My mother-in-law is at Autumn Care. My mother-in-law is bedbound, so she needs 24-hour care. We love the nurses. We love the quality of care. It is just the right fit for us. The nurses even text you with things like, "Hey, we think that she's coming close to the end. You need to get up here." So even on that side of it, I love it. I love the activities, too, that they have. They have activities scheduled every day. There are probably about four different activities throughout the day, and they can be as simple as music, bingo, arts and crafts, and things like making birdhouses. When the weather is warmer, they have old cars come out. Then they push the people around the facility to see the old cars. They are engaging with the families, too. They encourage you to be there. They do great with their rehab. Their rehab program is personalized, and they come to your home. It is a really great facility. My mother-in-law is extremely picky, and their food is decent. They also have snacks. If they don't like what's on the menu that day, they can always request something like grilled cheese, soup, or peanut butter. The only thing that's lacking is that the walls need to be freshly painted. The rooms are large and very clean. Everything is very, very clean. We have the capability to switch to Medicaid, and that's the biggest blessing.

Autumn Care Of Mechanicsville was clean. What I didn't like was that they had a lot of patients there, but all I saw there were nurses standing around talking on their cell phones. The person I was checking out while I was there, gave him cold food. The food was really cold. They had rec rooms, physical therapy and little bingo days, things like that. They had activities. So I think the activity part would be great.

There were many issues with this facility. Evident to those who work in the field or caregivers whose patient has been in and out of these facilities for years. First, when my parent was admitted there was no facility orientation and my parent was given a broken meal tray on which to eat. The drink and food would not stay on the tray. Secondly, an incident occurred wherein n employee made my parent and I feel unsafe. The incident was shared with the direct supervisor AND the facility administrator, Pamela. Both were dismissive, offered no apology and offered no solution to prevent the incident from occurring with us or another patient. The cleaning crew discarded my parent's personal items because they were deemed prohibited items, however, I was not aware that air freshener was prohibited as we never received an admittance orientation like other facilities. A friend of mine recently looked into Autumn for their loved one and was told upon admission they would be sheduled for a meeting to discuss the facility and expectations. Glad to know something good came out of the wrong, the bad and the mishandling of my parent and the safety concern. Shared rooms and shared bathrooms...with COVID still claiming lives, this should not be.

If you want a place that has beautiful land scape this is it. If you are looking for a place that has over worked staff that really doesn't care about your loved one this is the place to be. A couple nurses acted like they cared. One aide really cared and cried. The head nurse never said a word to me when I went to collect my brothers things the day he passed. This person would give very short answers like I was bothering her. The last day she never came to offer any condolences while collecting his belongings I went to the desk and some what exploded. How unprofessional. You work in a nursing home lady at least pretend you care. This is the head nurse. I complained to their social worker who blew smoke up my... while using catch phrases like "reach out to you"They told me the administrator would call me. Yea right. I also called director of nursing. Never heard a word from her either. This is a shame and was so disgusting. This person was his nurse the whole time he was there. No caring Angels at this place. Some will understand that comment.

I am a resident at Autumn Care of Mechanicsville and I love it here. The staff is very hard working and go above and beyond to help enhance my quality of life and ensure my well being. Brenda is always so attentive. She is dependable and considerate. Jenny and Susan are also amazing. Words do not adequately convey my gratitude and thankfulness. Thank you for all your compassion!

My brother-in-law was at Autumn Care. They accept Medicaid. He went there for rehab after he was in a hospital stay. It's a very clean appearing facility, it's big, and it's bright. I think they had staffing issues like anybody else does, but they seemed nice enough. The private rooms were really nice and big. The semi-privates were a little snug, but I guess the private rooms were actually semi-private with only one bed in them, so I guess that's why they look so big. I don't know what they have as far as activities. I remember he was kept pretty busy with the rehab part of it, but they didn't do too much on the weekends. He did get his rehab through the week. He never complained about the food. The dining area was very nice and very big. He loved the food. The facility was impressive.

Autumn Care is a nice facility. During my stay I have been treated well and with respect. The therapy team finds a nice balance between your medial/physical limitations and pushing you to do more. The therapist didn’t make me feel like new tasks were impossible and helped to set realistic goals. I wish I could have done therapy even more like twice a day.

My uncle is currently at Autumn Care of Mechanicsville and our family is so pleased with the experience. From the very beginning of the process the staff has been very friendly and attentive. The Admissions process is so well explained and they make you feel comfortable with knowing that your loved will be well taken care of. The nursing staff does a good job of keeping you informed as much as they can.

I came to Autumn Care after my surgery and I'm very glad I did. The therapists here treated me with respect and what they say is true, if you do the work you will get stronger! They gave pictures and diagrams to help see what to do and it gives me the ability to try them on my own if I feel up to it. The exercises helped me feel prepared to return home.

Autumn Care of Mechanicsville has very helpful staff, therapy and nursing. They are the reason I keep coming back here. When the hospital asked where I wanted to go I said Autumn Care! Where all of my girls are.

The worst place to put your loved one. My mother went here for 20 days and she will tell you herself that it was a nightmare! My mom was told to stop pushing the call bell after waiting an hour in-between pushing it. She could wait up to two hours to be taken to the bathroom. One time the nurse was so lazy she made my mom use a bedpan and spilled urine all in my moms bed, the next morning my mother was accused of urinating in the bed. This upset my mother to the fullest. My mother could walk before going in there with a walker but yet they always pushed her around in a wheelchair EVERYWHERE. They had a doctor see her everyday but yet she left there and one day later was rushed to VCU to the ER with Congestive Heart Failure. This is not something that happened overnight! They knew she couldn’t breathe but yet missed this? They kept telling us she could do this and that, when she got home she could do nothing , she was too sick! No rehab on many days when she was there for just 20 days. A couple of the nurses were great to her but only a couple. The wheel chair she was pushed out to our car had feces marks all on it and we were afraid she got it on my Aunts car. Got home and she had nothing on her pants. They brought her out in a filthy chair with dried mess on it! Her oxygen thing broke one day in her room, she pushed the call button, she told us her fingers were turning purple, it scared her to death. She slept in the wheelchair three days straight. She said the food there was horrible. I could see it through the window and some of it didn’t even look good. She said if you have rice one day, you will have it two or three days more with different toppings on it and the food was always cold. Her neighbor next door was one of my high school friends mom. She was treated the exact same and can tell you the same. On the day of my mother’s release she sat in the wheelchair and watched the nurse make up the bed without changing the sheets for the next person to come in and sleep in! For the two nights my mother slept at home afterwards she had to have the door open in the room and a light on, the scared her to death. At the hospital she got back to her usual self of sleeping in the dark again thank goodness. She is now in another rehab facility because we are starting at square one again with rehab. I received a phone call in my car on the way home after putting my mom in the car of my Aunts because her car was lower for mom to get into. The phone call was Autumn Care stating that one of their staff members had covid but my mom was quick tested and was negative. We all know the quick test are 50% correct. My Aunt she was riding with has Kidney cancer! I was furious. Thank goodness mom didn’t ever show signs of it. My mom came home with another patients medicine as well. With my mother coming home late on Thursday we had set up home rehab to begin to come as soon as possible. An Angel of a nurse from the home health care team we are going to use for rehab at home came that Saturday morning to access my mother. She told us that if we didn't get her care ASAP that she would not be with us in another week. How could this facility send my mother home so sick and never had picked up on it. With Covid-19 we could only go by what they were telling us on the phone which all ended up being no truth to it. Another thing that you need to know is that upon sending your loved one home they will tell you that your loved one is suffering from dementia, to try and cover up anything you may be told by your loved one. She was tested in VCU in many ways and they were shocked how smart and “with it” that she is. Beware of this facility. I called and left a message with the head lady there and she didn’t even return my call to discuss how they treated my dear mother. To think they missed her having CHF and having to go to the hospital for seven days right after leaving there for one day is a sign of their incompetence. Now rehab again!!!! Not there though.

My mom was a patient for 5 weeks in rehab after a 3 week hospital stay. We had visited and were impressed by the facility and what we were told. The rehab unit was amazing. The therapists were all extremely nice, knowledgeable and hard working. They did a great job getting my 92 year old mom back on her feet, eating regular food, and walking with a Walker. Could not have been more pleased with this department. Sadly, the rest of the place didn't live up to that expectation. Our family visited every day for several hours. We found the medical care to be spotty and unreliable. We could never get accurate info on what meds my mom was taking. My mom had a pressure sore that a wound doctor treated before leaving on a vacation. However, no one systematically followed her during his absence. All communication with the doctors came through the nurses who were spotty at best in their response to our questions. Some were downright rude and unhelpful. A couple even told us it was not their job to help mom when she had trouble getting to the bathroom or needed cleaning up. Likewise, some of the aides, especially on weekends, were hard to find and even harder to get to help. In fairness, many seemed overworked and there wasn't enough time to meet the needs on the unit. However, that was not comforting to us when my mom reported asking for help to get to the bathroom and was told to urinate in her bed because she had on a depends. Nor, the many times she needed help getting from the wheelchair to the toilet, rang the call bell, and after waiting for up to 20 minutes had accidents requiring complete clean ups. We had to asked several times to get her bathroom cleaned after some of these accidents and still found dried feces on the toilet chair, floor and wall. She was on a pureed diet which was often unidentifiable and unappetizing. We tried to discuss improvements with the staff and got some response but really very little change. We were faced with the prospect of having to keep her there after rehab even though it was very expensive. By the end of her stay we would not have allowed that. In fact, we did move her to a different facility at 1/2 the cost and have been very satisfied there. Again we would rate the rehab far higher, possibly a five overall but can not rate the facility any higher than 2. Edward Leslie

My mother has been a resident for the past 3 years. The first year or so, it was great. It seems as though ever since they were bought by Saber Care, the level of service/care has gone down. To be sure, all nursing homes have their issues and the only way to ensure that your loved one is well taken care of is to visit frequently. That being said, the food quality has decreased as well as timeliness. Dinner is supposed to start at 5:30, however, the residents are lucky to receive a plate of food before 6pm. If you're in the dining room, great. However, if you're not able to go to the dining room, you are served last, after the residents in the dining room. At times, by then, your 1st choice may not be available. Also, it can take quite some time for a resident to have a nurse respond to a call bell. My mother has received reasonably good care only because between my sister & I, we keep close tabs on her & do not hesitate to "go up the ladder" with a complaint if warranted. It makes me sad for all of those who don't have family visiting regularly to stand up for them. I had to have the actual cook come to the dining room last night because it was 5:45 and no food had been plated at that time; they were just getting drink orders from the residents which, in my opinion, is unacceptable. My mom couldn't go to the dining room because she had a procedure done yesterday on her right, dominant, arm. I was able to get her a tray of food that she wanted & I delivered it myself so I could be sure she got what she wanted to eat.
