Our LO was a resident at Springmeadows for 2 years. Staff very friendly and welcoming. Most needs were met right away. Yes the rooms are on the small side, but the residents were encouraged to get involved in the daily activities outside of the room. Usually smelled clean and not full of urine.

Spring Meadows Health Care Center was all on one level. The staff was really nice and friendly. They gave the option to have either an individual room or a shared room. The administrator has given me the information that I needed about the facility. They were very informative and answered all of my questions. They provided everything that would be needed. They will make arrangements for, like, the medication.

I found my family member slid to bottom of bet like a ball, soaked in urine, dirty hair and nails. It took looking for a staff member for 10-15 minutes to change change wet diaper and pad. Beginning of contractures from improper positioning. Rooms are small as are halls. Looks like an old depressing mental hospital from the 50’s.

Spring Meadows Health Care Center felt more like a hospital than a nursing home, but it's well-kept, friendly, and a comfortable place. The person who gave the tour was very, very knowledgeable and very helpful. Any questions you have, they're very, very good about explaining the small details to help you and assist you in making decisions. The dining area was pleasant and comfortable, too. There were some group activities where the staff was working with the residents, doing some exercises, and some speech therapy.

The rooms are big. The staff are friendly all times I have been there. The residents love making crafts and playing bingo. There are people that go in and donate items to the residents. I have heard preachers come in and sing with and have church with the residents.

I have never seen such an Unprofessional place in my 55 years.Ive seen enough in 2 weeks to shut This place down.My mother in law has fallen 2 times this week While hitting the nurses button Only to wait 31 minutes to get a response Very poor service . Honestly I feel sorry for everyone that is in this place .She has fallen twice had the neighbor Come take a dump in her floor 2 times that I know of.I could go on and on But due to the circumstances My attorney will take over From here I understand as a business owner the need for help .But I don't book jobs I can't do There probably at full capacity with 1/2 the staff.Im not sure but It takes time to get facts .God bless all These residents And I hope this place After the calls made . Something happens This isn't right for free much less if your paying to be at this facility . Fact's are what they are and I'm sorry I even had to waste the time to give out some important facts.God bless

I could not continue caring for mom at home. I talked with several others who had experienced their parent living at Spring Meadows and some that chose other nursing homes in town. I chose Spring Meadows over the others and have not been disappointed with my mothers care. The staff go above and beyond to keep me informed. I am very please with choosing Spring Meadows, and feel my mother is in the best place.

My mother stayed at Spring Meadows Health Care Center. I didn't get to see her while she was there because of Covid shutdown. I saw it from the outside. It looked like it was maintained. It didn't look like it was neglected. The staff was very caring and nurturing. It's very expensive.

I would have liked to have been notified of the positive Covid-19 tests in residents as well of staff, instead of having to find out online. They are slacking. The food is terrible

I highly recommend Spring Meadows for the best care of your loved ones they have by far the best staff in Clarksville the staff are some of the most caring and compassionate they go above and beyond to make you and especially your loved one feel that they are important .

The facility is very well maintained and very clean. I was most impressed by the staff. I had interaction with over a dozen staff members. All of then were so nice and so helpful for my mother. Many of the caregivers and nursing staff members were noticed giving such wonderful care to patients. I am so impressed with this facility. I have experienced many facilities, this is by far the best I have ever seen. Thank you so much.

I went to this care provider and took a tour, they were very kind and I liked the way you could set up the rooms just like home. My mother was at a place for behavior problems due to medacations and dementia. They said they would be happy to help with my mother when she was released. They sent forms for transport to spring meadows and after all the talk , my mother was turned down , they would not accept her. We don,t why or reason this happen, very dissappointedwith results.

I had a good experience with Spring Meadows Health Care Center's physical therapy. The staff was very kind. One of the things that is so important to me is to be called when they want to change medication, which they were not that good at doing.

I was in this facility for 3 months after kidney removal for cancer and a stroke. I was very pleased with my treatment and most of the young aides became my friends even though I am 76 years old. My only regret is that they say I am now doing too welll to be there. Their therapist are the best,

This facility is understaffed and is not competent to provide adequate needed care to their patients. The majority of the nursing aides working at this facility do not care about their patients, they are only there for a pay check. This results in patients not being turned and cleaned as needed. Many of the patients there have pressure ulcers. I understand some of these are hard to prevent, but many are simply a cause of neglect. The patients are left in urine and stool without being promptly cleaned. Falls are very high risk at this facility. I have witnessed patients sitting in the dining room with food in front of them with no way to feed themselves because they have no assistance. They need to hire more competent staff and staff who actually care about patient care and not just the clock.

My Mom has been at this facility since March 2008. When she entered she went because she needed more help physically than an assisted living facility could give her. She became friends with the nurses and aides and even some family members of other residents. In Dec 2009 she had a stroke that affected her speech and memory. With speech therapy her speech got better, not good like before but enough so we could understand what she was trying to say. Since then she has developed vascular dementia and is losing her memory some. There are days she doesn't know me, but she is well taken care of. She has fallen twice now, because she was up without help and lost her balance even with her walker. Now they have her alarmed up. She has one on the seat of her recliner and also one on her back as she scooted out of her chair a few weeks ago. They check on her often and now her walker is put away from her so she will not try to get up on her own. The Nursing Home has done everything they can, by law, to keep her from trying to get up without someone in the room helping her. She forgets where her call button is and also forgets what it is for. But I feel very comfortable knowing my Mom is well taken care of.
