Thank you Wexford House for the care you gave my grandfather for the last 2 months. You cared for him as if you were his family. You provided our large family that visited daily with love and support. Thank you to everyone who provided care who worked on the 2nd floor. Shelby, Nancy and the many others. You were truly a blessing to have care for him.

I WOULDN'T SEND MY WORST ENEMY HERE! My mother-in-law was here for skilled nursing. They showered her once in 2 weeks & that's because I insisted. Played the blame game on other shifts why it wasn't done. We had family at facility every day for 8-10 hours a day and despite repeated requests to speak to doctors, nurses, therapists, and social worker , no one ever came in or called us. After 12 days, we got a call to say she was getting discharged (48 hours later) or she could stay there for $10,000 a month. We only got a phone call back when they wanted paid. The administration, specifically, the social worker is incompetent and unprofessional. Do your loved one a favor and don't put them in this facility.

Filthy, accomodations. The staff is awful all they do is complain about their jobs and worry about their phones more than they do the patients. He asked them for something they never come or they come 3 hours later. They lose people's clothes, and put other people's clothes on other patients. PT & OT are a joke, the social workers are ignorant. They leave the patients lay in the bed 24 hours a day there are sitting rooms and there's outdoor seating they can't even take care of the patient let alone have someone take them out for fresh air. They deliberately put the call buttons away from the patient's reach so they can't call. It's more like a prison, no actually you're probably treated better in the prison. You better hope that people don't do this to your parents. Close them down

Horrible facility. Administration is a joke. No consistent communication. They lie and cover up for one another’s errors, and they make many. The residents are neglected and left to fend for themselves because there will literally be one staff member on a floor. Dirty and old rooms filled with urine sitting around and helpless residents screaming for help. Call lights are ignored and will deafen you because they’re always on. Employees will literally laugh in your face if you complain. They have no regard for the residents are no respect for advocates and family. Residents don’t always get medication let alone bathed. Meals are frequently just potato chips and hot dogs. Read the DOH surveys and do your research. Don’t be fooled by their spiel.. they’ll promise the world and deliver a nightmare.

It's a horrible place, noisy patients, staff is not helpful can't get any sleep. Won't turn TV down. Midnight and still loud

My husband's health went backwards there in just 48 hours. Only a few good nurses the rest were awful. They need to upgrade that place!! Don't go

Please do not do it. I have nothing to gain by saying theses things. The place is terrible, old and run down. Old beds not enough staff. Don’t go to the bathroom unless you want to spend an hour there. Staff doesn’t like to shut up at night and talks about patients. They need to investigate this place. I wish Joint Commissions had some control.

My grandmother has been here for rehab. For two weeks they were getting her out of bed only once a day. Don’t answer when you right for an aid so she would spoil herself and sit in it for hours. She literally has bed sore all over her backside. Today was the last straw I’m taking her home and hiring someone to help her. Came in first time. She was soaked through whole bed. Thought accidents happen. I changed the bedding and cleaned her up. Didn’t even bother the aids. Get a call at 2 am. I show up and she is covered in her own excrement. It’s everywhere. She said she pressed for an aid and they didn’t shown up. I cleaned it all up my self and rang for the aid just to test if they show. No oneanswered the call for over two hours. If you have a loved one mentally not all there this place is a nightmare.

My wife was a patient at Wexford Healthcare Center. I thought the care was excellent ! The therapy staff were so good with her - they got her up and walking within no time. The nursing staff took great care of her. The nurse aides got to know my wife so well that she knew each one of them by name and still talks about how well they took care of her. I would not hesitate to recommend them!

My grandfather was a long-term care resident here. Don't be fooled by the window dressing! While some of the staff seemed genuinely helpful and caring, overall his care was abysmally poor! On one occasion, members of our family discovered him eating breakfast. With an open faced poopy diaper sitting right next to him. Because he could not properly take care of himself, and tend to his own hygiene, he always had contacted fecal matter under his fingernails. Additionally, he was rarely ever bathed or shaved. His entire floor regularly smelled of feces and urine. And nurses regularly stack numerous residence in their wheelchairs. Immediately adjacent to the Nurses Station's. Where are they sleep for most of the day. Instead of receiving any kind of stimulation whatsoever. This most certainly interferes with residents being able to achieve any kind of normal wake/sleep cycle. This was an absolutely terrible terrible place for a long-term care resident. I would strongly discourage anyone from even considering it for any reason. They are simply collecting peoples money. Without any care whatsoever for human decency and dignity. Shame on the people who run this place!

Things are missing from her little refrigerator on a regular basis. They don’t bathe her very often. Today the room smelled like urine so bad that’ I had to open the window and it’s cild outside.

The therapy was good I when my father was in skilled nursing but once my father went to long term care and therapy ended In eyes some staff and dr. he was just a A body to care for there are good workers just not enough. You don't dare ask with bathroom needs/ changing between the 4:15pm and7ish pm because it is dinner for residents as we were told but had noticed many times it seemed to be a good time to take a smoke brake. Once I asked for help to get my dad back in bed I was told it would be a little while then saw the employee get on the elevator and return 20minutes later with a take out bag proceed to go eat (shift had only started an hour before) it took an other hour with asking because it was dinner time but no food was presant. This is just one of many many things. Poop on the shower gurney oxygen put on my father that wasn't order by hospice for him, a family of a roommate had asked a nurse take my dad out of bed and placed in the hallway because he was noisey this was a process because was lift but was put back before I arrived because they knew my routine. I was great full some one told me. I talked to admin and told nobody else family has right to have a nurse have staff remove my father from his bed these were just a few examples. Sadly my dad passed away there it has been well over a year since and I think about it often my family and I were there everyday and we would help it saddens me to think of people who have no one to make sure they are ok. Like I said there are good employees there but are short staffed and the few that aren't can really make a hard situation horrible. Also the food was no good.

i went on a tour of this facility yesterday. it smelled like urine so bad, they told me it was a shift change. i don't think so. i felt so bad for these poor old people that had to stay there. they need reported, i wouldn't leave a dog there. stay away.

My experience with Concordia of Wexford is very pleasant, and they worked well with me. Their staff is wonderful, informative, helpful, and very nice to my mother. They look out for her. Their rooms were very old, and my mom shares a room with another patient. It?s very accessible, and the grounds are very pretty. They do activities, games, and movies. My mother is 86, and she is very active now.

I was extremely pleased with the care and therapy at Wexford Healthare Center. They made my mom laugh and made her feel like she was family. She didn't want to go to a nursing home for therapy but in just a few days, she said it was a great decision to help her get stronger. The staff were great!

My father was in this place for about a week before we moved him. He could not get help for extended periods of time after pressing the "nurse call" button. Some of the daytime staff seemed very good, but the overnight and weekend staff was unpleasant and unresponsive to the patients. I'm sorry it took us that long to get him moved.

We’ve visited different communities and Wexford House seemed they have wonderful programs in place. They have a lot of daily activities. The facility itself is very clean. The people that I've met were very happy that they were there. The staff are wonderful. The food was excellent. They have hair cutting facilities and a gymnasium.
