Signature Health Care of Palm Beach, Lake Worth, Fl Following my hospitalization and surgery, I was told I needed to go to rehabilitation for therapy before returning home. All I know about rehab, was that I needed to be in a facility, where the staff communicated with each other. to better serve the patient. Fortunately, for me, I went to Signature, where that takes place. The list of nurses, nurses aides, physical and occupational therapists, swallowing assistant, is too long for me to mention here. But at Signature, they all communicate and work together for the benefit of the patient.I am not only grateful for my experience there, and the progress I personally made. But I would Highly and Sincerely recommend Signature to anyone needing the very best rehabilitation anyone could ask for. Thank You Signature Dennis Obrien, former patient

Place Does not look like the pictures. No barber/ hairdresser here . Went into a dirty room. But staff was nice. This all was the first day.

My mother is ignored and must wait up to an hour for nurse assistance. Now she calls me to call and ask for help for her. Noone answers the phone after 8 on week days. Just an answering machine. I am trying to get her transferred to some where better. She is a vegetarian but cannot get anything but regular meals

If bed sores are your thing, this is the place for your loved one. Decubitus ulcers are the HAlLMARK of bad nursing care. This place is expert in not padding, not moving and not caring. There are animal hospitals that offer better care. A must to avoid.

My aunt was there for over a year. She got covid twice and fell x 3, the last one resulted in a fractured hip and arm. The physical therapy gym has been under construction for months. This is bad for orthopedic patients to only get therapy in their rooms. We sent her back in hope of better care. That didn’t happen. She went back to the hospital in a week for a severely infected surgical wound, respiratory failure and altered mental status. The care was substandard. She was dirty and sick. She will never go back there.

I was a resident of the facility for approximately five days. During that time the toilet in the room didn’t work properly, there were roaches in the bathroom, I left the light on in the bathroom 24 hours so I wouldn’t have to see the roaches. The staff was wonderful and very attentive. The physical and occupational therapy staff were wonderful. The place is old and it really isn’t a physical therapy facility in my view for people who have who have had surgery. The food was horrible.

I'm in shock at the high ratings!! I went in, was not screened, saw walls and walls and walls of plastic sheets. Took the elevator to the second floor and got hit with the most horrific smell of urine and feces I've ever smelled. HORRIFIC! As I walked down the hall it was like training for an obstacle course there were so many things in the hallway....including a PT guy working with two patients in wheelchairs...in the hallway....right. When I walked into my friend's room it smelled so bad I couldn't stay long. This is NOT the days of old when one expected this smell from a nursing home. Today there are great cleaning products and of the nursing homes I've been in (a lot) this is the only one that was this bad. My God....no one should live in that odor.

My mom has been at this facility for over a year and half. I have noticed a marked improvement in the quality of care, meals and the attitude of the employees since Johnathan started as the new Administrator a few months ago. It was a rough start over a year ago, but the past few months since Johnathan arrived, I have seen many positive improvements. Keep up the good work!

The food was horrific. The staff was overworked, underpaid and under appreciated. The rooms were crowded. The only good thing I have to say is concerning the Physical and Occupational Department, they do an exceptional and outstanding job.

I was taken to Signature Healthcare after having been in the hospital with a stroke a week prior. I was so very weak and could barely stand when I came. The first 4-5 days I struggled to do the exercises that my very patient physical therapist (Dana) gave me. I had significant uncontrolled movement in my right leg as well. Through much encouragement from Dana as well as the other physical therapists, I slowly started to see results. I had physical therapy twice a day and I actually looked forward to them! The therapists are very skilled at what they do, but it is imperative that the patient have a positive attitude in order to succeed in therapy. By the second week I was taking small steps, followed by walking with assistance and later by myself. I’ve come a long way and it’s all thanks to the physical therapy I received while being at Signature Healthcare of Palm Beach! All of the staff that took care of me while I was here were wonderful too. Many thanks! Sandy G.

With only a VERY FEW exceptions, my recovery from a fractured hip recently, was very pleasant and I received wonderful care from nurses, aides, and physical therapy staff. I especially enjoyed my nightly visits with receptionist Brenda after I was up and walking. Thankfully I was in a room by myself, although many would not mind having one, I couldn't handle having a roommate. I don't really like people even visiting me when I'm infirmed like that. I prefer to spend my time walking and healing which is why I got out of there in 9 days. I'm due for a right hip replacement likely in November so I wouldn't have a hesitation in going back to signature if there's a bed available at that time. My mother was there many years ago when under different owners/ management, and I would not have sent my pet dead rat at the time... thanks to New Management and the nursing director Marvia, those times have passed. Although a bit dated, the facility was extremely well kept and clean. The physical therapy department was very good when my mother was there and I received excellent assistance as well. I remember my mother saying physical therapy room was like everyone getting together for a party... friendly and fun!

My mother came here after hip replacement surgery directly from the hospital, and I am so thrilled with the care and rehabilitiation services she received during the two weeks she was there. My mother is a young 75, and had never had surgery before, so it was quite an adjustment. The facility was very clean, and well-managed. The food was above average (when my mom had an appetite). My mother was very happy with the nursing staff, the nursing assistants, and especially the physical therapy facilities and staff. They were attentive, available, and it was always a pleasant environment. The best part was that they allow pets to visit just like people - visiting hours are from 8am to 8pm. In addition, there was usually a friendly dog in the physical therapy room cheering everyone up. My mom made quite a few friends there and will be bringing them a homemade flan very soon!

Great place! They are actually 5 stars rated by Medicare and Medicaid. It takes a while for the govt to update their system. Very helpful knowledgeable staff, very caring. Therapy did a great job with Mom!

I am very happy with the service at Signature HealthCARE of Palm Beach. The doctors and the staff are all excellent. They are taking very good care of my aunt, and I am very happy with that. At this point, my aunt is not able to participate in their activities, but they do have activities for their residents there.

My aunt is at Signature HealthCare of Palm Beach. The people are very responsive and very caring. They do a good job given a difficult situation. The food seems to be very good, and she is enjoying the food there.

My father was sent from the hospital to Signature Healthcare of Palm Beach for rehabilitation, for physical therapy, and they accepted him as a resident there. The place is very clean, and the staff is very helpful. Everything is fine. He has a semi-private room, a little small, but it is adequate. They have daily activities that they post on the board. They have Bingo, Hawaiian day; they have all sorts of stuff. They keep them busy all day.

Before my mother-in-law passed away, she stayed at Signature Healthcare. She was in there for quite a while. The place was clean. The food was OK. They actually had good activities during the day. There were no issues with that. The caregivers and the nurses during the day were fine. It was the night shift that was very neglectful of her. My mother-in-law would try to get up to go to the bathroom, but she fell, and one of their night workers came in and basically told her it was her own fault and left her there in her own waste. It was horrible. They could improve by having new night shift people and having a better knowledge of what's going on in their own facility at night. We would not put any other elderly parents in that facility.
