Welcome to Rainbow Rehabilitation and Healthcare Center. We are a 115-bed skilled nursing center that is located in the suburbs of Bartlett, TN. We provide a wide range of skilled nursing and rehabilitation services for both short stay recovery, as well as ongoing supportive services for individuals requiring long-term care. Our focus is on improving the quality of life for those we serve through our orthopedic, neurological, cardiac, pulmonary, wound care services as well as interventions for other medically complex diagnoses, admitting 24 hours a day, 7 days a week. At Rainbow Rehabilitation and Healthcare Center, we want to make your transition from hospital to home a pleasant and productive experience. My mom stated this place. She stayed in a semi-private room. The problem with the semi private room is that there is no quite time. There is no control over the TV which is for both resident. I end up talking to the director who talked to the woman in the room and she turned it down a little bit, but still left it alone, causing my mom issues, falling asleep. You could walk down the hall and smell urine and feces. The staff like it other places I assume of this caliber of them we’re good and others were bad. I had one of the nurses that was ahead, nurse to RN that wouldn’t even look me in the face when I asked questions gave me one and two word answers when I asked questions about taking care of my mother. They did not give my mother medicine like they were supposed to because I took the pills in and when I got them back, they miss giving her four tablets. She was supposed to get two a day so for the tablets were not given to her in a two week period. It was hard to get blood results back because my mother had some ammonia issues that needed to be checked and they didn’t check them like they were supposed to two weeks. I asked to have them checked, but they never checked them. She the last day she was there rolled out of bed twice morning And fractured her arm. I would highly suggest that unless you just can’t find some other place that is a more replicable nursing home for rehab to steer away from this one. The nursing staff and the staff were horrible. It was almost impossible to get them to feed my mother like they were supposed to because she was having problems feeding her self. I was going in at lunch and at dinner to make sure she got fed would’ve left the food there she ate it. She ate it and if she didn’t, they wouldn’t Have cared . The rehab therapist seem to do a pretty good job, but the nursing staff was horrid. I would say that only go to this place if you’re parent or something or you are capable of taking care of yourself without nurses supervision otherwise find another place.

My mother was a resident in this God forsaken place and if there had been any other option for us, I would have put her on my back and carried her out. From "Hazel" who told us not to use the laundry service because we wouldn't want mom's things mixed in with all the other crap and vomit from other patients or because they SELL the laundry for a $1, to Nicole who shook her finger in my mother's face and told her she didn't have to come help her, she only had to turn off the call light, the place was a disaster. Nurses gave my mom her night medication at 7:00 pm and couldn't remember what they were giving her and when she asked, they said "are you refusing your meds?" They overdosed her on pain meds- twice. I believe it was intentional so she didn't push the call light. However, she was bed bound and needed help using the bedside toilet. They wouldn't help her and often told her they were OUT of toilet paper (which we provided), out of ice, out of wash rags, out of wet wipes, and not once did dietary give my mother the correct meal. I have pictures of the food that would turn your stomach. The nursing and cna staff are cruel and mean and have no empathy or concern for patients. I wish I could have recorded the way they spoke to my mom. The only redeeming factor is the REHAB staff. They are wonderful!! However, if the CNA staff is mad they will tell REHAB you aren't ready or don't want to come to rehab and you will miss it. This place is a horror story. RUN don't walk to anywhere else. My mom's room had rat or mouse droppings behind the door for 3 weeks and not til the State showed up was the floor cleaned.

some nice staff, but generally indifferent to rude and mean. clean, but very quiet because it is very understaffed and very hard to get a response from nurses or aides

This place is horrible! I could give you a list of 50 reasons not to stay here. But I will start and just give you a couple. One. They changed our mother's medication and overdosed her without talking to any of the family. 2. They did not take our mother for assisted eating a couple of times in a row. 3. They often would not answer call lights and when asked questions were very rude to the family and patient. Our mother was in the final stages of her life and this was a horrible experience..... we will be filing a formal complaint

Do not take your loved ones to this facility. They do not take care of there patients they just lay in bed and develop open wounds and bed sores. The nursing staff are lazy and do not care.

If you love your love ones keep them from this place! My sister was there only two for rehabilitation for a hip surgery. She was only 59 she was coming calling for help and the only thing that was said we will get your nurse. She suffered two heart attacks and she went to long without oxgennto her brain. I don't give them no stars.

They told me when my father was in there that they keep people as long as insurance approves their stay, whether they need it or not. I told them my father could not afford the co-pay and the director told me that it would be okay..he would not be expected to pay...now they are threatening me with collections and refuse to send the bill to my father. They do not return phone calls or emails and the financial department is rarely there if you "pop" in on them If despite knowing all this you go there.. Do not do their laundry service half of my father's wardrobe went missing!!

Would not send my most hated enemy here - In my 30 years as a nurse I have never seen the level of incompetence, lack of compassion and downright lack of care in every aspect in any facility I have ever worked at or been at with a family member or friend. When you have to call 911 to get someone to come to come to your room after 2 hours of waiting on an urgent need and then have staff lie and say you are "confused" it was two hours and they did everything you asked well it's time to throw in the towel- I will be reporting to the state board - spent an entire morning trying to straighten out medications, treatments and just getting personal items such as bedside commode & wheel chair - bad luck if you are admitted on a weekend - all extra TV remotes, wheelchairs and bedside commodes are locked in a shed out back and maintenance isn't there to open it till Monday- one nurse helped with medication mess, one social worker who the nurses told us "didn't come in on weekends" did come in and assist with some other items - according to her a social worker is there everyday - I had a resident who was confined to a wheelchair due to partial paralysis stop me and ask was my loved one getting decent care then the poor man who despite his physical limitations was very clear headed proceeded to tell me the many horrors he endured but b/c of insurance and no nearby family members he was stuck there without other options - not to mention when one staff member also tells you please don't leave any personal items b/c a lot of stealing goes on here it is just surreal that one place could be that bad - so we will pay out of pocket for private sitters to be there when we can't until we can figure out another option - every staff member there should have to endure what they put patients through - dirty rooms/ no help with personal toileting when unable to help self / dirty bedpans no one takes responsibility to clean/ and the poor roommate in with my family member who started throwing up but was unable to even push a call light and we did for her and they made no effort to come down till 20 minutes later .. promising to come back clean her up and bring anti-nausea meds - you guessed it that never happened .. pitiful excuse for a care facility - only care they give is their self as they cut up at nurses station - and the story we heard from another family member about a dementia patient who just recently walked out of facility down the street and was thankfully "rescued" by a good Samaritan in the middle of Hwy 70- the staff had no idea she walked out the front door which is "key padded with security code" !

My father went there directly from a medical hospital. When he was transported back to the hospital for a surgical procedure, the staff was not informed he was gone. I went the next day to get his property, and no one stopped me. One actually gave me a luggage cart when I asked. Not one nurse or administrator asked any questions, and no one called to ask if he was returning. I could have walked out with anyone or anything.

I wouldn't recommend this place to anyone. Made the mistake of admitting my 92 year old father. He was only there barely 5 days before he passed away. We were mostly ignored when needing something or felt like we were asking for the moon & stars for asking a simple question. He never saw a doctor while he was there and there is supposedly two on call.. So now we have been waiting for over a month for Doctor Hines to sign the death certificate. Please don't take your loved one to Rainbow. You will regret it. I do.

The best thing I can say about this place is that it is CLEAN! It does not have a bad smell and is very well decorated. I really wish I had read these reviews before putting my Mother in there. While my Mother was here, she contracted a Staph infection and received 5 shots to help her get over it. I was not aware that the healthcare facility would "let" her go until the infection was cleared up, but when the insurance had covered the number of days for my Mother's rehab and was stopping coverage, they couldn't get her out fast enough. She still has the infection! She was told how to manage it when she went home! She's 85 and in better health than most people there, but she will not go back in the future.

It was clean, roomy, and quiet; and the CNAs that cared for my mom were great. However, I would not give you a dime for the doctors, nurses, and administrators. My mom went in for rehab and came out in worse shape than she went in. She is bipolar, and I told them that without her psychiatric meds she'd be unable to benefit from the therapy she was there to have. Despite that, her psychiatric meds were slashed; she was given an Alzheimers med (that can cause additional mania in the bipolar); she was drugged out on pain meds. Worst, whenever I tried to address the meds with anyone, I was lied to me about what they were doing. At least she is home now, back on her proper meds, and doing well.

I had the same experience with this nursing home when my loved one was there. He passed away in December 2013 from Alzheimer's. He was also shipped off to a mental health facility because of their lack of compassion and understanding of dementia. I would not recommend this facility to anyone, but it is particularly not good for an Alzheimer's patient. They have too many deficiencies to count; it is just not a good place. The only things it has going for it is that it is a clean and pretty place, and of course, that is important. However, you have to look deeper than that; what good is it if the facility is pretty but dysfunctional?? Please do not put your loved one there; I think you will live to regret it. I know I did.

I am a past client of this provider

I do not recommend this facility. It is a nice, clean facility, but the CNA's (Certified Nurse Assistants who do the day to day care for your loved one) are not bright at all. Rather than helping my grandmother eat (who was extremely weak), they stood over her and told (barked at) her to eat. They do not treat the elderly who are in need of full support with compassion. My grandmother developed a UTI while there and was sent to a mental health facility because she was "disruptive." Her dementia had escalated as a result of the UTI. The next facility was horrible, and within 2 months, my grandmother passed away.

I'm satisfied with Rainbow Healthcare. This home has a very friendly atmosphere. They have pretty colors on their front door. The room that I have is big and painted blue. The staff members are really nice people. They are also very helpful. During craft time, I'm not very artistic but it turned out to be quite fun. What I don't like about this place is their food. They always serve mashed peas. They are supposed to make good meals.

Visited this facility

Sherry (the Social Worker)...is never available doing normal business hrs. Normally Business should be hrs.8-4:30 or 9-5:30. When you are able to reach her and tell her what your family member needs. She doesn't follow-up. The Administrator has been made aware of the issues but doesn't plan to do anything about it. Before admitting your love ones to this facility, make sure there will be a Social Worker on duty.
