I had terrible experiences with this facility. They neglected my loved one; she was always thirsty, legitimately. She contracted a UTI because of it. She could not help hydrate herself. After her passing, I discovered a huge bed sore had developed that she had never had before. They did not let us know about that, nor how fast she was declining and would not let us go in and see her when she still could recognize us. They did not call or alert her sisters that she was declining rapidly. I called the evening before and the front desk lady did not like that I told her we need to call our reps who bring down these draconian rules of isolation for these residents, and she hung up on me, multiple times. After my loved one passed, I called to register my complaints to Heather, the manager, and she eventually hung up on me as I told her I had one more thing I wanted to say. I guess that is how they deal with unhappy people. I never shouted, never cursed, just simply stated the facts of their negligence in regard to my loved one. I would NEVER TRUST THESE PEOPLE TO CARE FOR YOUR LOVED ONE! IF THEY TREAT THEIR CUSTOMERS BY HANGING UP ON THEM MID-SENTENCE, HOW CAN YOU TRUST THEM WITH SOMEONE YOU LOVE?!!!!

I only came here after a severely broken leg for rehab because I didn't know of another at the time. My father had come here for rehab after back surgery and the pt personnel were good to him. I WILL NEVER EVER COME HERE AGAIN NOR REFER ANYONE I know to this place. Food is terrible, cleanliness and operation of equipment is horrendous. Wouldnt send my dog here.. I understand the cv19 quarantine, stuff. However how can u put someone fresh from hospital in with someone from another hospital on oxygen and noncompus mentus.? 4 hrs before finally asking where call LIGHT was.. pulled night stand away from wall only to find mask, glasses and debris, no activities. No available amenities outside of a walk/roll in shower at present. I took pics of condition of base of overbed tray, and wheel chair as given to me. I can only imagine the illness or infections this was capable of causing my 45 staple incisions!!. Eggs never over easy,and with eggshell in them food always something being substituted. Why bother giving anyone a choice when its whatever is available? I'd like to add the photos but not sure how.. but as a retired nurse I was absolutely appalled.. was basically strong armed in to staying and took 4 days for social services to get back to me on my preferred transfer. I dont care how much less it maybe out of pocket after insurance, it is NOT worth it. IT should probably be shut down just from the pics and this review. Can you please tell me how to post these photos?? Thank you ..

If I could give less stars I would. Smells awful. Rooms filthy. Mean staff. Be prepared to stay and defend your loved one. Medicine was denied. Mom left dirty, crying and hungry. The head nurse bullied her and nothing was done about it. Pathetic.

Don’t come here unless it is the last resort rehab is great but the care is a nightmare I have left messages for the administrator to call me but you only get call backs if they want to not professional here. I had heard about this place before we came here all the bad stuff but I felt we could handle it because of the good words about the rehab you have to watch them like a hawk. One time Gary’s oxygen ran out. When I stopped someone and asked for help I was told that I need to find his nurse they have not always given him his medicines and they hate to name off the meds that they are giving out was told to check with the head nurse they did not always put his c pap machine on him they took him to his doctor’s appt late and he had not had a bath in several days he did not always get his breathing treatments and they told me on a Friday he was being discharged the next day I asked why when he was improving. They said my insurance denied more treatment I called my insurance company they said they did not get another update on his progress so he had to be discharged when leaving I was told I had to push him out someone finally did it for me. But he had no idea if gary was to go home with oxygen or not. He finally checked on that. Gary only had intense rehab for about two weeks not the entire time he was there never again will we use this place

It may look nice and the hospital may even recommend this place, but there is not enough room to explain the horrible experience. After a survey my family came for rehab. The nursing staff laugh and play all day and ignore you as much as possible, they don’t give you the medications they say they do, at night my family had t beg the staff to give her pain medication which took hours. Her cal light was mysteriously taken away during the night while she slept. But don’t bother complaining to the administrative staff because they will be hiding in their offices. Not one time did the administrator bother communicating with me about concerns. My family ended up getting an infection and going back to the hospital and we went elsewhere.

My father-n-law has been a resident at Providence Park going on 5 years. I chose this facility because it was clean, staff/ nursing is caring and the rehab is exceptional. He ended up needing long term after rehab and I could not say enough good things. The facility is not perfect but they care about their residents. I have worked in this field for a long time and their quality of care and rehab out does any other facility in this area.

My wife was recovering from a stroke and needed help for everything, especially getting to the bathroom. She often had to wait too long, as she also has bladder incontenince. There was an error on one of her prescriptions that I had difficulty making the nurse understand that she could not take that medicine often, as it caused hallucinations. She required daily laxitaves, which staff insisted she was getting, but after I took her home, it took me 3 days to get her bowels moving. She was dangerously constipated. Finally, on the money side of the issue, I took her home after 3 days withhout dr. approval, and Providence billed us for a long time for co-pay for 7 additional days that she was not even there.

My husband was in Providence Park. They were very crowded and full. They did the best they could, but their food was not very good. They specialized in doing rehab, but they had permanent residents, too. He was there for two months. I thought they did a really great job with their therapy. All of the therapists seemed really interested and worked hard. It was a much busier place with more of a commercial feel. It was not homey, but everyone was very nice. Their rehab was good, and they were very kind to him.

I wouldn't even have given this facility 1 star but I couldn't wire s review unless I selected a rating! My father was placed here to recover from a stroke. Getting information was difficult, there was no follow up from anyone other than the business office on payment. He actually declined while here and didn't make progress until we moved him to another privately owned facility. Many of his belongings were stolen and never replaced. I would encourage everyone to look past the fresh paint and smiling faces when shopping for a facility. Take your loved ones somewhere else.

No communication. Patients and family don't have a clue what is going on with their care. 1 care plan meeting the whole stay. Roger the social work is very rude. The administrative staff are very uncaring. Would not recommend this place for rehab. My mom went when they first opened and it was great. Not so great anymore. Staff is always walking halls on their cell phone. While visiting my friend for several hours a saw a patient wheeled into his room facing the corner and left there. No one ever came back to put him to bed nothing. Very sad.

This facility was recommended by one of the local hospitals for my mother for rehab, and I will be sure to share this information with them also. I found the staff to be very unprofessional, most of the time walking around with their phone in their hand and seem to be agitated if they are asked to help. I have heard people crying for help from their rooms, and no one is in a rush to help. My mother could not walk unassisted and had to make numerous calls and finally had someone help her to the restroom after 45 minutes. After 3 days, I found the linin closet so I could change the sheets on her bed because in those 3 days, she was not given assistance to shower. If in the future she would need to return for therapy, it will not be here.

my dad has been in there since 9-11-2015 . I would go everyday and spend 5-6 hrs with him. during that time their policy is to check on the patients every 2 hours. not one time would anyone check to see if his underwear needed changing or if he had water to drink. if this was going on while I was there, I could only imagine what was going on when I wasn't. so I started switching days going in unexpecting and found my dad in bed underwear full both in front and back, no water where he could reach. other days found him in his wheelchair shoved in front of a tv, dirty clothes on and didn't look like they even washed his face or anything. I complained numerous times even had photos to show the neglect. he also was suppose to have breathing treatments and they quit doing them because they said they didn't want to take the time to sit while he was doing them, because they said he would try to take the mask off. but when I was there and I would put it on him he would fall asleep doing his treatment. on 11-16-2015 i went to see my dad. he underwear full of feces, and this was 11 am in the morning. he was still in bed they never got him up for breakfast, mucus all over the side of his face and bed caked on. so it was there all night long. I called texas health and human services and reported the personal,basic need, medical neglect from this facility. I also reported to the nursing home neglect hotline and told them if they need the photos I had them.

In one word..RUN! Please never send a loved one here. They don't care if their patients are laying in their urine, make them wait hours for pain meds and heaven forbid you ask for something. Best description is rude, sarcastic, uncaring and lazy! The food is unnutrional, horrible and either over salty or drowned in dressing or sauce. The hard boiled egg was probably the only nutritional item I ate.

My daughter and I visited several facilities and Providence Park seemed a clean and an inviting place. It had also been recommended to us so I was able to put my husband in there. The facility is also very well kept. I heard that their food is good, but when I ate there I didn’t find it to really be excellent. The portions were small. They do have activities to do, but my husband chooses not to participate in them.

This facility promises many kinds of good care, but does not follow through with their promises. My parent has been there and I have seen that the nursing staff for the most part simply does not care about patients. My parent has slept in urine soaked bed, been left without food. If a patient can voice complaints and tell what is needed, the nurses might do as they should. From what I have seen, they are extremely apathetic. I could not recommend this facility to anyone looking for care for an elderly person.

This facility was recommended by a case manager at a local hospital. My mother stayed at this facility for over three weeks. During her stay, I was thankful that my father could be by her side on a daily basis because I do not believe the staff at this facility would be able to provide the care that she needed. Most of the nursing/support staff are apathetic and work at a snail's pace. The LVN that provided my mother her daily medication consistently appeared unkept and his nails were always dirty. Facility does not separate residents with mental/emotional issues which can be noisy and upsetting to those residents seeking a calming environment. Food service is slow and servers do not serve residents in a timely and orderly manner.

The physical plant is beautifully decorated. Very impressive; however, the dietician needs to be more skilled in proper nutrition and presentation of food. The women servers have loose hanging hair - food is not served hot -not even warm. It isn't unusual for there to be a shortage of food i.e. soup and or dessert-substitutes for soup are made such as a ruined baked potato (we recently was served a ruined/black potato, wrapped in crumbled (looked like used) foil. It seems that unless there is a family member present the patients would not have an Advocate to warm their food or to protect what they were served. Please do not let the reputation of this beautiful facility become tainted by the dietary negligence.
