My dad was a resident of Woodlake Nursing and Rehabilitation. The grounds were well kept. They had a water fountain in the front. The staff did what they had to do. They were nice to us. My mom had dementia. So, she would call up there thousands of times and they were nice about it. My dad didn't do any of those activities because he couldn't walk but they would do physical therapy with him. I don't think they had many activities because I was asking about it for my mom to go in there, and they didn't have anything but bingo or something like that. I never ate the food, but the food was decent. I wasn't upset that they couldn't take him to our doctor's visit on Tuesday and Thursday, which they said they could. The outside was nice, but when you came inside it smelled like a nursing home.

I visited Woodlake Nursing and Rehabilitation. The staff seemed very caring. The rooms have shared bathrooms and are good for what they are. The maintenance seemed very good. Their dining area is pleasant. I've seen the rooms, the rehabilitation gym, and the lobby. They have a lake and a fishing pier. The setting is nice, and it has a good sense of community.

This facility is average. My mother lives here. At least one of her nurses is rude to her all the time. Other nurses are nice. The serve meals on paper plates.

The aides are very unprofessional throughout my visit I over heard personal conversation with voger language staffs are not mindful of residents. I also witness multiple staffs at a time outside throughout my visit Charge Nurse being heard talking with another stafff member about coworkers. Most of all rude aides when asked for assistant with family member so it makes me wonder who's running the building makes me conserned more training is needed along with staffs wearing name tags I will not bring another family member to facility no time soon. Needs improvement

My 82 year old Grandmother (mom) has temporarily stayed at Woodlake Nursing Center several times in the past few years for rehab. Therefore, I have spent quite a bit of time there and have nothing but great things to say about them. Neither of us have any complaints whatsoever. Everyone there is very caring, friendly and professional. The facility is nice and clean and the property is beautiful and peaceful. My mom will be staying there again as soon as the hospital releases her. It gives me great peace of mind to have them take care of her when I am not with her. Everyone there adores her and she feels the same about them! She told me this evening when I went to visit her at the hospital that she can't wait to be back at Woodlake to finish healing after the surgery she had last night. Woodlake is awesome and I wouldn't ever consider another nursing facility for my mom! She wouldn't either.

I couldnt be happier with the care that my relative has been receiving. I recently visited him at Woodlake and he looks so much better thanks to the great physical therapy that he has been receiving! The staff does such a good job and I would definitely recommend them to anyone!

My Father was a patient at Woodlake. When we arrived at Woodlake he could not walk and was very confused. The nurses and the therapy team worked diligently with him and I am relieved to say we were able to bring him home. As he was completing his therapy, the team worked with me to overcome the obstacles he would be up against at my house. The transition back home was made much easier because of this. Everything wasn't perfect everyday but I have no complaints. He was well cared for and I know the staff cared about my Dad. The facility is not new but it is clean and the staff are excellent.
