Jefferson Nursing and Rehabilitation Center is beautiful on the outside. When you walk into the lobby, it is beautiful. It looks like you're walking into a hotel. They have a great big round thing for the nurses' station in the middle of this atrium area that you walk into. Then down the hallway is where the clients are. There are 32 people down a hallway, and there is a nurse and a patient care tech for 16 patients. The smell was bad because they had so many patients, and so few nurses to take care of them. I could smell the urine and the poop. I don't know if that's because they have staffing problems, if it's because the other management people were not on top of it, or if it's just because there are so many people in the hallways that come out into that central area. It looks like a hotel. The lady there was nice. We asked what the Medicare rating was, and she proudly said one. They said they just got new management though and hired four nurses, and four people in the upper management area. I don't know if it'll improve or not, but my overall rating of them is low. They showed us the setup of the rooms through a showroom. They have private rooms, and they have a cable and Wi-Fi that goes with it. They have a lot of amenities there. They also have a speech therapist, a physical therapist, and an occupational therapist on staff there. They have meals that are cooked in-house, which is nice. The lady that took us around and those that we met during the tour were very friendly, so I would say the staff was fine.

Staff has no concerns are care for the patients. They are not seeing that my family member gets his meds, nor or they taking the time to see about his physical needs. One lady went so far as to tell him to do it hisself! That is very unprofessional and unacceptable! Also no one told us this is a place with COVID patients and require visitors to get fully suited just to visit. Places with lack of care like this needs to be shut down permanently! 🤬🤬🤬

Food is terrible. No fresh fruit. Limited vegetables. Weekends are the worst. Soup and sandwiches served a lot. No indoor activities due to covid. Have to ask to go outside. I have complained many times to director social worker etc. Showers 3 times a week if your lucky.

My husband was a patient in this facility from Oct 23 2017 to Nov 22 2018. The staff was exceptional. They were caring and very professional. My husband was at the least to say very reluctant about going to rehab. He was very at ease and while not happy about being there, they made his stay much less stressful. I would recommend this faculity to anyone needing a rehab for their loved one. Only suggestion is to bring them extra snacks and food because the food is a little bit bland and could stand a to be a little more appealing. Other than that small complaint my husband had no other issues and we were very happy especially with the night nursing. They were awesome.

Jefferson Nursing and Rehabilitation provided me with physical and occupational therapy for a period of 3 weeks in 2013. The therapy departments have very professional and qualified therapist that work one on one with each patient to assist them to receive the best recovery possible. The staff at Jefferson Nursing was very caring to myself and my family and the care I received was excellent. I would recommend this facility to those who are in need of short or long term care.
