My mother was a patient at the Oaks. She was admitted for rehabilitation and later transferred to Long Term Care. The rooms were clean and larger than most nursing home rooms. She had bathroom with a large walk in shower. She had a room mate that was not very nice but the facility moved my mother when they realized it was not working out and she ended up with a very nice lady. She and my mother became very close. They enjoyed going to Bingo and crafts together. My mother also loved to read and the Activity Director kept my Mom well stocked with books and magazines that she liked. They took great care of my mother. Any problem we encountered was always addressed. The medical team were so nice and took great care of my mom. The Dr. and nurses were patient with my mom as well as with our family and kept us well informed. I would not hesitate to place another loved one at the Oaks!

The rooms are fair and the staff is friendly. There is always someone available if you have a concern or have questions.

First, I would like to thank staff and administration for the service rendered to me, during my stay at the Oaks Rehabilitation Center. I want to give a special thanks to physical therapy/occupational therapy for helping me reutilize muscles that hadn't been used in a long while. To the staff, the nurses/CNAs that cared for me Day/night, YOU KNOW WHO ARE!!! Thanks. Lastly, administrators give more attention to those that really take time with the patients and do their jobs. It's very demanding. The laundry department could really use some improvement. I understand that the family of the patients are encouraged to wash clothes, but in those instances where they can't, if you put your name in your clothes, they should be returned to you in a timely manner. I was impressed with the facility. I liked the cleanliness, keep up the good work.

This place is absolutely terrible. Lazy, untrustworthy staff. They treat the residents with no respect or kindness at all. Filthy doesn't begin to describe the entire facility. My grandfather never has his own clothes on and we provide everything for him. His belongings are gone every time we visit. We have told them multiple times that we do not do his laundry and yet they can't seem to get the message. Residents are left to fend for themselves leading to unnecessary falls and other injuries. The staff is too busy gossiping, playing on their phones or watching their OWN children to see to the resident's needs. I remember when this facility was one of the good ones in town but it has fallen hard. Please do not leave your loved one here to suffer.

The staff are poorly trained to take care of the elderly. The Rehab services are below standard. The nurses do not assist with baths or feed people. The administration staff are not professionals. They point fingers and say things to make you feel better, but no actions are ever taken. There is no team work. My family member was there for 3 months and more than 7 employees were fired or quit. My family members health didn't improve, it got worse. Do your research, and stay away from The Oaks. They are under the Liberty Health Management. They will not return your calls if you complain.

The rooms are small and not cleaned daily. Bathrooms not sanitized, often had to request housekeeping to clean the room. CNA staff is always short of help. When one CNA calls in, patients do not receive average care or no attention. When reporting complaints to the nurse manager and head nurse, they always say "we are working on team-building skills and hiring additional staff. No team-building or additional hiring since July. Some employees often use profanity, especially when they are short of help. The food is just horrible and is an easy fix. Use another vendor. Nures (RN) do not listen to patients. One RN is often searching the internet for children's clothing and she's quick to explain why she's not doing her jo. The lighting in the rooms is poor. They need to use a white bulb instead of a yellow one or install ceiling lights. CNAs often forget or choose to leave the call bell behind the bed so patients cannot reach it. Laundry is terrible and often not changed. Do not leave your family members alone for staff to take provide care. Select a different/better facility.

The Oaks is a Great place, I have had family and friends to stay at that facility and they all had wonderful experiences. Left there with great rehab and nursing care services. They would highly recommend The Oaks anytime. Rooms are big and private. Housekeeping is wonderful. The staff is very kind, caring and compassionate to the residents and famalies. Activities are great. Favorite activities are Bingo of course.

This facility is not a facility to send your family. They do not comunicate with family. They do not service the patient. The food does not even look edible. The staff is uncaring to patients and family. If I could rate more then terrible it would be selected. Do not let hospital send family here. This was to be for rehab if you call it that. 15 min session and if he wanted to get washed up they said that is his rehab.

I am very disappointed in this facility. My mother went there for rehab after surgery. Although the staff was friendly and courteous and her physical therapy sufficient, I feel they didn't tend to her personal needs the way they should . The food was mediocre at best. The worst thing I encountered was their lack of communication with the family. We were missled about several things. The public areas were clean and bright but her room lacked a little to be desired. Not like the photos on their website.

My wife was at The Oaks for five weeks. It was good. They offered skilled nursing and memory care. The place itself was good, nice, and clean. It didn't smell like the traditional nursing home. The rooms were very large and that was a nice touch. The staff was good. They looked after everybody. They seemed to be very caring and very considerate of my wife's injury and problems. They had excellent therapy staff, too. The food was sufficient but mediocre. I wasn't impressed with that. My wife was in no position to go on any of the trips or any of the activities that were planned.

My 86 yr old mother was a resident at The Oaks: I'm very disappointed in her care: although the medical staff, therapists, and professional staff are excellent: this facility has inadequate CNA coverage and horrible housekeeping. I removed my loved one within 3 days due to unsanitary conditions and lack of CNA care!

My mom-in-law was at The Oaks at Forsyth for rehab for 8 days. We were told she could be there for 21 days -- the insurance paid for it -- and at the last minute, they told her she had to leave and that we had 2 days to get her to move. We were not given the proper notice.

My family member is in The Oaks at Forsyth right now. They have excellent service, excellent information, and excellent caregivers. There’s always somebody knowledgeable on the premises. Any time I go over there, I can talk to someone if I need to. Her care has just been wonderfully fine.
