. It wasn’t as nice, and it was too far away for us. It just didn’t appeal to me. The rooms were fine.

I am a friend or relative of a current/past resident

My parents moved in the middle of November because of issues related to my mom's Alzheimer's disease. By the first of November, she was in memory care, and she passed by Christmas. While there were a few hiccups when my parents moved in (no place is perfect and many of the staff were new), they were quickly resolved. Everyone there took such great care of my mom and have been a HUGE support to my dad during and after mom's passing. There are very few places that accept Medicaid and are affordable, that also do such a great job. I can't say enough about what a wonderful community this is and how hard the staff here works. Excellent.

My parents have moved in one month ago and have been very happy, the transition has been smooth due to all of the caring staff. The people living in this facility are all referred to as residents by the staff. This facility is small enough that every resident is know by their first name. Staff are very friendly and helpful.

I am a friend or relative of a resident

We're not too happy with Avista. There doesn't seem to be enough caregivers there. My friend has fallen twice and gotten hurt. It's not a very good place. The rooms are big enough but not clean every time we've come. Their work attitude is good; there are just not enough workers, so they're always stressed. They haven't really done anything with her. She's been too sick, and she's not eating at this point. They need more workers there.

North Mountain just seemed right for my mother-in-law, and it's a location that she's familiar with. The people are happy, and the people working there are doing a good job. It seemed they would be caring people. The dining area is clean, the lobby is nice, and some of the things have been updated. She is in a one-bedroom apartment, not luxurious, but nice and clean. We took it because it wasn't as expensive as some of the other places. It's not a real big place. For the money, it's very nice.

I am a friend or relative of resident

What I was told about Avista and what actually happened were two different things. I was dealing with trying to find assisted living care for a friend from quite a distance away and on short notice. Unfortunately, my friend was declared a "flight risk" and put onto their Memory Unit (locked) floor without anyone ever talking directly to her. This was $1000 more per month, and my friend was miserable there. It was very hard to hear "get me out of here" every night when I called her. A nurse from Avista evaluated her in person (finally) about a week after she arrived and determined she should be on the regular assisted living floors. A search was to begin to find her another unit facing the mountain, as her current one did. Three months later, with some hospitalizations in-between for my friend, I began to realize that Avista was not the right place for her. When I first mentioned that to the person in charge of the Memory Unit, all of a sudden (literally 2 hours later) a studio became open to move her to on another floor. By then, it was too late. My friend was too "with it" for the memory unit, and not with it enough to live on her own - so I set out to find her a smaller setting like a group home. The staff put a push on to try to keep her there, which got her very agitated because she felt she didn't know where she belonged. I was simply trying to do what was best for my friend (who was miserable the entire time she was there, but very patient about hoping for a positive change). None of the staff said anything about doing what was best for my friend....it was all about trying to keep her there. While my friend was at Avista, the staff were very good about returning phone calls, emails and texts. When I let them know the move to her new residence was set, communication from their end stopped. Nobody from Avista has ever said they were sorry things didn't work out there, or that they hoped she would be happy at her new home. When I flew out there to move her belongings, only the office manager asked how my friend was doing. I called the director and spoke to him about my frustration with her entire stay. There was never any sort of empathy for how the events had transpired and how unhappy it had made my friend. After talking for nearly 20 minutes, I told the director that I would be doing a review, and he accused me of threatening him and said he would refute anything I wrote. He said he would think things over and call me the next day, but he never did (it's been a couple weeks now). That alone exemplifies my disappointment in Avista. I'm sure there are people who have loved ones in the memory unit who are happy they are there - as well as people who are happy with the regular assisted living facility. Unfortunately, I can't count myself or my friend in either group. The entire stay there got messed up from the beginning, and rather than have a caring attitude about my friend, I felt that she was nothing more than $$ to them. If they truly care about people, then what is wrong with admitting their facility wasn't the right fit for my friend and wishing her well? I was told when I was signing the forms in the beginning that "we strive to make this a place we'd be happy to put our own parents". I will just say they are all smiles on the way in, and not very caring on the way out. I'm so happy my friend is now in a group home setting. She adjusted within 10 days and is happier than I've seen her in a very long time! In the end, it's her happiness and quality of life that matters....at least to me.

I am a friend or relative of a resident

We liked Avista because it has various options available in the level of care. It's a relatively small community, I think there are only about 80 residents, which is very nice. My dad speaks Russian and there happened to be some Russian-speaking staff there which was wonderful. They provide three meals a day, housekeeping, and laundry. My dad has a lovely one-bedroom condo with a gorgeous view of the North Phoenix Mountain. They have a lot of activities for the residents. The caregivers are really attentive and very easy to talk to. They all immediately know him by name and every time I'm there, they know me. I would absolutely recommend Avista.

I visited this facility

My first impression of Avista Senior Living North Mountain, walking in, was very, very good. It just feels very friendly, comfortable, cheerful, and clean. Activities offered and the staff was wonderful. They were very helpful and accommodating. The rooms that we saw were vacant and clean. We felt they were large enough, such that you don't get to feel crowded or cramped. The residents looked happy. We feel that it was the place for both my mother and my sister.
