If you don’t mind a run down place that looks like a prison and NO response to phone calls asking for information or assistance with a family member there, this is your place. I waited 45 min on someone to come back on the call once. EVERYTIME I ever called, I never spoke to who I was asking for. However, they were pretty attentive when the state inspector was there. Only saving grace, many of the CNAs had compassion and a heart for their job.

This place was dirty day shift staff not soo pleasant rooms were not clean it smelled when u first walked in. Building needs to be updated like redone. Honestly do not see how they are open still.

My Grandmother has to be there for a rehab, and they have not bath her in over 2 weeks. They do not take her to the restroom they make her go on herself and then when they have time they come and half way clean her. She went to the doctor today she has a skin infection now and I am calling the state because this is horrible. I have worked in nursing myself so I understand having more than 1 patient but this is just damn wrong. She will be home or moved ASAP.

My loved one was in Willow Park for a few days but left as soon as we could get into another facility. The building itself is very old, cinder block walls which makes it look like a prison, shared bathrooms for private rooms, furniture not cleaned, even rails along hall ways are sticky with who knows what. Willow Park is very unprofessional in everything they do. The food is horrible, the response times to call lights is way too long (up to 30 minutes when they knew a person was left sitting on the toilet), cleanliness of rooms is poor, director of nursing seemed caring and concerned in first meeting, but later was too much in a hurry and didn't want to be bothered with medication and cleanliness issues, and administrator is not made aware of issues, although she does seem caring. CNAs and others don't wear name badges, or were placed down at bottom of their shirt, so didn't know who is who and their main responsibilities, and didn't introduce themselves. Couldn't get my loved one out of there fast enough. Medicare ratings are misleading. Do your homework.

We visited this facility in August 2017. Facility was unclean, smelly, and disgusting. Our cousin had eyeglasses two week ago, now he doesn't. Staff doesn't know anything about that. Talked to various staff members; left with the impression that the staff members did not communicate with each other. Discussed moving our cousin to a facility closer to his home and was met with sustained and strong resistance from the director of nursing. Will definitely be moving our relative out of there.

I have a friend in there and the care appears to be adequate, but the attitudes of the administration is terrible. They are all about their pockets...not about my friend.

It was heartbreaking to see my daughters' great-grandmother here. My older daughter couldn't stand to go back. Rooms were small, without good visiting facilities/places to sit.
