My mom was at AHC Clarksville for rehab. They were all super nice and they tried to get to her as quickly as possible when she needed something. It's slightly rundown, but it was still a good place for her to be. Everybody seemed super nice, the therapist that worked with her was good. They catered to her, as far as physical therapy went. It just seemed a little on the shabby side, but I don't know what nursing homes can do about that. This was a few years ago so they could have done something by now, but I haven't been there since then. They could freshen up the hallways though, like a fresh coat of paint or something. The room she was in had plenty of space. It was well-lit too.

My mother was in this nursing home before AHC took it over and it was a great place at that time, but once AHC took over it went downhill and very fast. My mother passed away before it hit rock bottom and in a way I am thankful because we would have had to move her out of there if she hadn't and that would have been a hard transition for her. My father in law was a resident there for about a month and it was the most horrible experience for him. The staff were not attentive at all, he fell out of the bed and fell with assistance going to the bathroom. When he asked for assistance, it took forever to get anyone if they came at all. We had to transfer him out and for elderly people, changing environments is difficult. Think about all this before you consider this place for your loved ones. I would never send another loved one there.

Very disappointed of Care not given. Food was poor that family lost large amount of weight. Left patient in feces and urine for hours. No answering light 1 to 3 hours at a time. Refused to help take patient to bathroom. Nurse stated not her job and walked away. Noted patient didn't checked on ever 2 hours as required. Please read all reviews before making a decision placeing a family member in this physicality. I am so disappointed with promise made and things better and never happened.

I was a resident for 3 weeks for rehabilitation after a fall. It was in the middle of a Pandemic so I was not able to leave my room very often. The food was good. The staff was mostly good but there were some that definitely did not care. The physical and occupational therapists were outstanding. I couldn't have asked for better care. Due to Covid-19, there were no activities, however, once a week a wagon came down the hallway with either cookies and hot chocolate or popcorn and soda. It was a wonderful distraction to a long day. I am rating the activities as terrible because there weren't any due to the Pandemic.

My mom has been here a few years. It is currently the worst it’s ever been. I’ve seen nurses bullying staff and residents. There’s no love there anymore, they just give pills out and move onto the next patient. The good staff has gone and there’s practically no management so the bad nurses seem to run the place. They need a change to happen there, and soon. Some good CNAs have stuck with it and are still there, but many have left.

Be aware of what medications are being given to your loved ones. My mother was administered a transdermal fentanyl patch without our consent. She became comatose and was on death's door. My mother was septic, had pneumonia, a 104 degree temp, and was extremely dehydrated. My mother was there for rehab. The PT and OT personnel were fine. I have issues with the people we trusted. I've learned someone needs to be there 24/7 not just every day.

The nursing care is tip-top. Our loved one was there for rehab after a hip replacement, not for long-term care. A wonderful nursing staff and culture. As for their bureaucrats, well, they're bureaucrats. We had a 10 day period where our loved one could no longer do PT/OT--there was a sudden decline with extreme pain. The bureaucrats incompetently, rudely and stupidly used this as an excuse to boot him out (no longer improving on the rehab front, so you can't stay). Their communication with us about it was incompetent, pathetic and dishonest. We scrambled to bring him home, and the day after we did, his hip scar exploded from the pressure of all the infection in it. A procedure that would have detected it was scheduled for a couple of days later. The infection was so bad that the surgeons wanted to do another hip replacement because that was the only way to really flush it out of him. He and we declined, and he's now home in hospice care. Beware their Kafkaesque bureaucrats, love their nurses. Activities for residents are a lot of fun. Food ranges from, wow, this is hospital food? to oh, yep, that's hospital food. Comfortable rooms, a little small, good TV, and I can't say it enough, best nursing staff ever.

My father was in Clarksville Nursing and Rehabilitation Center. The general facility was nice, organized, and clean. It's much larger, too. I felt that the staff did not meet the needs of my father, and he was not getting the attention he needed. The food was more like a banquet meal that you would heat in the microwave and eat. He only participated in the rehab. He did not participate in the activities, and I was not aware of what activities they had. There could be better communication amongst the doctor and nurse practitioner to the patients' families and more oversight to the patient's needs.

My mom has lived there almost a year. She has had one very bad experience, the PN raised her meds too quickly and almost killed my mom. The staff also never contacted me to tell me the change in my mom when that happened, I was very angry about it when I walked in and saw the way my mom was and no one thought to call me. My mom is still there because the majority of the staff is very good and it's a very clean facility. I call my mom every day, check with the nurses often and double check her medications whenever there is a change. So I recommend this place as long as one can be involved in all decisions for loved one.

Clarksville Nursing and Rehab was very clean and very nice. I thought it was a nice facility. Everybody was very professional and nice. My mom was fine to start with but then she got home sick and just freaked out. It was not their fault; she is just an old, cranky woman who wanted to go home. I would recommend Clarksville Nursing and Rehab.

My wife was in here before, she liked it and I liked it so she stayed there from the hospital. It's close to home and is handicap accessible. It's a brand new facility and the rooms are spacious. The food's all right.

My mom's 84 years old. She went in for probably 31 days. The staff is wonderful. It's an excellent facility. I'm a big smell person, but the place is immaculate. I couldn't complain. I would recommend this facility.
