My mother-in-law is in Legend Oaks of Gladewater Rehabilitation & Nursing Center. So far, they've been excellent. The food has been decent. The rehab has been what it should be for her condition. She has a private room, but they don't promise that it'll continue to be private. It's clean, and they change the bedding regularly, and bathe her regularly. They have church and they have singing, and they have different activities. The place has been up-kept and painted. We talk to the nurses and they keep us informed. I can't think of anything that's gone wrong so far.

Facility is clean and odor free. Lunch looked and smelled very good, however, I did not eat there but the residents enjoyed their barbeque brisket.

My aunt was given good care and the nurses were really attentive and helpful. She was there for therapy and they did a great job helping her get back home.

My friend was in good shape upon enterin this facility. Not 3 weeks later she has received her medication late or not at all and connoty. b Her room has cockroomes in them. Her linens on her beds dont get changed. her bed ridden room mated doesnt get fed.

For the past seven years one or both of my parents has been a resident at Legend Oaks. My father spent the last four years of his life there suffering with Alzheimer's. Mom became a resident shortly before Dad's passing after breaking her hip in a fall at home. Six months after she completed rehab from the hip, additional hip surgery sent her back for more rehab, and the need for more assistance than we could give at home, she has been there ever since. Both parents required attention above and beyond the norm for at least part of their time at Legend Oaks. The core staff and management has been attentive, compassionate and truly caring. In a business environment where a mountain of regulations and short supply of quality help impede performance, I have been truly grateful for the level of care both of my parents have received. There is a helplessness attached to turning over the care of loved ones to others and quite a bit of frustration when a loved one suffers even a moment of unmet need. The director and staff have been very responsive to all of my concerns. I can truly say that overall, the people at Legend Oaks have done, and continue to do a terrific job caring for my parents.

My mother was here at this facility for rehab after her fall at home. I feel the home is down to earth and inviting, It does not look like the Hilton but I wouldn't want it to. It's not all about the looks. The rehab she received was excellent. They got her back to better than before. The nursing staff I feel was very attentive and met her needs quickly. I will say that the nurse in charge is not very friendly or approachable, but the rest of the nurses and aides are wonderful. I would not want to have her placed anywhere else. We will be back if the occasion should arise.

Legend Oaks is fairly bad, and the only reason I'm moving my dad there is because they are close. It is bare bones, and the hallways have nothing on them. They have a central nursing station that has this annoying beeping that is going on constantly. It has poor reviews on the internet. I have been dealing with a difficult and testy head nurse who as of today she and I had a little heart to heart on the phone because she was so testy. She was testy to me, she was testy to two people at hospice, and she was testy to my sister. Under normal circumstances, I was seriously considering dropping them altogether and going to somebody else. I do not desire to have to deal with the stress of dealing with her every time I go in there. I even called the business office there, and I have called the administrator there; I have not done that at all with any other place. They need to know that this lady is a pain. She was a pain to deal with, she was a pain to hospice, she was demanding, and she has just made this entire move difficult to start with. I have heard from people who had family there, and they said they have good skilled nursing care. That is the heart of any nursing home. You can do without pictures on the wall, and you can do with it being sterile. It is very sterile and uninviting. It is the kind of place you wouldn't put your dog at, and that is how I felt when I walked in. I also know that in end of life, you have to be very involved, and you have to be there a lot. This is only 8 miles from my house as opposed to 35 to 40 miles for any other. It makes a lot more sense for me to have it out with the nurse and get things on an even plane and be able to be there as opposed to being 40 miles away and not be there. Other than looking at it, I haven't been able to walk through the door. But I just had a horrible one week experience dealing with this woman. The room is dark, sterile, with mismatched furniture and is not inviting. But I've heard good things about their care, and that is what it really boils down to. I don't know what the food looks like, and the dining room is bare and sterile.

My mother is here. I could not ask for a more caring down to earth staff to take care The staff goes out of their way to make sure she is taken care of. It is a nursing home you will have problems from time to time, but I feel the nurses in the office Kelly & Judy take care of the problem promptly. I feel often people are hard on nursing homes and employees. It is what it is. If you loved one is taken care of, and happy, then I say that's more than enough. I cant take care of her at home and if you have put a loved one in a home then obviously you can't or don't want to either.

This company doesn't care about it's residents. They will lie and tell people anything they want to hear to get them to put their family in this place. A lot of promises and big talk, then after you find out its not all they say it is. Food is cheap starchy , and thrown together. Loud place, shouting over an intercom all day. residents do not look happy. The staff act like they are scared for their jobs. Not too many smiling faces .nurse aid has way too many patints and nurse seems to spend the whole shift doing computer work.I wouldn't want to live here.

I have a close family member in here. I talk to her on almost a daily basis. When talking to her the other day, she was in a lot of pain and had rang for someone to come in. When they did she let me go but forgot to hang up the phone so I could here what was going on. She asked the CNA to help get her up as she is paralyzed and cannot move her feet but experiences leg cramps daily that only standing up will relieve, she was told NO...she then asked for several things and again was told NO and when the CNA left the room, my friend was told that she, the CNA was done with her and left..Now my friend told her she was afraid of her, this is not acceptable. I got hold of my friends husband and told him what I had heard..it was verbal abuse. He went to the director and told them and the director told him that the CNA lacked people skills...and her responsibility is working with people...the CNA was just transferred to another wing of legend oaks so she can continue to verbally abuse other patients. This just does not seem to be the correct way to handle a problem like this. Most of the CNA's at legend oaks are caring but this one is not and is most definitely in the wrong business. Another thing I notice is that the Hippa laws are ignored and patients and their problems are discussed where everyone can hear what they are saying...Are Hippa laws not observed in Texas as they are not being observed in this nursing home!

Overall, this facility is kept clean and tidy. Sometimes the halls will smell of human odors due to diapers being changed at certain times of the day, but it is usually taken care of fairly promptly. One problem I do see, and my grandfather complains about this all the time, is the food. It's edible, but it seems like they slop it together quickly which will end up with tater tots broken up to tiny pieces, meat is too dry and overcooked, food has no flavor, etc. However, this is an understandable mistake when it is considered how many mouths are being fed and the ratio of cooks to residents. The employees at Legend Oaks are always friendly and hospitable, they visit with my grandfather quite often. They may need some work with pairing roommates though, since it seems that sometimes they pair a more mentally agile person with someone in severe mental decline, and it sometimes causes problems between the two. This also is, however, something that cannot be helped at times. Especially times that they are at resident capacity. The employees do a very good job of transporting residents to physicians when need be. They work very quickly to get things taken care of. All in all, I am satisfied with the performance of the employees and owners of Legend Oaks Healthcare and Rehabilitation Center. They are taking very good care of my grandfather.
