My wife was a resident of Avamere Rehabilitation of Oregon City for a short time. The facility was pleasant from the outside and very institutionalized on the inside. My wife was in a two-person room where she was a single occupant most of the time. They brought the meals to the bed. My wife wasn't able to participate in anything except rehabilitation at that time. The staff members were absolutely great. They were very friendly. Everything that she needed, they were there to take care of it. It really seemed that they cared about her. The facility seemed to be cluttered. There was a lot of excess equipment stored off to the side in the hallways. It was covered by insurance.

My wife was in a two-person room where she was a single occupant most of the time. They brought the meals to the bed. My wife wasn't able to participate in anything except rehabilitation at that time. The staff members were absolutely great. They were very friendly. Everything that she needed, they were there to take care of it. It really seemed that they cared about her. The facility seemed to be cluttered. There was a lot of excess equipment stored off to the side in the hallways. It was covered by insurance.

My Mom went there for 2 weeks to "gain her strength back". She died 2 weeks later of dehydration, kidney failure (from the dehydration), and a terrible infection.

Too of high staff:client ratio although most of them seemed nice and pleasent, they just seem to have too many people to care for if they had even a few less clients they could have time to spend getting to know the entire person which might help staff and client alike . Poor treatment from the house Dr. He might try working on improving his bedside. And even though I have my own team of Dr.s working with me and managing my care and medications very very carefully house and sent standing orders for my care & medications Dr. attempted and then did change standing orders and medication that had been closely monitored by several Dr's the entire experince with this medical house Dr was frustrating and extremely upsetting. The food didn't seem appealing to me at all. As for quality I can't say since it was not real inviting, and I could not get myself to eat much. I was lucky my my husband was close by and brought me in food. And then had concerns about the cleanliness of my surroundings......I had a shared bathroom with several other rooms , the toilet went most of the with dried fecses (poop spray on toilet seat) on it, my husband noticed it as did I and staff was told and housekeeping was supposed to come clean it. Again I think if staff wasn't so over worked it wouldn't of been an issue, as well maybe the staff taking a client to the toilet should be in charge of clean up instead of waiting for housekeep or at least regular cleaning schedule (more than once a shift especially when several people are sharing a bathroom. ) Basically I cried from frustration and pain most of my stay and was packed and ready to leave at least once a day. (even if it was AMA)Which could of lead to insurance not paying the $4000 a day bill.) I would suggest not using any Avamere facilites ( because they would all have the same basic company policies therefore the same basic client/paitent treatment) Especially if you are putting someone might have trouble advicating strongly for themselves and have a strong outside support system close by for outside asisstance. I had a struggle and I in my 40's educated in social service's and the medical and care giving fields. As well my husdand and children and friends that were all only 3 miles away and where able to come a check on me and bring items I need several times a day.

If you have a choice between being cared for at this facility or living under a bridge, take the bridge. The food is inedible. Allergy and other dietary requirements are ignored. I found oxygen unplugged on two separate occasions, and the telephone unplugged from the wall once. I witnessed a time period of over 1 1/2 hours between pressing the call button and someone wandering in to turn it off - not to offer help - on several occasions. Patient went over a week and a half without a bath due to miscommunication and indifference by the staff. We experienced a weight loss of over 30 pounds in just one month due to staff not watching or caring if the patient was eating, which precipitated a heart attack. And when patient was taken to hospital by ambulance, the staff threw all belongings into garbage bags and locked them in the social worker's office. Do your patient a favor. Keep looking elsewhere.

Most of the people at Avamere Rehabilitation of Oregon City are pretty good, but I don’t think there are a lot of communication between the management and the workers. I had to go to the head of nurse to say some very important things, but it was never passed on. The communication is an issue. The place is very clean. I think the atmosphere is nice. The food is excellent.

My Grandmother was a resident at Avamere and we thought it was great. The facility was clean. The staff friendly. The food was great and I purchased meals there often. I would recommend this facility to anyone.

If you have a dog in need of rehab care, do NOT send it here, it wouldn't eat the food. This facility is terrible, the nurses are not properly trained, some good, some awful, the aides are OK for the most part. The facility is older with not much newer equipment in the PT and OT departments. Housecleaning does a terrible job, the room smells worse after they clean. The laundry takes days to a week to be returned and some of my laundry was lost. Terribly understaffed, you can wait up to 1/2 hr. for an aides help. The pharmacy is awful, needed meds are not in the building in a timely manner. What can I say, stay away...

ad has been in a few different facilities so far since his illness and this was hands down the worst experience we have had with any of them. His clothing went missing regularly although they did replay him for new items after many weeks of complaints. There was some complication or another with his medication orders and supply daily and he had multiple incidents of medical issues (requiring emergency room stays) that, had the nursing staff been doing the required charting and well being checks would have been far less serious if caught earlier. The strict dietary restrictions he had weren't followed by the dietary staff (dessert being served to a diabetic, salt on the tray for a no sodium diet) and his blood sugar management was atrocious. He would tell the staff he was feeling like his blood sugars were low and it would take over 30 minutes to have his glucose levels checked. This happened at least weekly. By then he was so low it required emergency intervention that possibly could have been avoided had they responded in a timely manner. He was left in the shower (on strict orders for assistance when standing) for over an hour once and had to slam on the wall with a shampoo bottle until someone finally came. The room was not cleaned frequently and the staff regularly left messes and obstacles in the room which made moving around freely in his wheelchair impossible for him. What prompts this review though is his treatment upon his most recent week long stay in the hospital (which was caused by Avameres nursing staff not noticing a 40 pound weight gain in 35 days.) Upon being ready to return to Avamere his social worker at the hospital was informed that Avamere would not accept him back as a patient and would not give a reasonable reason as to why. Because of this he was forced to move to a facility over 30 minutes from us when Avamere was only 5 minutes away. A staff member "packed" (aka shoved everything he owned into garbage bags and a box) and locked it in the Director of Nursing office. There was no inventory completed nor was there a second staff present as a witness and we have not had a chance to go through them to check for missing items yet. We were unable to retrieve his belongings that weekend and had a terrible time trying to reach someone to set up a time to do so as the office was locked both times I was able to stop by. When we did get to speak with the DN he was extremely rude and disregarding to us, refusing to even look up from the papers in his hand while we were speaking and blowing us off stating we needed to talk to the administrator. After numerous complaints from dad they finally sent his belongings over with the maintenance guy almost two weeks later but it put a ton of unnecessary stress on him. The care received by a few of the staff was great but most were highly unprofessional and rudeness seems to be the norm. I regularly found the Nursing Assistants hanging out in a group by the front desk, eating and talking loudly about their patients or the management and their issues with them. I also can count on one hand the amount of times I witnessed a staff member wash their hands and had to ask that they did on numerous occassions. This was usually immediately after they had left another patients room and entered dads attempting to provide care. The absolute disregard for patient privacy was horrifying as I regularly overheard patients full names being used as well as detailed descriptions of their health and or behavioral issues (and this was by the Director of Nursing and management as frequently as the care staff!) Do yourself/family member a favor and skip Avamere. Unfortunately, good skilled nursing facilities are hard to find but I can wholeheartedly recommend you stay away from this one.

My mother was at Avamere Rehabilitation. The food was excellent. The staff was extremely good. The rooms were the exact size and very clean. They had church services, music and bingo games. I would absolutely recommend them to other people. They're excellent.
