Southern Oaks Rehabilitation & Healthcare provides a wide range of skilled nursing and rehabilitation services for both short stay recovery, as well as ongoing supportive services for individuals requiring long-term care. Our focus is on improving the quality of life for those we serve through our orthopedic, neurological, cardiac, pulmonary, wound care services as well as interventions for other medically complex diagnoses, admitting 24 hours a day, 7 days a week. We can help avoid readmission to the hospital when a patient goes home to quickly by offering caring staff with 24/7 assistance and quality therapy to insure a smooth transition. I am a friend or relative of a current/past resident

My father was placed in this facility for rehab from a UTI after a hospital stay for a week. The facility was dirty on Thursday evening when we checked him in, there was a urine puddle in the hall that was never cleaned up while we were there that evening. My father was a fall risk from hallucinations from his medication and illness, I requested guard rails for his bed, and we were told they did not have any, all they could do is put a mattress on the floor next to him in case he fell off the bed. My father complained that during the 8 days he was at Piedmont Post Acute Care he missed multiple meals. This was confirmed by his roommate at the time that both of them missed meals. There was a shared bathroom for my father’s room with the room next door and the whole time he was at the facility the toilet would not flush. The water supply line was turned off for some reason. My father was at this facility for 8 days and never received a bath or shower. My mother complained to the staff, and she was told he would get shower on Friday August 23, 2024. She brought a clean change of clothes for him to put on after his shower, she called me at 2:30 pm to say the clothes were still on the foot of his bed. When I arrived at his room around 4:15-4:30 the privacy curtain was pulled around his bed, when I opened the curtain, he was sitting on a bed pan and covered in his own feces. I asked him how long he had been like this, and he told me for over 45 mins. He stated that he had called for assistance to go to the bathroom, and no one ever came to his aid. I noticed his clean clothes were now placed on his nightstand. I asked a nurse to please come and clean up my father and get him dressed. I asked to speak with the nursing director, and I was informed she was on vacation, but the assistant nursing director was in and I asked to speak with her. On the way to her office, we passed the Directors Office and I asked to speak with him and was told he must not be in the facility because his door was closed. On meeting with assistant nursing director , I informed her of the situation I found my dad in and that he had not had a bath the whole time he was there, and she told me she didn't know anything about it but she would check into it and "Get back with me." I told her I wanted to talk to the director but was told he was not in at the time, but she said he was but was at a birthday party down the hall. The director came to his office, and I told him what was going on and his response was "I wish I had known about this sooner!" I told him that I shouldn't have to tell him that his patients were not receiving the care they needed by a family member, and he should be checking on this staff. I decided them to remove my dad from this facility immediately. The director requested that we stay until the next day so that all the paperwork could be completed. After speaking with my father about leaving, I was on my way back to the director’s office I overheard a staff member asking the other patients on that hall if they had received their baths or not and overheard two different rooms state that they had not gotten baths either. I informed the director that we were leaving now and please get all the paperwork completed. When I got back into my father’s room there was a nurse undressing him, I asked what was going on and she said that she was going to give him a bath. I told her no that we were leaving, and she could just help me put clean clothes on him. She was helpful and apologized that he hadn’t had a bath but she didn't work on that hall and was asked to come over and take care of him. We got my father home, and he is doing much better. I called the director on Monday to follow up on why this had happened, and I was told he was on jury duty. I asked them to have him call me and he returned my call Tuesday morning. I asked if he had any answers to what happened with my father’s care and he stated, "Oh I didn't know you wanted me to follow up on that." I told him I thought he might want to know why his staff is not doing their jobs and I didn't need to tell him to "follow up!". He told me he was too busy taking care of patients still at the facility and that didn’t leave and that was his focus. His lack of concern and follow up is concerning.

I am a friend or relative of a current/past resident

My mother has moved into Piedmont Post Acute. The main reason we chose it was because she had been there once before. It was under a different name, but it's the same staff and everything, and it's close to her family. She really likes it there. Right now, she's there for rehabilitation to help her walk again after the break of her leg. The staff has been very helpful and very attentive to her. She likes the staff, especially the therapeutic staff that are working with her. She has commented on how good the food is. She was surprised. She does memory testing and things like that. What they're doing with her right now is therapy to help her walk again. They have a salon and a courtyard and a formal dining area, and if her family wanted to come in and fix a meal and have a meal with her, they can. They have quite a few amenities that would be admirable for most places. Right now, her fees are being taken care of by Medicaid and her insurance.

I am a friend or relative of a current/past resident

My mother was in Piedmont Post Acute. They need to improve on communication and the staff. There's not enough staff available and reaching somebody is difficult. The room was average. The food wasn't great, I saw the food but I didn't eat it. The portion was small and there was no selection. A lot of the amenities were not active, there was a hair salon there, but no one ever used it. During the three weeks that we were there, I didn't see them utilizing some of the things they have.

I am a friend or relative of a resident

The staff was always very friendly but the facilities were sub par. I really did not want my family member placed there because her outlook on life changed dramatically once she was moved. She was unhappy and lost all sense of independence. Nursing homes that accept medicare are always very run down and dingy. In order to reside in the nicer facilities families and residents must be prepared to pay a lot out of pocket.
