I have had a few family member in facilities for rehab or end of life care, and Four Oaks is by far the best. My grandmother, aunt and father to name a few have all been at Four Oaks and received excellent care and was treated as part of the employees own family. I actually moved my father from a different facility to Four Oaks because of the get people and care there. Everyone seems to care about what they do from housekeeping to administration. When a concern is brought up, it is handled. I don't have to worry. Thank you all at Four Oaks Healthcare!

Feedback from my dad, a 96 year-old patient for a one week rehab stay: I stayed at the Four Oaks center for rehab. I would recommend this place only if you can have someone stay with you the whole time. If you are here on your own, the care is not reliable. No one shows up if you really need help, however, there were exceptions. You can sh*t in bed and in the meantime still be alone for too long. If my daughter were not with me, I would have screamed and probably would have been mean to the staff. The nurse who gave me the enema did really well. The rehab services, they did good. Food is very basic. The place is clean. My feedback: I was lucky enough to stay the whole time with my dad and be there right away to meet his needs and be his advocate. The nurses and staff went above and beyond in supporting us and provided exceptional care, with the one exception my dad noted above about defecating in his bed. I was very impressed with Trish, Trish B and Terri, our nurses, as well as the PT/OT staff, Dr. Hoover and his NP, Rachel. They were very friendly, helpful, caring, and genuine in the service they provided . The maintenance staff, kitchen staff and others were also very helpful when I had questions or needed items for our room. My feedback for the DON. Through effective leadership and feedback from your team, lead your team on how they can respond to the call button more timely to meet the immediate/urgent needs of the residents. I understand staffing issues, but being there for a week, day and night, I also think there is a desensitized culture in their response to the call button. Also for the executive leadership of the facility. A one-star CMS rating should really be addressed and a strategy put in place to improve your rating, all for the benefit of your residents. A few of your local competition has 5 stars.

My mother stayed at this facility for 22 days. She was ignored and mistreated. She would need assistance going to the use the restroom. They would ignore her hitting the call button for help. They would put her in a chair for 12 plus hours at the nurses station for punishment. Due to not let her moved around now we are dealing with her feet and legs swollen to the size of a Mellon. Because of the abuse my mother suffered there she is afraid to sleep in her bed , she screams and yells all night long. I regret the decision to let my mother go there. I pray she will be able to put this terrible and abusive situation behind her. I wanted to let everyone beware of this horrible place and don’t let your love one go there.

Four Oaks center in Jonesborough impressed me tremendously. I walked into this building during a B2B sales job - and I was completely unaware that this building was any type of elderly care center. The building was immaculate and well-kept, and the staff were very friendly. The front desk clerk bought a Domino's Pizza coupon booklet from me to use for the guests when they ordered pizza on weekends.
