My wife received excellent care during her rehabilitation. The room was clean and well maintained. The staff was consistently supportive and professional even in the face of potential challenges like being short-staffed. The food was well proportioned and reasonably tasty according to my wife. I want to recognize the hard work the rehab team put in to help my wife reach her goals. The discharge process was a bit bumpy and needs improvement because I could not retrieve my wife’s CPAC machine for six days after she was discharged from Avamere. It is for that reason I am giving 4 stars rather than 5 stars.

Totally unable to recommend this facility. If you take your loved one here, be prepared to heavily advocate for them. You will have to ask for medication lists, you will have to follow up with whether the patient is even getting PT or OT. The booklet says a weekly care conference is held, you have to ask to be present for it to get information. Had to ask for a soft diet but our elderly Mother lost 7 pounds in 15 days, was living on Boost we supplied. The social worker assigned did not keep commitments. The rooms are dark and depressing and they basically just left her to lay in bed most of the time. Choose another facility for your loved one. Staff (CNAs were kind, just overworked).

Small when each person has a wheelchair. There is no air conditioning in the rooms so you will need a fan. Sometimes just to circulate the air.

The nurses and aides at Avamere are kind and friendly and the facility is very clean. Unfortunately we experienced continued issues with my mother's care and safety, and would not recommend this space if your family member needs transport to a dialysis or other medical appointments. My mother was left alone in a lobby for hours unattended in the evening following a transport drop-off, and missed multiple dialysis appts due to a poor coordination with the volunteer-based transportation service they use. The staff also did not notice when my parent was ill with a UTI and c-diff, resulting in us asking for ambulance to address her illness. We tried working with all levels of staff, including the administrator, to find solutions and there just doesn't seem to be a system in place to keep track of patient needs.

My mom is in Avamere Rehabilitation of Cascade Park. They do a good job with the food and the service. They're so short-staffed that they can't get the physical therapy schedules like they should because they just don't have the staffing. She's very pleased with the food and more pleased with it than the place I've gotten her into. She has a shared room with another client. It's very tight, not a lot of space, so it's very difficult to visit. You're kind of invading the other person's privacy. Even though they have a curtain, the curtain is so tight that you almost have to push against the curtain as you put your chair in there. So it's very tight quarters. They've got the TV. They have a nice desk there for them to do their stuff for when they're out of bed. They have a nice, shared, shelf. They can open up the drawers on either side. The building is kind of rundown, it's not kept up well in that sense. I noticed that the bathrooms were not always clean. You're paying $200 a day, they're there, but they're not paying a lot of attention because they're so busy.

100 degree day outside. The rooms have no air conditioning. The head nurse was very rude when I commentated bout how hot the room was. They brought me a 6 inch fan as if that would solve the problem. While I was waiting an older woman across the hall way had been ringing for a nurse for at least 5 minutes. Finally the older lady came out into the hall in a walker and fell. Her only problem was she needed to go to the bathroom. The staff treated her like she was a jerk. That is when I decided to check out of this ill run and equipped dump and checked out. My whole stay was at most 30 minutes and that was too long. What a joke-don't ever go there.

It was awful treatment for my father who came from the hospital for re-hab. He is 89 has dementia My dad was ignored and the staff didn't know how to take care of him properly.Left him sitting in Lobby for other nurses to watch over him .Told me he had in infection and pneumonia he ended up back in hospital was dehydrated from no water given he didn't get meals at regular meal times. My Dad was left in his room in underpants sitting with damp blankets I heard him calling help me I'm cold. My daughter and I dressed him in his own clothes we waited for 30 once the call button was pushed before the nurse came in..

My father passed away in Avamere due to Covid. He evidently picked it up in the hospital before going to Avamere, so that’s not what this review is about. But I did have some questions for the Director of nursing about his stay there, and she was very rude to me. I live out of town, so I just wanted to get some more specific info about when he entered the facility, etc. and other info about a call I made for an ambulance to take him to the hospital. The woman I spoke with actually had the gall to ask me how often I spoke with my father before he became ill, as if I were a bad daughter for living out of town. I just couldn’t believe it! Then she made another rude comment to me a few minutes later, so I just thanked her for her time and hung up. I would never want to deal with that woman again She was unprofessional and rude. I personally would not recommend this facility because of her behavior.

My Grandmother was placed here after surgery and they skipped her meals, only a few min of Physical therapy not the 1.5 hrs they say. The staff is 50/50. Some were very helpful others didn't seem to know there job. There was very little compassion for their clients. It felt like they were there just to get a paycheck.

The caregivers at Cascade Park Care Center (Avamere Rehabilitation of Cascade Park) are good. They have quite a few activities that you can go to if you want to, like sing-alongs, Bingo, and movies. Entertainers come in and they have a Church that comes in on Saturdays. They also do therapy there and their therapists are good. The place is really nice and they really work hard in keeping it clean even though they have several patients that are incontinent. They have several dining areas, but the patients can eat in their rooms if they choose to. One of the dining areas is where residents who need help are fed. One dining area is also their activity area and it is so nice with lots of lights. If you have a problem, you can go to the staff and they take care of it right away.

My experience with Cascade Park Avamere was excellent. It's physically clean, and everyone was willing to help. My husband was in this facility. They had a wonderful staff who did an excellent job. The dining room was nice, very comfortable, family-style, and attractive. The food was good. The staff was very caring and became family after awhile.

I have chosen Cascade Inn, and we're working on getting Mom in there in June. I like all the amenities and activities they have, and I like the rooms. I like the facility and the things that they can do there. The room that she would be getting is a studio, probably 12' x 16', and it has a bathroom also. The staff is very nice, and the founder of the Cascade Inn and other facilities around there lives there as well. I thought that was a big plus. They have exercising and card games that she likes, like poker. They have bingo, and they make things.

Cascade Park is a well run facility. It is clean, the staff members are friendly, and from what I can tell they seem to care about their residents. They were very welcoming to us, and gave us a nice introduction to the facility and what would be taking place there. I think they are understaffed a little, and even though they have monthly meetings to explain the status of each resident with the family, sometimes the communication between staff and family members doesn't quite go as well as it should. I think that the staff sometimes thinks that the family members fully understand what it is happening, when many times the family members are confused too, and need a little help in understanding the whole process of a loved one moving from independence to assisted living. But as a whole, Cascade Park is a very nicely run facility.
