The rooms are very nice and well kept. The staff is friendly and most helpful. I would like to honor and praise Jasmine, Discharge Planner as she is our guardian angel!!!! We were able to arrange top notch Long Term Home Care set up for my brother, Louis Mottley. Thank you for holding my hand and guiding me through the process. You are truly an asset to PA Health and Rehab!!!

So Not put your family member or loved one there! The food is not something a human should be fed! If someone weds assistance or falls and calls for help they do not come. They do not bring your meds in time note give their patients the correct meds. They are way under staffed and very unprofessional. The smell in halls and in rooms are unacceptable! I had to get my loved one out of there by transferring. BEWARE

This place smells horrible. From the lobby entry and all the way thru hallways and my mother in laws room. This IS NOT a clean smell. I feel panicked to get out of here it reeks so bad.

I would like people to know that this facility is not a place to put your family member at!! My mother has been there for a long time and the help is not enough there , Not enough CNA's for the residients and when they are called to a room it takes them almost a couple hours to go check on the resident . My mom's food has been brought to her room not warm and when she is not able to feed herself they leave the tray and say they will be back to feed and don't come back for a while and the food gets cold There has been only 1 nurse on staff to take care of a whole floor. Mom dosen't get her meds like she should. Her room is not kept clean and they don't give bath's like they are supposed to. When you have people tell you from there that they would go some other place...Well that says enough. I' ve seen from first hand how they are there . VERY UPSETTING how Some of the CNA's are. Most call out on weekends so hardly ANY help.

My dad is at Princess Anne Health & Rehabilitation Center. I do like it. It's clean and the people are nice. I feel like the food options have been good. My father appears to be well cared for and his rehab team has been very good with communication and making sure he's getting his OT and PT. The value for money is comparable.

Rooms were clean and neat. Staff was very friendly-especially Amanda-the activities director. Her residents enjoyed the variety of activities that she had to offer like music of which she plays her guitar for them. Just a beautiful facility- decorated in a very tasteful manner, location is great. Wonderful activities. Great activities staff. Good meals.

I am a family member of a resident. It seems that the facility has changed hands. My mothers room is so dirty, and saw a big roach in her room that is still running around I'm sure. Under staffed. Mom sleeping in a wet bed and smelly sheets. Have to ring buzzer forever to get a nurse in the room. Not getting her appts made for her. It infuriates me . They are understaffed. They aren't up to code with having enough nurses. Her bathroom is dirty. Seriously going to look for another place if nothing is done. Don't let your loved ones live there. The patients deserve more dignity than what they are given. Disappointing.Not getting diabetic diet needed.

Consider the rehab portion of the facility a danger to resident health. The staffing is low. You can press button requesting assistance and wait over an hour. The food is generally served lukewarm or cold. Therapy department is excellent. The nurses giving out the medicines are normally excellent and provide meds on schedule, but there are times that medicines are delayed for several hours. Meds supposed to be given at 7 am may not be given until 11 am or as late as 1 pm. A response from a nurse when asked for reason: complain to management, they don't provide enough staff. There are times when the aids have to go to the other side of building to get meals and no one is available to assist patients. Find someplace else.

There are no words to describe the shoddy, minimal care that my father received at this facility. He was taken there after surgery, needing skilled care, as well as rehab. He never received sufficient care from the Physician Assistant or the attending Physician. Never once got to speak with the attending Physician. I was told by the staff that they video chat with the Physician. The Physician Assistant was rude and non-caring. Very unprofessional talking about patients and family members to other staff in front of visitors. The care he did receive was given by a FEW good nurses. They are very short staffed there (working long hours and as many shifts as they want). After two long weeks, his health had declined. I was told by a nurse – (not saying her name, because they get in trouble for recommending a patient be transferred to the hospital) get your father to the hospital, there’s something wrong. Fast forward – it’s been 2 weeks, he’s still in the hospital. The physician assistant’s negligence could have cost my father his life. We are heartbroken by our decision - Princess Anne Rehab. Don't allow your loved one to be sent here. This place is a nightmare.

We have been impressed with the staff here. They are kind and responsive and there is more staff present here than I’ve seen present in similar facilities. My Dad has been here several weeks, hoping to get better and go home soon.

The staff were so friendly but You have to press the bell and wait a long time for someone to help you. The last time I called it took 1/2 hour and my neuropathy foot was dying in the commode. Anything you ask for takes a long time. The night shift lady told me to just go in my diaper because that was she was told. The therapists were amazing! I cannot praise enough for the RNs and Manager. They’re very accommodating!

This place is horrid about taking of patient especial at night the night staff told this patient to pee in there diaper because they did not want to be bother to get this patient up so a diaper rash happen because of it. You call the night staff and if they come into your room it is with a bad attitude due to you bothering them, there were times that he could not get any assistance especial at night. The place is always short staffed and the staff they do have does not follow the DR. direction, they do what they want. One day they were unable to get morning meds to the patients till almost 11. There is times you call up there an no is able to answer the phone. You talk to the administrator she will give you BS answer an not try solve the problems, just tells you if you do not like here transfer out to another rehab. For a few weeks they did not have a Director of Nursing on staff. The night staff is the worst they do not care about the patient only want to put time in. There are a few that care but most do not care just putting time in that is the impression I get. Place needs to be shut down until they have enough caring staff to take care of patients especial on night shift. The rooms seem good and the dining room appears to be ok The PT & OT department is outstanding and do very good job at helping the patient get better. Do not want to leave my name for fear that my family member that is still there that they will take it out on that family member.

If you want clear communication on patient status or real care i suggest taking your relatives elsewhere. No communication. Lack of care. Ignores simple requests for patients. Total 360 from previous care. Highly disappointed

Very sad to have only one phone for 30 residents. I let the phone ring for over 10 minutes. It's time to modernize and keep families on touch.

My husband had dementia which they were not at all equipped to handle and probably should not have accepted him in the first place. Always short staffed, the month he was there I think he was gotten out of bed into a wheelchair twice. Room checks were not frequent enough and there were times that he could not get assistance with eating his meals or being changed. The chain of command and management contacts were not clear and I never knew who I should be asking for what I needed. They did have some good med techs however and the PT/OT dept was very good.

The staff is excellant. They are extremely caring and display a high degree of professionalism. The facility is clean and kept in good repair. The nursing staff ensures each patient is well taken care of. The physical therapists and Personal therapists are fantastic and work with the needs of each patient

The end result of my mothers 22 day stay is she was asked to leave the facility for failure to participate in PT. When I arrived to take my mother home she was laying in a diaper and shirt covered in fecal matter. She was taken by ambulance to Sentara Princess Anne Hospital BEFORE we even left the grounds (not one person assisted me getting my mother out of the facility and into my car), admitted to the IMCU with a BP of 88/47, elevated WBC and immediately started on IV antibiotics for SEPSIS and a BROKEN BACK....yet this facility MEDICALLY CLEARED HER. There was a reason she wasn't participating in PT.....she was rotting and had sepsis.

I would not ever send any of my family member here.My mom went there for a broken arm and had a hip replacement and end up getting pneumonia, kidney infection and sepsis shock.She is now in the hospital fighting for her life.I ask the doctor so many time to do blood work and told her she was still wheezing and she said everything was fine and just gave her a breathing treatment .Rarely did she come to check up on her.And she just kept saying i just checked on her the other day she was fine and with all the stuff she has no she is not fine.She said she was going to do blood work but never did .Mom would stay wet sometimes up to 4 hours at night.Her legs was leaking fluid and sometimes blood the bandage should of been changed every day but wasn't .This place is so short staff u are lucky to have anyone check on the patient especially on the weekend,I have no use for this place and would not send anyone i know there

Fair warning, my mother was sent here for rehab after a lengthy stay at Princess Anne Sentara hospital which I also caution people about. This facility is very nice but the main problem is the administration staff is terrible and a bad attitude from above rolls down hill. They did little to help mom because she has some dementia. Mom was still sick upon entering the facility And they allowed her to lay in bed for days with pneumonia which is a very bad thing and did little to get her up. 26 days later they give us paperwork saying they have to end her care because " There is no evidence of medical complications or medical reasons that require the skills of a professional nurse or therapist" even though I was insistent that she was sick and needed medical help. I removed her from the facility several days early took her to a different hospital and she has been in ICU the last 4 days and is of course still bedwridden. This is a typical we want your money only place with an attractive facade. I returned to get a copy of her medical records and they still showed her as a patient in the facility after she had left. I will be filing complaints with anyone and everyone that will take one.

A friend is in Princess Anne Health & Rehabilitation Center, and it was nice and clean, and they're very helpful to the patients. The rooms are nice, clean, and roomy. The dining room was beautiful, nice, and clean.
