My mom stayed for a week after a stay in the hospital to have her physical therapy that was needed. Everyone was friendly, Patients included. Rooms clean. Staff stopped by often to make sure She had everything She needed or just to say hello. The physical therapist worked daily with mom and were impressed how well she was doing. They answered any of my questions I had and told me to call if I had more or any concerns. The admitting staff were very friendly, helpful and took many calls from me with later questions. Mom was quarantined so we were able to have window visits and bring her dog as often as we could. Thanks again for a great stay. God bless you all!

My mother was sent to this facilty from Winchester Medical Center. She arrived late afternoon and was dead less than two days later. It is my belief that the medical staff breached standard of care. This facility stated to WMC that they were providing breathing treatments again after their shutdown from Covid a few days prior. Post death, this is what I was told by the admissions office, and a Nurse Administrator however when I called the evening before my Mom's death the nurse told me that the doctors "absolutely not write for breathing treatments because it puts aerosols into the air." My family attempted to get my Mom her needed breathing treatments throughout the day and into the night. I was told by the nurse that they had observed my Mom "overdosing" on her inhaler at which point I told the nurse to take it away from her. I explained that she is confused, she came to them 24 hours prior alert and oriented and now she is not making any sense, she has asked multiple times over the past couple hours to be changed, and she sounds like she is suffocating. I was told that the nurse called the doctor and he stated my Mom did not need a breathing treatment because she has inhalers. She was a stage 4 lung disease patient who was unable to use inhalers appropriately and required breathing treatments to breathe. This was her standard of care and the facility knew when they accepted her, she received these nebulizer treatments every 4-6 hours in the hospital and at home for the past several years. When paramedics were called and arrived, they report they were met on the porch of the facility and were told she had gone into respiratory distress hours before. Unfortunately she was unable to be saved. It seems that the doctor and staff played an integral role in her suffering a painful death as she seems to have suffocated herself into cardiac arrest. Weeks after the investigating nurse was supposed to give us answers, she could only report she had reached out to the doctor who refused her proper treatment and the doctor had not returned her call. I would never send a loved one to this facility again. It's disgusting what they did to my Mom and very telling when the doctor doesn't even put a little value in her lost life by responding to his own company's inquiry.

I was told i would get 2 hours of pt a day. That was a lie. I got maybe 1 hour. In bed the rest of the time. Horrible place and so was the slop they called food. Look into other options before going there. I signed myself out after 3 nights. My arm and hands were swollen I was told they would contact my doctor. I never heard back . The first night I was there no one came in to let me know anything. Its like I was left there all by myself. My roommates daughter checked her out because the staff sent her to the doctor with the oxygen turned off. This 80+ lady could have died. I will say the travel cna's were good and helpful except the day i discgsrged myself. I have ms. My my mobility is limited. Three of them picked me up and flung me in the wheelchair. I was glad to get the heck out of there. If you care about yourself or loved ones then don't go or send them there. It wasn't rehab at all!

My biggest complaint is that the nurses and most CNAs are very rude. They seem bothered when you ask questions or make a request for a family member. Envoy is understaffed which results in the residents waiting too long when using the call bell and basic needs, such as weekly baths, not being met. Clothes that are washed at the facility take a week or more to get back, which resulted in my mother going without clothes for days. The food is apparently terrible too.

My mother stayed at Envoy for rehab after being released from the hospital. We visited all the local facilities, chose Envoy and were very pleased. My mother did well and enjoyed working with the therapists. We were able to get updates easily and everyone was helpful.

I received a list of facilities from hospital. My father and I chose Envoy. He spent 22 days there for rehab and we were very pleased with the care he received. Would recommend to others . The housekeepers even spoke to him while cleaning his room. He went from not walking to using a walker. It was nice having him close to my home so I could visit several times a day. They even let me bring him in goodies to snack on.

My father was being discharged from hospital, and Envoy was on list of facilities that had an available bed. We didn't have a lot of time to look around, so he was a patient there briefly. The hospital should take them off the list of facilities-Envoy was HORRIBLE! My father was unhappy the entire time he was there and begged us to get him moved. The staff lost his clothes, and we had to make a claim to be compensated for them. One day we visited, and he was in bed with only a Tshirt and diaper on, absolutely unacceptable!!! At his first Dr.'s appointment, he had lost MORE weight, and the Dr. was furious and immediately helped to get him moved. We found out when he was moved that Envoy was not giving him enough nutrition in his feeding tube per the Dr.'s order!! He is much happier at the rehab place to which he was moved. He gets taken to group activities and to eat in the dining room. He is getting better care. He gained 6 pounds in the first week there!! BE WARNED--DO NOT SEND YOUR LOVED ONE TO THIS HORRIBLE FACILITY!!!!!

Absolutely horrific care!!! Both, my grandmother and grandfather were without oxygen at least twice(that we know of) because their tanks ran out. My grandmother passed after 2 weeks of being there, luckily we got my grandfather out before something grim happened to him. No responsibility on Envoys part, not even an apology. If you care about your family member please learn from our mistake and do NOT let them go to Envoy!!

My mom was a patient here for a short time due to rehabilitation, She was originally placed at the sister facility Envoy of Woodstock but we wanted her close to home. Previously my father was here 5 years prior and the facility has completely turned around FOR THE BETTER. The Administrator who is new to this building but not to the job field was the most helpful and knowledgeable Administrator I have ever met in my life. If I wasn't mistaken she could have been my mother's doctor! Her mottow is everyone is there to do a job and their first priority is the patients! Any questions about mother's care or concern's we had no matter how busy we were she always made time for us. Therapy worked wonderful with mom and her plan of care was more then we hoped for. I have noticed that at any facility staffing seems to be an issue, It happens, people get sick, hurt whatever the case may be. It is nothing new as I am more then familiar with the medical field so I am more then familiar with the process. With the nurses that I met at Envoy - they were always on top of their job and went beyond all measure's to make mom happy. Anything down to wanting something different for her meals, they made sure it happened. Now to be realistic you cannot go into any facility making unrealistic demands. We were willing to do our part and to make sure mom was compliant with what was expected and they were sure to go above and beyond to make anything happen for mom. We felt she was in good hands when we left her for the night. When we couldn't make it to take mom to her appointments due to work, they made sure she went and took care of transportation - she was on time to her appointment and I was able to have a conference call with her and her doctor during her visit.. That took a lot off of us and to know I could depends on them for the trip to Berryville made me ecstatic. This took a lot off of me and I appreciate Envoy for all their professionalism, patience, love and care that they showed with me and my family as anyone in the Medical field has a stressful job and to keep a building running it takes a Army! They're still open and taking patients so obviously they are built army strong!

My family had a wonderful experience at Envoy. The women in Admissions welcomed us, the Executive Director was in to see us before my father was even settled in! Nursing staff became a second family. The therapy he received got him strong enough to return home stronger than before he was hospitalized. Very pleased with our stay!

My father was at Envoy. Previously, I had heard they were a good facility. The hospital had them as a facility for Dad to go briefly to get stronger. The hospital should take them OFF THE LIST OF FACILITIES if they really cared. ENVOY was terrible. The nurses were rude. The staff bringing the food did not even say "hello" to us when they brought the food. He could not eat the food. I saw the food and it looked very unappetizing. The young women (except for one) helping him with rehab were very uncaring. Staff was rude to the family. We were so wrong to take him there. I still regret it to this day!!!

Understaffed. Not organized. And it takes 5 different people to answer a question. Personally it has nothing to do with the Cnas. It's got to be management for not helping them and providing them help and support.

My mother fell and broke her hip here. It was due to them not changing bandages on her legs. She spent a week in hospital, no one called us for 24 hours, and we had to call the police to get them to give us her personal belongings. They had packed them and threw thew in a shed with lawn equipment less then 24 hours after her fall!!!! The staff is rude, mean, and lazy. We are going to sue them.

First off let me say some of the workers there are very very helpful and very very concerned. Others not so much. The biggest problem NOT enough staffing. My dad left there yesterday. He will never go back. He was NOT ready to go home. But what are our options. Most of the time (not always) I saw they did what they could with the staff that they had. The caring ones ran around like crazy people. That is one way to make a major mistake. When he was discharged, my mom, my boyfriend and I loaded him into the car ourselves. Kind of the last straw on a miserable almost month long experience. There was ice everywhere. My dad had very little mobility on his own. The wheel chair ramp was covered with snow and ice which I took pictures (gotta love camera phones). The parking lot and sidewalks were covered with ice as well. There was no one to even walk us to the car. This was typical of his entire stay. They were just over worked for the number of staff members that they had working. While we sat waiting by the nurses station to get his stuff ready which was a treat in itself. The phone at the nurses station rang 5 times without being answered. I was so tempted to go answer it myself. I think if you can do for yourself this place could work. But if you have to depend on anyone, I would say NO NO NO. I know most of the staff tries their best. But it was beyond belief what one or two people seemed to be in charge of. So sad that this is the way that most of us will end up spending our last months on this earth.

I had heard a lot of good things about this facility. When my mother in law had hip replacement surgery and was placed at Envoy I was ecstatic. However when she arrived she could never get a straight answer on anything about her medications, when she would go home, etc. She would pull the call chord and it would take them a better part of an hour to respond which is unacceptable. When I would call and try to get answers no one seemed like they had any idea what was going on and I would never get a call back on anything. When it came to her discharge I had a heck of a time getting her prescriptions and now I am waiting on a call back about her in home therapy from the Social Worker and she is now 3 hrs late calling me back. The most caring person there was a wonderful lady that was in maintenance who came in and gave my MIL a hug and wished her the best. Bottom line....there are some severe deficiencies in regards to lack of communication and response time that needs to be addressed before I ever put her back there.
