my precious mother was at autumn leaves for many years. and we loved it. it is very well kept i never walked in and smelt nothing and that is saying alot.the nursing was wonderful many witch become your family.many of us had our favorite aids but mostly all were ok. we loved our private room with own bathroom. the food was not always the best but always had plenty of it. i would recommend autumn leaves the best .

When my mom passed away the aides and nurse were upset and angry as I sat in the room with my deceased dear mother The hospice nurse was on her way and via phone had told the nurse and aides to move my mom from her recliner where she died in to the bed and to also change her soiled clothing.They were saying right in front of me why do we have to, she's already dead.They didn't want to have to lift her dead body into the house lift.Totally disrespected my sweet mom even in death.I was horrified and I will never forgive them.That tells you everything you need to know about the laziness and non-caring staff in this nursing home.DON doesn't care so why should employees.

Autumn Leaves was a very friendly and very diverse place for rehab and long-term care. They had a lot of equipment to work with the patients. My father lived there and loved walking there. They were really good. He was in a private room with a private shower and bath. It's a new facility. In that particular wing, most of the laundry was done by the family, but they had a laundry area there as well. It's very modern with nice linens. I met a lot of the staff, who would have care meetings and were really into giving information about your loved one there. They made sure that everyone was fed. They're very informative.

Our 96 year old Mother recently passed away after living in Autumn Leaves Nursing Home in Henderson, Texas for nearly 10 years. We feel our long term experience with this home might be helpful to those who are considering this facility for a loved one. Mother entered the home in relatively good health and loved her private room and bath which was decorated with many of her personal things. For many years she enjoyed the variety of activities offered and made many new friends. After her 90th birthday her health began to decline and she began using a wheelchair. As her dementia and Alzheimer's grew progressively worse our sweet Mom became increasingly difficult to deal with, and often combative. In spite of this, the Autumn Leaves staff continued to care for her in a manner far exceeding professionalism, always with kindness and compassion. In our opinion, Autumn Leaves is an exceptional home with employees who truly care about the welfare of their residents and are supportive of family members. We realize no home is perfect but our experience over the last 10 years has been extremely positive!

Autumn Leaves is with a beautiful setting, atmosphere and one of the best kept homes that I have visited. They are very well staffed. The cleanliness is amazing. The care of my dad is remarkable and with sincere kindness. After reviewing and inquiring what the star system was all about and how people determine the right home to choice based on the 5-star system, I realized it is not a true reading of what the Home actually has to offer, what the Home is able to provide and most of all what the Home can do my loved one. This gathered information for the 5 star system does not reflect the identity of the Home based on the care that is being provided or the support that is being given on a daily basis to people living/staying in Autumn Leaves from a care aspect.

This facility is very poorly run and we have not been happy with our experience dealing with Autumn Leaves on the care for our elderly family member. Medicare dot gov rates this facility "Much Below Average" with 1 (out of 5) star. The Health Inspection rating is also "Much Below Average" with 1 (out of 5) star. The Staffing rating is "Below Average" with 2 (out of 5) stars. I strongly advise that you seek another facility to care for your loved ones in their most vulnerable years.

My grandmother likes it there. She would not go back home even if she could. She might miss something! There is so much going on all the time. She enjoys keeping me up on the news about everyone who works and visits there. They keep her busy when she wants and let her rest when she wants to. It's clean and friendly. She likes their French fries.

This Nursing home is remarkable. I enjoy going to visit my loved one here. He is so happy living here. We get to sing together while someone else plays the piano. They also have games that the residence can play and enjoy. If he decides he wants to stay in his room and relax he can. He has a TV in his room with available cable. The staff is very friendly and take real good care of each and every one the residence. During they holidays they have Christmas and Thanksgiving parties. Families get come out and have dinner with there loved ones and exchange gifts. We are able to sit and visit for a little while after dinner and discuss family and friends. If I was to every meet someone that's needed a Family member or Family friends that will also need to be in a nursing home or Assistant living facility. I will recommend Autumn leaves because the environment is very relaxing and the staff and friendly. They keep the facility clean and odor free. A wonderful place for the disabled to call home and enjoy living there.

Autumn Leaves is one of the most elaborate places I have visited. My family member lives at AL. The staff is very professional and courteous. I feel that Autumn Leaves is one of the best places my loved one can live at.

Autumn Leaves definitely has that home-away-from-home feeling. I loved that each room was set up like a personal bedroom with colorful quilts and curtains. They didn't have the stale 'hospital like' set up some other places have. The staff was very friendly and took time to go over our grandmother's needs. We even met with the facility's chef/dietary director, who was able to make a custom diet plan just for her. With all the extra amenities they also offer, we kept Autumn Leaves at the top of our list.
