Cooperative Home Care was established in 1986. This means we have significant experience caring for aging parents and for individuals of all ages with disabilities. Our services include skilled nursing, therapies, 12 & 24-hour care, personal care, household assistance, companion care, errands and appointments. We accept government funding (Medicare, Medicaid and VA) as well as private funding (health, long-term care and worker's compensation insurance) and self-pay. We also offer a Reduced Fee program, and we work with area disease associations to provide payment assistance for those who qualify. In every situation, we'll help you access your insurance and other benefits before you pay out-of-pocket. Amount can vary from 1 hour visits to 12 or 24 hour care. You can depend on our staff whether you need us 7 days a week or just a few times each month. Skill level can range from skilled nursing to therapies to caregiver skills like personal care and household chores.Our hourly rates are based on the skill level of our staff and the duration of each visit. Caregiver mini visits are available to help with any person care or household chore tasks. No hourly minimum required.Hourly care (rates for 3 hours or more) -- You may need a caregiver to assist with bathing, personal grooming, household chores, or take you to the doctor. Our trained caregivers provide these services and many more. Call us for a free assessment.12 & 24 hour care -- Many clients need round-the-clock care. We'll staff 12 & 24 hour shifts in your home to keep home care affordable, at a significant discount from our hourly rates. We also provide live-in caregivers and our 24-hour Care Reliability Guarantee - Cooperative Home Care guarantees that we will not leave a 24-hour live-in client during a staffed shift, except at the client's request. Plus, we offer free monthly nurse visits for 24-hour live-in clients.
