My sister is in rehab at the Santa Fe Health and Rehabilitation Center. Her Alzheimer's is kind of declining. And they have shown good care for her. I have talked to them, and they have promised me they're going to keep her where she is, her room and everything. She seems happy with the caretakers and the CNAs that are there in her hall. They are wonderful with her. So, I'm thinking at this time we're just going to stay there. She's had a lot of changes and a lot of things and I think it's caused her declining in Alzheimer's. But she seems to be okay and she's happy where she is. So I don't want to risk moving her again and losing her more because let's just say she forgets a lot. And when I say she forgets a lot, I can be there in two more hours and she doesn't remember that I came. The majority of the staff are fantastic. There are some exceptions, but that's everywhere you go. So the ones I have direct contact with are great.

The rooms are standard size for this type facility (two beds/roommate) and are kept clean and sanitized daily. I requested a private room and was guaranteed to have one available at the time I arrived later that evening. I was entering the facility on rehab status for a minimum of two weeks then would be re-evaluated for an extended stay. The dedicated rehab area within the facility is fully staffed, has up to date equipment and well coordinated with emphasis on patient safety. I stayed for 30 days and felt that my needs had been adequately addressed. The administrators, nurses, specialists and staff are punctual and polite; I had a very pleasant stay.

This is the second time my Husband has been at Santa Fe and the staff continue to be so amazing. All Staff Members were caring, informative and professional. In fact, he liked them so much he has requested to return in the future, if needed. I didn't list names as there are so many great people I didn't want to miss anyone. If you're looking for wonderful care for a loved one this is the place.

My one star rating comes after many months of thought and heartache. My mother was a resident here on hospice care. The night before her passing a nurse called my mothers phone trying to reach the family to come in due to her health declining and her not getting out of bed. Unfortunately for me her phone was next to her on her nightstand. Therefore, I did not receive the call. I am not even sure why they called her phone as I was listed as emergency contact. They also didn't call out hospice as this would be a time of need for CC care. The following morning her hospice nurse arrived to take report and was told my mother wasn't getting out of bed. She went into the room where my mother was and she turned and looked at her hospice nurse and took her last breath. The hospice nurse then called a code blue and the ambulance was called. I was then called by hospice, whom asked has anybody called you? I said no, what is going on? They said my mother had been down for 20 minutes and they are currently performing cpr. I arrived 1 minute too late. I could've been there for my mother had I been made aware. Had someone just called ME. But it never happened. After they pronounced her dead my sister, husband and myself went to talk to the DON to find out what exactly happened because I had no warning that anything was wrong. They had no report to tell me. My mothers oxygen in her room was on a 9 and the hospice told me it was on a 6 so someone turned her oxygen up. And someone didn't take an accurate report. My mother died alone and it is the fault of this facility. That will never sit right with me because you stole my last moments with my mother by not simply calling the correct number or trying to call again or even informing hospice before they arrived for their usual visit. And now, I am left with a bill for her stay because it was not taken out of her social security check and now social security has taken the money back. I will tell anyone and everyone from here on out, if you care about your loved one, find anywhere but here to take them.

I DO NOT recommend you let your loved one admit to this facility for any kind of care.. My mom went a week without staff helping her to bath. She infrequently was able to brush her teeth. She waited hours for her pain medication. I was never given a plan of care or a goal for her care or status updates during her stay. It was very difficult to talk to staff or accomplish communication.. I was never given alternatives if I felt she was not ready to discharge home. Unprofessional and poorly run. Horrible experience overall!!!

The staff is pleasant but slow and lazy. They’re also very argumentative about orders. Take own meds for at least 5 days; took them 4 1/2 days to give me my pain meds. Food is horrible and always cold...kitchen is a joke. Never follows orders. Like allergies etc. Their budget must be a thread, don’t have enough pillows, count trash bags! I Love believe that you hun I will never recommend this place. Oh, Rehab is so slow. Think they want to work with you 2x the time. After 2 weeks they are just now really trying to do something. No amenities. Rooms are very tiny, no chairs, table. Only activities seem to be cards? Not sure they never were clear.

My long term boyfriend was in Santa Fe. The first room he was in was not good. The roommate, who had been there a long time, had the room full of his things. He was hard of hearing, so the tv was on and up loud all the time. My boyfriend took s turn for the worse. The facility moved him to a room by himself, Big difference! I do think the aides could use additional help (More staff) but the nursing staff was awesome. They were so attentive and kind. I am so thankful that he was there.

I want to say thank you to all the staff at the Santa Fe Nursing home. My Mother has been there for 2 and 1/2 weeks and they have made her feel very welcome and cared for. My Mother is a very outspoken and a little hard to handle sometimes but they have taken the time to try and figure out what works best for her. Thank you!!

My Mother was there for 20 days and she was not given her pain meds for her back surgery pain in a timely manner. At one point she was told they were out . I had to go up to the nurses desk and ask them to give her a shower on the third day she was there. I would not recommend this place to anybody

My husband is in this place right now. I am not impressed at all with it. The rooms are nasty. They have animals running around. For someone like myself with severe allergies its wrong..place is suppose to be sterilized. And clean. As for the staff. Well one word RUDE!!!! My husband is so heavy medicated its pathetic. You say something about at and you get the look as UR bothering them. He fell asleep in bathroom AND NOT ONE PERSON CHECKED ON HIM FOR 5 HOURS. MEANWHILE his supper was on table in lunchroom cold as ice and wanted him to eat it that way another thing is the can ONLY TAKE SHOWER WHEN TOLD TO. AND U HAVE TO SHOWER WITH EVERYONE. NO RESPECT OR PRIVACY AT ALL IN THIS PLACE..SO If you love your family member or whom ever they maybe do not place them here.

I don't recommend this So-called Nursing/Rehab Facility to anyone. I have been here since 1-13-17 after a total knee replacement and I have to wait hours upon hours for Meds or for them to put me on the CPM Machine. I am reporting them to the Board Of Health

This facility is not the newest in town. However, the facility is great! The staff are caring and the residents are happy! These two things are huge!!!! This place just has a great vibe. I highly recommend this facility for short term rehab or long term care.
