This place is disgusting! The smell will make you throw up. The patients are dirty laying in bed or in wheelchairs moaning. The food is the cheapest frozen junk they can feed someone. There was NO staff two nights in a row. I mean NONE! My brother walked the entire facility and no one was there until the 7am shift came on. There was no one to let you in the door, I was told she just got up and left. Even the workers arriving could not get in, kept pounding on the door/window glass and calling. Took 20 minutes to be let in. I would not ever put someone in this place. Will be reporting to authorities.

While care may be fair during the daytime during the week nights and weekends you are pretty much on your own. Don’t bother pushing the button for help response times are 90-120 minutes if not longer. 2 mornings at 4 am I was experiencing pain and called for my prn medications. I finally received them after 8 am. A trip into the halls at 5 am looking for staff revealed no staff at nursing station and no staff seen in any hallways. It was a ghost town. This was on a Friday and Saturday morning. Lots of excuses. Forgot to order meds. Understaffed. Assigned extra patients. On and on. When they do respond staff are courteous.

Our mother is there, which we are picking up today. We try to call at least 3 times a week to talk to her. I've had residents answer the phone or be put on hold for over 30 min to then be hung up on. We visited her last week. Upon entering the facility they brought my mom to a licked door, allowed us to enter then took us to an empty patient room. We were told this is the room for COVID-19 patients when they arrive but for now we have to visit there. My mom has not and does not have COVID-19. The nurse came to let us know dinner was being served 20 min after us being there. She was extremely rude about it, we asked her could she please save her tray. It seemed to be a huge inconvenience for her. After speaking with my mom yesterday I asked her to see if she could locate the social worker there. She yelled for over 15 min. Then finally a social worker called us back about 10 min after hanging up with my mom. All that was said is that "when were you going to tell us that she's leaving" this is the 2nd time since she's been there that anyone has every called me back regarding my mom. She has stage 4 cancer. Even the hospice nurse can't even get through on the phone with anyone or they don't return calls. There is so much that could go on.. but please take heed that no one should EVER be put there!!

I visited this facility due to a family member. I was impressed with what I saw and also with my interactions with the staff. My family member has enjoyed his experience there, particularly the activities and the social engagement aspect.

I was well treated by a friendly and caring staff.It is kept clean and sanitary.the rehab staff was excellent and the food was above par.I fell like family there .

I went to see my brother. on Dec.16.2017 the facility is run down. it smells the patients not been cared for like they should. My brother clothes were nasty , no aide or Nurse came in to see if my brother needed any thing .They said that they were doing therapy on him but are not. So unprofessional. Don't carry your love one there.

I recently visited this facility and I would have to say that I will never Vist this facility again. The smell is horrid as soon as you walk through the door, the nursing station is very unhelpful, the nurses are very careless and not respectful of the patients. Very disappointed.

My Mother-In-Law is a patient there and I love it. It is a clean facility, the nurses and doctors watch her with compassion and keeps watch on her like it was their mother or mother in law. This is the 3rd facility she has been in . She gets the attention she needs now, never did in the other facilities. They even cut her nails for she was scratching herself with them. Very happy with Brian Center and I am so thankful she is there. Thank you all for the good care and attention you are blessing her with.

My dad said they were mean at Brian Center Health and Rehabilitation. They did not have good meals provided. It was not a nutritious type of a meal that I saw them bring, and he did not get the care he needed.

My mom has been at this facility for five years. During those five years, the care provided has been consistently on the decline. Early on, I had experiences whereby the resident doctor basically made me believe that my mom was dying, needless to say, this should have been my first clue. Not nessarily the doctor's fault, as the problem stemmed from the staff mishandling her urine samples and thus receiving false readings. There has been quite a few signals that I should have picked up on that this facility was run like an elderly puppy mill. Recently, the staff there fractured my mom's arm. To add insult to injury, their rationale behind how this happened is totally ludicrous and obviously a cover up. I am moving my mother from this facility as soon as I can, I would strongly suggest that if you care about your parent, you NOT leave them at this facility. In my opinion, it is very poorly run!

Mom has been in Brian Center since November last year. We thought that was the best place for her because of family and friends who are close in that area. It's a lovely place. The staff is very helpful, very friendly and professional. As a matter of fact I have a cousin that's a nurse there so everything is going well. I've been there several times. I visit every other week to see her and just to check. They have activities like physical fitness, cognitive exercises to keep their mind active, they read a lot, which I think is excellent. I would recommend this facility.
