I visited this facility

I toured Harmony at Five Forks Memory Care. The place was beautiful and I was very impressed. It was a place that I would consider. I only looked at one room because they only had one room available. That room would have been, in my opinion, too small for my husband, but they have other rooms that if you're not in a hurry, you could wait in a larger room. So apparently, they have different-sized rooms. The staff was great. She was very well-versed. She knew exactly what she was talking about. We had a very good conversation. I think I was there for about 45-minute tours. She did a very good job.

I am/was a resident of this facility

We have moved into Harmony at Five Forks. The staff is very nice. They seem to do a good job on the staff. The facilities were very nice, too. We have a relatively large apartment. That's one of the reasons we picked this place because they have good-sized apartments. It's a two-bedroom-two bath, and it's a ground-floor apartment with a patio that goes outside, so it's very convenient. In assisted living, they don't allow any cooking or anything, but they provide a refrigerator and a microwave. Out of the places we looked at, this was by far the best. The food is excellent. We're sort of vegetarians. They don't have a lot of vegetarian options, but they have some, so it's really good. They have a lot of fitness activities. They show movies and lectures and so on, and they have happy hour. It's nice, but it is very expensive.

I loved Harmony at Five Forks. I liked the community events and the happy hour lounge with the live music. I liked the cleanliness. The staff during the tour was nice and thorough. The apartments were efficient. The studio was spacious enough for one person. I did not try the food, but it was restaurant style and it looked good and the menu was great looking as well.

Harmony at Five Forks was very clean and very well-organized. Their residents all seemed to be in a very happy state. It looked like a nice place. It's a place I could put my mother in. The rooms were clean, they were well-organized, and they were safe. One of the things that I like is that their memory care is separate from assisted living. The staff was very pleasant. My contact with the staff was very limited, but everybody seemed to be doing their thing and enjoying it. And their exercise class was full, which I loved. They also have a library where a lady was putting together a very major jigsaw puzzle, and then in the memory care unit, they were having a group that was watching television, and then they also had a group playing bingo. I only met with sales and marketing, but the staff was friendly. The community dining areas looked great. The dining for independent living is a very upscale type of dining room. It's a fairly new facility.

I toured Harmony at Five Forks. The facility appeared clean and well cared for, well-kept. The director that met with us took the time to speak with us, took the time to walk us through the facility, showed us what was available, and showed us what options were available. The dining area was acceptable. They seem to be doing some activities with the residents. They seem to want to engage the residents and keep them involved as opposed to having them in their rooms.

Provider Response

Thank you for taking the time to visit Harmony! We strive to provide a caring, loving environment at our communities. If you ever need anything please don’t hesitate to reach out!

We toured Harmony at Five Forks. The facilities are nice. The apartments were nice. There was a staffing problem. I had a meal there and there was a long wait. They seemed to be short-staffed. I was sitting near the front desk, and while I was waiting, a resident went to the front desk and was complaining about the wait times and the lack of staff, so that made me very wary. The meal was very good. Activities are very typical of all the assisted living like little enrichment things, maybe a cooking class, the typical bingo, and flowering arranging kind of things. They have a gym. They had a salon, but when I went and toured, they were currently looking for a new person to operate the salon. They did not currently have anybody to operate the salon.

We'll be moving in at Harmony at Five Forks. I like the friendliness and the cleanliness there, and all around it was a good place. It's all pretty new, the grounds and the rooms look good. They have two-bedrooms and three-bedrooms. The food is great and the staff that we met were very friendly and very knowledgeable.

I went to Harmony at Five Forks yesterday. It was a very nice place. Everything was clean and well kept. I went to all three areas. After talking with the representative, the level of care for my mom would probably be memory care. The rooms were small but very nice. They had a large bathroom big enough to get a walker in or a wheelchair if you had to, and everything was nice and clean. The lady that took me around explained that they had some activities, but I don't remember what they were. It was right around lunchtime and there weren't any activities going on at that particular time. The dining area was nice. Everything was clean and homey looking. It reminded me more of a restaurant than a facility where they take care of people. The staff were very nice, especially the lady that took me around and showed me on the tour. She was very nice. Everybody was very friendly.

My friend will be moving into Harmony at Five Forks. We're waiting for a bed, but we've already paid a down payment for it. It was quiet, it was very clean, and it didn't seem like a nursing home. It was more like apartments and very private. The staff was very pleasant. The food was very good. They had games and there was music going on when I was there. A little boy was playing the piano and it was very nice. The layout of the apartments was very good. It has a little kitchenette. It's like a small apartment, and it's nice. They have a therapy room that she will be using. They have a beauty parlor and an outdoor area. There was a screened-in porch that was very large and very private with ceiling fans. A lot of their amenities were included with the price. There wasn't a lot of extra stuff. What you pay is what you get.

I had an in-person tour of Harmony at Five Forks. The atmosphere was nice. The person that took me through was very nice and accommodating. The food was good. The people didn't seem to be quite as friendly and as happy as they were at the other facility. They have one meal a day and housekeeping. They had sliding doors, which make the room brighter because you have more windows. The dining room is shared with the assisted living. The price was higher for what you're getting. They have a refrigerator and a stove. The food was good. The staff was very personable and likable.

Harmony is not equipped to give quality care to Memory Care patients. Management cares about itself and very few of the care staff have the temperament to care for these precious people. If your family member isn’t easy to deal with they will quickly suggest you take them elsewhere. At the first possible opportunity they will try to impose a price increase even though you had a “locked in rate “. You can’t call yourself a memory care facility and not be willing to care for those with Memory Care-problems. Don’t call yourself a full spectrum care facility when you are not! It’s false advertising and unfortunately our family took the bait. Please do not bring your family member to this facility. You will regret it.

Harmony at Five Forks was a very nice place and they've got very friendly people. The communication was open and answers to my questions were given without hesitation. I like it. The staff was very friendly. It's clean and it didn't have an odor. It's a good thing that you know everything is taken care of the way it's supposed to. They have a beauty salon and a game room where they do different games and activities. They have plenty for them to do.

The tour at Harmony at Five Forks was good. The facility's layout is very good and very clean. The staff was very good, the dining area looked nice, and I noticed their amenities. One thing I don't like about it is that it has an oversized kitchen in every room, and the kitchen is not needed. It takes up the biggest part of the room, you have like 20 kitchen cabinets, and you don't need a single one. The bedrooms cannot hold two beds, even two twin beds, so the room layout is not good. The units are not laid out right. Who needs a huge kitchen when no one's cooking. I only saw two staff members, and they looked competent.

Thank you for your honest feedback Ted.

The person who took me on the tour at Harmony at Five Forks gave me much more information than the other places did. I liked that their meal plans were very healthy and that they had choices on entrees for each meal. The memory care facilities were pretty. The apartments were nice, too. They offer various activities. There's a guy that likes to fish, so they had a whole bulletin board just for him for fishing interests. They have an area for people who are interested in mechanics, and things like that as well.

Harmony at Five Forks is new, it has nice amenities, a great staff, and great services. I went to see independent living and I also just want to see what assisted living looks like. I thought the staff was excellent. They have a progressive level of care so it's nice. They have multiple avenues. It has a salon, a gym, a library, and a fine dining area. I think they had a walking area, but it wasn't that big.

Our tour experience at Harmony at Five Forks was good. We liked the actual size of the apartments, and it came with a full kitchen, with a washer and dryer inside the unit, which was good. The staff member who gave the tour, Adam, was very good. He was very friendly and very informative. They had cards, outings, activity nights, and a bunch of stuff.

I am a friend or relative of a current/past resident

I’m at complete disbelief and am saddened by the lack of compassion and understanding this facility had towards my grandfather. It didn’t give him a fighting chance. It’s hard getting someone adapt to a new environment with his condition(PICKS/FTD), but this facility evicted my grandfather because he “made too much noise” after only staying there for 2 weeks! In all fairness, if that’s their logical reason to why he deserved how he was treated, then the residents that called my family monkeys and the ones that walked in other people’s rooms need to go too. My family wasn’t even notified that there were complaints against my grandfather until we received a call stating that he had two weeks to comply otherwise he will have to leave the community. Apparently the complaints were from residents. Why were we not notified at the time of the incident? First and foremost, according to DHEC the verbal 2 week notice violates the standard 30 notice we were supposed to be given. We then received an email stating we had the 30 days to comply. Well that’s contradicting the phone call that was made to us stating that we only had two weeks and they only ordered him two weeks worth of medicine(not 30 days). They never said “let’s create a plan to see if this will improve his condition and if that doesn’t work we can look into other options.” My family panicked because my grandfather needs the help. We started looking for other facilities. He needed a facility that understands that he has a condition and actually tries to help him. He will make noise when he is nervous or he needs something. Where is the compassion and understanding for our loved one who is a father, a husband, a grandfather, a friend, a veteran? It disheartens me to realize that people will treat your loved one as if they don’t belong. We found a facility that would be able to accommodate my grandfather. During the moving out process we discovered bugs in his room at the Harmony and that in the two weeks he had been there, they never even washed his sheets (it still had stains and had the same folding creases it originally had when we took it out the packet)! When we tried to give the key back, it’s as if they put us off. Nobody wants to return our phone calls or give us a time to come drop off the key. The lack of professionalism and communication from the staff with complying to get his records transfer to the new facility, was frustrating and a horrible experience. Now instead of taking ownership of what they did to my family, they are placing blame on my father stating we didn’t give them a chance. I’ve read the email several times and no where did it state let’s discuss our options or let’s put him a plan to see if we can see improvement. My family did not deserve the treatment we were given for trying to help our loved one.

Harmony at Five Forks was very nice in every way. The staff who toured us was very accommodating to us. The place was pristine and clean. The room we saw was very nice and very attractive. I would say the place looked modern. The outdoor space looked very nice too and they offered a lot of amenities.

Our experience with Harmony at Five Forks was great. Adam was amazing, and the chef, Keisha, was super great, super sweet, and very knowledgeable. It's just the financial aspect did not work for our family. Otherwise, I would have chosen them. The facility was clean and nice. What I liked the most is how happy everyone looked. I liked the way that the place looked. They're just very aggressive in their approach to convince you to move your family there. They don't like to take no for an answer. I like the staff, but overall, they're just very aggressive. I wanted some information, and they called me so many times to schedule a tour, even though I told them I didn't know.

Provider Response

Thank you for considering life at Harmony at Five Forks. I am delighted to hear that you enjoyed meeting our residents, Keisha and I. We do welcome you back anytime and promise to present achievable solutions that will help you overcome any financial barriers to move into our community.

I really liked Harmony at Five Forks. I was very impressed with the their staff, and the gentleman I spoke with, he was very nice. I saw a studio and a one-bedroom, and I thought they were clean. I liked the fact that it had a little bit of an efficiency kitchen with a refrigerator in it. I also liked the bathroom size and the way that the shower was set up. In the studio area, I thought it was just a little bit on the tight side for what I was looking for, but when you're downsizing, they have to look like that. Their library and eating areas were very nice too. They had a place for physical and occupational therapy, and for me, that was interesting. On holidays, they decorate their doors for the residents, and I think that's real sweet. They have a lot of activities and stuff for the residents. The facility looked very clean and it was very presentable. It looked pretty much current. I don't think that they've been up very long; they're fairly new. They had a decent menu, and they had a good chef and stuff behind that. Everybody that I spoke with that day were all wonderful and very nice, very helpful, very friendly, and smiling people.

Harmony at Five Forks is where I want to go so bad, but it costs more money. I like that they keep you busy. They've got a lot of interesting things to do, they go places, and it was super. Their apartments were really nice. Their amenities are outstanding, they've got parking in front of it, and the outdoor spaces were great too. Everything about it is great. They have a lot of interesting things and interesting people. Everything was perfect, I couldn't ask for more except for money. I would recommend it to anyone. Everyone was just friendly and they were really welcoming. The food was great, the place was just unbelievable.

We went to Harmony at Five Forks. It's good, I can see it as a very comprehensive independent living, assisted living, gradual change type. We visited the assisted living, but it was kind of hotel like to me. The staff was very enthusiastic, very nice, and very knowledgeable. Everything looks very neat, very orderly, sophisticated, but it can be a bit standoffish for people that are not gregarious.

We toured the Crossings at Five Forks and it's brand new. It hit us as more of a resort, which is fine, but it was three-stories high. I found it would be a problem for my mother because she's in a walker, and although there were elevators, I didn't care for that facility. It was pricey, and everything that should have been included was not. The staff member I met was very thorough, answered any questions I had, took us all through everything, and introduced us to everybody we passed. The rooms were very nice. I was told I can rent one of the rooms at an extremely reasonable price when I visit my mom there. The dining area was beautiful. It was four people to a table, with a tablecloth and cloth napkins. It was gorgeous. They have a board of activities, it's full, they have assisted and independent living, and they have activities for both. They have a beauty salon.

The Crossings at Five Forks is a brand-new place. The staff has been very responsive to any request that we have had or any issues that have come up. My mom gets a ride to church every Sunday. The food is good. She seems to be happy with it. There is a library, they have a pub with a pool table, and they have an informal gathering in that area. There is a dining room, a theater, a beauty shop, a fitness area, and physical and occupational therapy options. There are a variety of activities, like bingo, scrabble, bridge, trips, crossword and jigsaw puzzles.
