The Azalea Trails Assisted Living & Memory Care was nice. I visited their assisted living for my mom and it didn't seem like anybody was there. Everybody was in their room whereas in the other place we looked at, it was more like a community which I liked. They had happy hours and stuff like that but I didn't see anybody there. I thought the grounds were very beautiful though. The room I saw was very nice, and the person who accommodated me was very nice and knowledgeable. It just didn't seem social. It might have been just the time that I was there but I didn't see anybody out and about like you do at other places. It did seem like it was more upscale compared to other places I've seen.

The tour with Azalea Trails Assisted Living & Memory Care went very well. I was very pleased with the layout, all the amenities that were offered, and how they treated their guests. It was pretty nice. If we would be able to get in there, that would be one of the top-rated places I would like to get into. I like the different hobbies that the seniors may be involved in, the outings that they go to, different venues, they have gardening, they have a media center which they also use as a workout center. They pay close attention to the seniors with their own particular needs and their interests, and they pay close attention to that. I was impressed with that. Janet was very knowledgeable and very pleasant to speak with just like a normal person and just genuine and honest and very transparent with everything that goes on at Azalea. It was a very nice place, and a very, very well thought out plan for the residents that are there. They were trying very hard to help my dad out, so I was impressed by that.

The people at Azalea Trails Assisted Living & Memory Care were very friendly and very helpful. The memory care rooms were very nice and very well kept up. It almost looked brand new. They've got just about everything. They had a place to do your hair, a nice place to eat, and nice activity rooms.

We chose Azalea Trails for the memory care unit for my MIL. She had Lewy Body Dementia. We were given a great song and dance about how wonderful it would be and that they could provide the level of care she needed. The marketing, website, tour and talks were great but we soon realized they were all a lie. They were not able to provide the level of care promised. Workers did not like their job and it showed. There was no compassion and love just sighs and grunts and anger that they had to take care of her. We would be told she had gotten a shower yet all of her towels we sent were still on the shelf clean? How do you give someone a shower and not use her towels and her hair isn't wet. I could go on and on... The director was serving two rolls as someone was on maternity leave so they were short staffed and very little supervision. Yes this was during a pandemic, but like others have said, you can still provide good care during a pandemic. All we wanted was Azalea Trails to do what they said they were going to do! Just do the minimum. We could barely get that. It is very sad. Our loved one ended up getting Covid-19 from one of the workers and passing away shortly after. Overall this place looks great on paper and online but it is not! At least for the memory care side I do not recommend. I feel like we were lied to and deceived about the care they provide. One positive is the food. Our caregiver told us that our loved one totally loved the food there. Hats off to the chef and the delicious food. If you are faced with the decision of having to place your loved one, do not put them here. There are much better places in Tyler.

I went on a tour of Azalea Trails. The apartments were very nice, they were updated, they were functional, and they were attractive. The dining area looked more like a restaurant.

I was in a desperate situation to move my dad to assisted living. The Azalea Trails staff returned my call after 5 pm on a holiday. The executive director, Ms. Recynthia, understood my situation and guided me through the entire admissions process. She took a very high stress event and turned it into a beautiful story. I cannot thank her and her wonderful staff enough for all they do for the residents and families.

Azalea Trails is a small community, and the staff is very friendly and attentive. My mother's room is spacious, bright, not too big, and she has all the furniture that she wanted to bring. She likes to watch people play games, but they have lots of activities going on. It's worth the money.

My family member was at Azalea Trails Memory Care for almost a year. The staff was wonderful! You could tell they really loved him. He could get really unhappy at times packing to go home. They were so good to redirect him. He got pneumonia at 91. The staff watched him closely and he got over it. Most people at that age die when they get pneumonia. I credit it totally to the care. I'm convinced if he had been anywhere else he would have died. The last two months he was bed ridden. He had no skin tears at all, another only for the care. They really care about their residents. I give five out of five!

Azalea Trails was the one I was most interested in. Their rooms were really pleasant, but they didn't have enough activities for my mom. The place was very small, nice, and quiet.

Azalea Trails has assisted living on one side and memory care on the other side. It was a smaller place, and the care ratio is 5 or 6 residents per nurse. As far as memory care, I liked it; it was one of my first choices. The director was very knowledgeable. They do a lot of activities where they include residents so they feel useful. They do a lot of games and had outdoor areas where they could go and still be safe. They have nice rooms in the assisted living area, some with a patio, and pets could visit.

Azalea Trails was clean and it was a good facility. The staff was nice. However, at the time that we needed it, we weren't really pleased about the location of the only room available, it wasn't furnished.

The staff at Azales Trails make all the difference. My grandmother was a resident here and the staff would call my Dad to let him know about certain issues. When my grandmother seemed particularly lethargic and perhaps a little dazed, one day, the staff alerted my Dad, as well as her physician. It was determined that a newly prescribed medication was not interacting well with her existing medications. The new medication was changed and she was soon back to her normal self. By paying attention and by knowing their patient, the staff was able to catch this problem and have it resolved quickly.
