We sent my grandfather to this place thinking he would be in good hands but it was the opposite. When my grandfather arrived to this place in December of 2021, we were not allowed to visit him per their covid protocols for 10 days, on the 9th day we get a call saying the facility is in lockdown because a staff member tested positive, therefore, we had to wait another 14 days before we could see my grandfather. Unfortunately, a few days later we get a call from the nursing home saying my grandfather was on his way to the ER. Two hours later my grandfather passes away. We had so many questions as to how things transpired and we weren't getting any answers from staff. Instead they became rude and acted like we never communicated with them. After the state conducted an investigation they found that this nursing home had deficiencies. My grandfather's medical records specifically said that isolation would be detrimental to him. WA laws specifically said we had a right to see my grandfather while he was in the facility. I will never send another family member to this facility. I know we aren't the only family this happened to.

My husband had a stroke in January, he went directly from the hospital to Toppenish Nursing & Rehab Center, and he was there for two months. The staff was friendly enough. His room was just like a hospital room. The food was OK. The only activities he had were physical therapy, occupational therapy, and speech therapy three times a day, three times a week. However, they were short staffed; they didn't have enough nurses.

My mom will be moving to Toppenish Nursing and Rehab. Proximity was the main reason why we chose this nursing home. They have their own bathroom and I believe the rooms are pretty spacious. They do have a dining room, but their patients could eat in their room if they wanted. The administrator said that the facility is a part of a community where the residents can take part in the activities of the community. He did mention that they do take them to the fair when the fair comes, and they said that local people come over and do trick or treating. He also mentioned that they do have people come in to cook and things like that. Kids from school also come in to sing when they are on the holidays.
