My Dad passed away last Sunday at the nursing home. The funeral home along with myself kept calling trying to get a death certificate. Five days later we finally got one. They made up all kinds of excuses on why one was not issued. Due to the fact that they took five days for a death certificate the crematory is behind schedule and my Dad still has not been cremated one week after he passed. My Dad was only there for three months and I wouldn't let my dog stay there. The staff talks down to family members and I can only imagine how they treat their patients. Towards the end my Dad couldn't speak and couldn't move his arms. I asked if he had a stroke (which I am sure he did) and they informed me that he didn't because his mouth wasn't drooping. Four days before he passed away I had a phone conversation with his case worker, PT staff and a nurse. When I asked the question why he is not able to speak they informed me that he could speak just fine. I talked to one nurse and she told me that team was not allowed on that floor he was in so they hadn't spoken to my Dad and had no idea how bad off he was. Before my dad was unable to speak he told me how badly he was treated by some of the nurses and CNA's. My Dad couldn't walk and there was no way that I could physically take care of him. I wish I had done my research before I sent him there. He would beg me to get him out of there. My heart is broken that he had to go through this. There was only one nurse that was so caring and her name is Tiffany. She spoke with me and informed me about my Dad's health. The rest of the staff there acted like you are putting them out. I actually had one yell at me on the phone because I wanted to speak with my Dad. Please before you send a loved one there, do your research. I did my research after the fact and it was too late.

I cannot express enough how dirty this facility is. My family member is currently there and was actually getting good care until she had a stroke last fall, then was placed on another unit. Since then, she’s declined. Pressure sores on several occasions. Call bell is NEVER within her reach. I’ve battled with staff about her care for months. The cleanliness is nonexistent. The owner should be ashamed and embarrassed of his facility.

The nursing and therapy staff at Evergreen Rehab were warm, caring and very responsive in meeting my Mom's needs during her stay for rehabilitation following hip surgery. The overall facility and patient rooms are clean and comfortable and staff interacts well with patients.

First day there I saw blood on the handles of my family members bed. It was dry. My family member had just gotten there and it wasn't theirs. Beyond disappointed with this facility. The staff can be frequently found outside smoking. When asking for assistance with a patient/ my family member, a nursing staff member frequently rolled her eyes and was put out because she had to do her job. Communication was poor amongst all levels of staff members. At times my family member had to wait up to 5-10 minutes when needing help with the bathroom. Meanwhile there were nurses at the nurses station just sitting there. The only good thing about this facility were there OT, PT, and SLP staff.

I went to Evergreen Health and Rehab. It wasn’t very impressive at all. It didn’t look very clean. There were very few residents out and about. They all seemed to be in their rooms. There were not much activities going on. The staff was cordial. They were nice enough. It didn’t look like there was a lot to do for them.

My husband was at Evergreen Health and Rehab. I liked the facility very much. They had a very nice space. The older people seemed to like it very much. I thought it was very clean and nice, and I liked it. The shared room was very nice and had two men in there, my husband and another man, but they had their privacy. The caregivers were really nice and helped him a lot. I enjoyed being there. It was a brief stay but what I saw was very nice.

Evergreen was very nice and really clean, and the staff was very kind to my mom. She is even Facebook friends with some of them now. The food was terrible. That was one bad thing, and everybody was complaining. My mom wouldn't eat, so I had to bring her food. The people that worked there were wonderful, but the food was bad. The only other thing that I had kind of a complaint about was she had hip surgery and she was supposedly there for rehab -- but we didn't know she was going to need a longer term or whatever -- but everybody at the place was in a wheelchair unless they were at physical therapy for like one hour a day. So there was nobody in the bed, then the wheelchair, then the walker, and then out the door. There really seems to be steps because they are all a fall risk so we can't let them wander about, but on the other hand, how can they get stronger? They had all these alarms and everything on the wheelchairs, so I felt that was really a detriment. And then, my mom started to get really kind of hooked onto the wheelchair. I felt maybe that was just the system, but I didn't think that was very good. They do have a lot of activities there. I don't even mean take them out; I just mean let them -- as they are strong enough -- to use the walker or the cane. Now, she is home and using the cane because she is walking around her own house and everything. If she stayed there, then she would end up needing long-term care because she was getting so helpless. But I couldn't say enough about the workers there. They treated her like their mom.
