The rooms are filthy. My love one's room hadn't been cleaned in the five weeks he was there. Couldn't use the nurse's call bell because the wires were coming out of the walls. The toilet was too nasty to use. The food was awful. On several occasions I couldn't find a nurse or technician anywhere on the unit to get assistance. Someone came into my love one's room and stole his neighbor's socks, batteries from the tv remote, hospital socks, his pants and some other items. A patient was stripped of his clothes while he laid helpless in his bed. And, another person's coat was stolen. The residents are constantly begging for money. I do not recommend this facility for anyone.

This is the worst facility ever. The Social worker Mary is rude and disrespectful. I was in a meeting with this person that left me in disbelief of how she conducted herself. I have never felt total numb and unable to say anything, knowing I had to leave my love one in her care. Since she conducted herself in this manner with me a disable senior who had the ability to leave the fàcility, what will she do to my loved one who could not talk or walk. She was totally heartless. Not only that the place stinks. As soon as you walk in the front door, the smell slaps you in the face.

My brother is currently at Clinton Healthcare Center. It's old, and right now, the heat isn't even working. In the last couple of nights, it's been pretty chilly outside. The first room he was in had black mold, and I advocated to get him out of that room. I haven't seen him do any activities with them. I would come up there and I would look for his blanket. They're not as attentive. If I wasn't coming up there, I don't know how he would be. I would not recommend it. They told me they have activities, but I haven't seen them do anything at any time of the night or any time of the day. The staff on the floor never answered the phone. I try to call when I'm not there and I could not get anybody on the phone. The food is always cold, doesn't look good, and doesn't look appetizing.

She was initially isolated from the population due to not having her 2nd Covid injection. However, upon receiving the next injection, she was later placed with a roommate. She enjoyed going to the recreation room and listening to the sounds of the 60's, singing, watching television and meeting new people. The staff were accommodating. Especially Jay G., the Lead Social Worker. He was great and very personable with the family. He was extremely helpful in connecting her with a Long Term Home Health Care Facility. Much appreciated!

I met a lot of nice people during my stay. My recover went well. When admitted I wasn’t able to to eat by mouth. I was on a feeding tub and now I’m eating by mouth. That staff did and amazing job caring for my health.

I have been at this facility for a month. I am discharging today. I want to thank the carrying staff members that cared for me. Rithe from Admissions, the entire therapy department was amazing. Ayani,Phyllis,Felecia,Toi,and Olivia the Nurse Practitioner Wilber is the Leader of this team and he leads with respect. He has a nice staff. If I ever need help again I would gladly return to their facility.

I came to this facility from Meritus Hospital in Hagerstown. I had covid pneumonia. This was only place that could take me. I am here to rehab my legs arms and breathing. Staff is friendly. This facility isn't the cleanest, linen hasn't been changed since I have been here. Food is almost always cold. The missed my meals on 3 occasions. If this facility had the funding im sure it would be better. Staff here works hard to do there best. I dont want to come back here. I came to do my rehab and go home. I'm not here long term. Tbh long term isn't recommended. If you ask for somthing they will do there best to get it if they are able. Like u said before staff is great. The facility is what lacks

First of all my stay here was the worst. I asked this lady name Bonita if she could leave my door open and she told me no she could not leave it open and I was eating my food she came in my room and told me that I need to hurry up and eat my food . I’m a sick patient and you can’t force me to eat my food quickly my entire stay here she was very rude and nasty towards me like I did something to her. This place is unsanitary not clean and the food is horrible. This place needs to be closed immediately. If you love your loved ones do not send them here. I slept on a air mattress my entire stay here. I’m so grateful to be out of this place. I do not know how they stay in business.

My mother's experience in Clinton Nursing & Rehabilitation Center was not very good. The rooms were not clean, she said she saw a roach crawling down the wall and she said that some of the staff were not nice to her. She said they pretended like they're queens but they're very street-like. The food was not very good either. I think she had a TV in her room but she didn't have a closet, and she had no stand to put her stuff on. It was just like a ward area, it wasn't a home-like setting. One of the case managers, she's a newer one, was very open and very honest to me but that was just one. The other ones were very condescending and they weren't honest. She had dialysis there but there were no other types of therapies offered.

If you really love your love ones don’t send them to this place. DC jail is cleaner than this place. The health inspector need to come thru this place and shut it down. It smells and is not clean at all. The cleaning crew comes thru everyday and clean your room. They do not change patients linen or even have clean gowns for patients. The food is horrible. My dog would not even eat this food. For the most part the staff is friendly except for Bonita. I ordered food my entire stay here. My room had a air mattress. I cried my entire stay here. I would go back to Southern Maryland Hospital any day at least it was clean and I could eat the food. Stay far away from here. I don’t know how they are still open. The county health inspectors aren’t doing job cause if they were this place would have been shut down. I had a employee going thru my personal belongings while I was taken to my doctors appointment.

I was transferred to this facility from the hospital to take antibiotics I need for a surgery that I had and it was the worst mistake coming here. The staff and nurses are friendly and they take good care of you but this place is old and outdated. It needs to be renovated or shut down immediately. The food is horrible . This place is very unsanitary. It is not clean at all. They don’t even provide patients with gowns. They don’t change patients linen. I actually cries my whole time here. I thought about walking away from this facility if it wasn’t for my mom telling me not to do because insurance will not pay l would have done it. She would bring me food every other day and if it wasn’t for her I would have never made it my stay here. Do not send your loved ones here. Stay far away. The health department needs to shut this place down immediately.

This is the worst facility I have ever experienced. My mother is a resident there recovering from COVID-19. They have left her to figure out how she will get to the rest room, she was told by a nurse(CNA/GNA) that she was on break and then she walked off. They have not feed my mother on several occasion, she was without heat for two days and I was told that I could not bring her a heater and they refused to move her to a different room. I have spoken with several staff members and they all seem to be so surprised when I have a conversation with them. This facility is the WORST place I have ever seen.

This place is horrible, employees have a nasty attitudes And are very rude to the patient. Food is always served cold and mot eatable just enough on the plate for a child nothing you would ever serve an adult. Inspectors need to go in check the place out. Do not and I repeat do not send your love ones here.

A disturbance concerning my Mom happened at the facility. Three of us went to speak to administrator but we were told for almost an hour that he was on the phone but when we had a question his assistant would call him and get immediate response. I asked the administrator if I could speak to him about a matter concerning my Mother when she was at his facility but he said, "I do not need to hear your side of it." Arrogant.

This facility should be shutdown. The money that they receive from patients could be used to update this facility. The elevators are in poor working condition. The furniture is broken and below standard. The staff act like they're doing you a favor tending to the patients. Some days the stench in the hallway almost make you gag when you visit. House keeping is below standard. Patrons miss meals or meals are left for hours if they're doing medical procedures and served cold. If you care about your love ones think twice about putting them in this facility. I would not put my worse enemy in this facility.
God help those who are there.

This place is not just for a human to reside in. Very short staffed and they work that way here. 2 gna's can not care for over 50 people. But "State" keeps this place open. Its best to pop up on your loved one to speak on the lack of care they're given.

The patients are not a priority. The environment is unpleasant - like an old never updated hospital. Too many of the staff working the floor are cold and doing the bare minimum to keep their jobs. Patients go unbathed for weeks. No one noticed the extreme large infection on my friend's legs (when my friend showed it to me, I immediately showed it to the nurse on duty but it had been there for a long time - my friend passed on a week later.). They would not tell me anything about his passing on even though my friend had no family or other friends and they knew I was the only person visiting him-he had absolutely no one else in his life. Sickening odor in the air all of the time. No one should be placed here. This facility is not there to help people; it is there to make money. Things were constantly being stolen from my friend. The atmosphere is unkind and harsh.

Watch out. Pure lists. This is not somewhere a human needs to reside for any length of time. Dirty, nasty unskilled staff, old, smelly, unprofessional workers & staff.

As a person who has seen first hand how this facility operates it's not really bad nor is it the best facility, but it can get better. I've seen great nurses and nursing aids who bust their buts to make sure the patients are well taken care of. We have nurses and aids who picks up extra shifts or days cause we are challenged staff wise to help out with the care of the patients. I can't speak for other workers in that building but speaking for myself it's hard to give adaquate and proper care when you're working short 75% of the time. We don't keep staff because they're not being treated right. There is a big problem far communication is concern. A big problem when the nurses don't listen to the aids about a patient. The Lpns and Rns are not going to help us when we are short staffed but will give us a list of directives to fallow like we are super humans. Overall the department heads need communicate n interact with the staff to ensure things go according to plan.

By far the worse place I have visited. Workers arguing in the halls. Patients not receiving medicine or care. I don't think there USA doctor or even a nurse in the whole place. No stars. Should be investigated.

I worked up stairs on the 4th floor on the Dialysis unit and let me say this is by far the WORST Nursing & Rehab facility I have EVER been in. It smells HORRIBLE and they told me it's because I'm not use to the smell, the Nurses don't know who their residents are, they don't send there residents up for dialysis. They don't update their residents records. Hygiene is NOT important, residents come up for dialysis smelly or not changed. I would NOT recommend this place to anybody.

My mom was diagnosed in April 2016 and transferred to this facility after one week in the hospital. I was told this would be best for observation and treatment purposes. I was told by before she was admitted her stay would be covered 100%. Why do I keep receiving a bill for over $6000.00. My mom stayed almost 90 days. It was like they were trying to kidnap her. She stayed in a room across from a room filled with 3 males. One male always undressed and another kept coming into my mom's room. Very unsafe in my opinion. My mom who walks, made it off her floor because her alarm bracelet didn't go off. Now she suffers from dementia. She shared a bathroom with 4 other women. The bathroom was filthy. When she didn't eat, it was no concern so she lost 12 pounds. She was 128 when she went there. Her clothes were always missing. Because residents were allowed to wander, theft was a problem. There was a severe shortage of staff in my opinion. My mom mentally deteriorated so quickly. It was like her will had been broken. She feared certain nurses. I can't prove that they did anything to her. I paid to have her hair done. It never happened. I complained and was referred everywhere which led to unresolved. Very depressing place. If you didn't want to shower,it was like oh well. If you didn't want to participate in physical therapy, it was like oh well. There were a few aggressive residents allowed to wander the facility. There were some great workers their but not enough to make the center run as well as it could. Trying to reach to reach a doctor or assigned social worker was close to impossible unless you contact the director at their corporate headquarters. To the director, for God sakes and the lives of the residents, Hire Some More workers,

One Star -- Terrible! I originally went to CN&RC to rehabilitate from a stroke. However, at the time I also had serious degenerative joint disease, affecting primarily my right hip, but the therapist they assigned me ignored the medical history. Consequerrntly, my left hip had to overwork itself and needed to be replaced along with the right hip. Nevertheless, the rehabilitation unit is one of the better departments at the center; the rehab folks are subcontracted from another company. The nursing, social-service, activities, and housekeeping departments (all completely run by CN&RC) leave a great deal to be desired -- REALLY! Especially, labor (not enough housekeeping, nurses, nursing aides, social-worker designees, administrators, etc.); linen (towels, wash clothes, pillowcases, etc.); Scarcity seems to be an important component of their profit margin. One must remember that Clinton Nursing & Rehab exists to keep the books in black ink and the individual workers with rent/mortgage payments and car notes covered, while staying at least one step ahead of the regulators. If you know a person, whether you like her or abhor him, you should try to keep them away from CN&RC and places like it. Good luck.

This place Is the worst. My family member was in this facility about a year ago an it wasn't to bad. she is in there again and the quality of this place has drastically declined. The funny thing is they are under new management. It stinks all the time. There is no excuse for that. The majority of the staff members are rude and unprofessional. Their records are not accurate. My family member was moved from one room to another and at least a week later they were still giving visitors the wrong room number. How hard is it to update the records? The new management needs to improve their standards ASAP. If you can avoid this place please do. (and I hate to say that because I do my best to support and promote my neighborhood facilities)

my son has recently been a patient at this facility. the place was filthy, smelly and seemed to lack in organization. I was quite frankly appalled with the whole experience. his clothes were dirty upon our arrival and my biggest concern is infection in this environment. i'm not sure what we can even do at this point but whomever inspects this place is not doing a thorough job.

After my brother was discharged out from the hospital, he went to Clinton Nursing & Rehabilitation Center. The room he was in was small and crowded, but it's relatively clean. He shares a room with another guy, but the thing is he does not have his own sink and the closet that he has in the other person's area. They offer no TV and no telephone and my niece needed to provide that for him. He is on a feeding tube and the nurses help him to eat. He sometimes is having pain especially with therapy, but they are not giving him any pain medication or any muscle relaxant at all.

Despite not having a lot of experience with rehabilitation centers, it is clear that the Clinton Nursing & Rehabilitation Center was far from a prestigious one. That being said, it was not necessarily a bad one either. The atmosphere of the place just seemed rather bland and empty. I know a rehab center is meant to be a party or anything, but a more vibrant setting and something for patients to do besides lay in a bed all day and night would be appreciated. There was what seemed like a lounge area for patients, but even that seemed a bit old fashioned and unappealing. Aside from the slightly dull front desk assistant that I encountered, the staff did seem friendly. The nurse or patient attendant had a jolly personality and smiled. Also, despite visiting hours ending, we were allowed a bit of extra stay time. That is definitely a positive gesture by the center that gives it a higher rating. All in all, though there may easily be better centers for patients, this one meets the basic requirements of patient care and can be an acceptable option.
