My sister was at Transitional Health Services of Kannapolis. She got physical therapy and occupational therapy. It's extremely clean. The staff was very helpful, friendly, accommodating, and willing to help the patient and the family. My sister doesn't like food that is not from her home so she wouldn't eat it, but I like it. I had a few things. They're very nutritional and a good amount. It was very nice. The staff members are good. They're excellent. They're very attentive to their residents.

My sister was at Transitional Health Services of Kannapolis. She got physical therapy and occupational therapy. It's extremely clean. The staff was very helpful, friendly, accommodating, and willing to help the patient and the family. My sister doesn't like food that is not from her home so she wouldn't eat it, but I like it. I had a few things. They're very nutritional and a good amount. It was very nice. The staff members are good. They're excellent. They're very attentive to their residents.

The staff is an inclusive group that seem to really care about each other. The building was clean with no odors. The staff engage with the residents and strive to keep them clean and healthy. More than adequate number of staff. Translators on site for Hispanic residents.

Please don’t send your loved ones to this facility.Not enough staff to take care of patients. Their pt was basically non existent.Staff lied to meet goals and I was left in soiled diapers for24 hours it took forever to get someone to help you. Doctors again non existent. You’re risking your loved ones health by sending them here. Stay away and find somewhere else

As of October 2022 this facility is on its third administrator as I've seen so far. Most of the older staff are nice enough. I have a sister in for long term care. Long story short: I received on friday Oct. 7, 2022 a call my sister had tested positive and that day she had tested negative. For some reason instead of retesting they had used the same test (to me that is controversial) and the staff had read the 'negative' result wrong. How does a trained staff read a covid test wrong and hours later use the same test claiming it was actually positive without retesting??? this new administration seems to reek of incompetence. How do you have a staff that has no idea how to read a covid test? keep your loved ones AWAY from this facility!

Angela moved my mother during COVID to a hall by herself. My mother was bed ridden and unable to feed herself. Basically she was left to starve to death but Angela noted it as A Symptomatic COVID death. Yet she had no treatment for COVID plus had bed sores. We were unable to visit my mother from March 2020 to Oct 2020 when she passed. They knew how to get the money from the government on the most vulnerable patients. Do not trust them with your loved ones

This place is awful, find anywhere else you can. The food is gross, the facility is smelly and dirty, the beds are antiques and the staff is incompetent and the administrator is rude and uncaring. The “activity” bus is always broken. Patients lay around in soiled diapers while the call lights go unanswered. I don’t see how this place is not shut down.

My father was at Transitional Health Services of Kannapolis for six weeks. It was fairly clean, but it was old. As far as his care, it was OK. They took care of him, bathed him, and fed him. They served three meals a day. When they first moved him from the hospital to the nursing home, they put him in rehab, so he was able to do some of that. He never did see a doctor, but he did go to dialysis three times a week and they were good about transportation and about providing lunch for him while he was there. However, they need to improve on the communication between the family on what's going on. Most of the staff members were kind and caring, but some you can tell are just there for the paycheck.

After having hip replacement surgery my sister decided on Transitional Rehab based on recomendations from our senior neighbors. Immediately I was struck by the kindness and welcome by the entire staff. The facility is not the newest but has heart. My sister has had 2 strokes on her right and hip surgery on the left and had minimal rehab at the hospital and was feeling she would get no better. 5 days later she walked 25 ft using very little assistance and has a new outlook. The staff is very professional, friendly , well trained, very, very caring and have a natural vocation for dealing with patients with pain and handicaps. We are very happy to have chosen Transitional.

This is the worst facility I have ever visited. Please be aware and not put your loved one here, The administrator is rude and does not talk well to staff, unprofessional. Most of the nurses are incompetent.

My Mother was there for 19 days and I have never seen such stuff as I saw there I had to go to the director of Nurses and ask for them to do three write up.

The rehab hospital gave us Transitional Health Services as one of the names to call, and we did. It was conveniently located. Although the therapy was excellent, there were other things we did not like, so we ended up bringing my dad home. They were understaffed, so we could not get the help we needed in a timely manner, but the staff was very nice. The food was terrible, and it was of very poor quality.

Two years ago, when my husband had a surgery on his back ,he went to Transitional Health Services for rehab. It was a somber place. They didn't do much for his therapy when he was there. My husband was in a quite bit of pain from surgery and they told him to let them know when he needed the pain pill. When he did call on them, he had to wait before he can have his pain meds. They were always far away and the nurses were just working their way down the hall before they can get to his room. I also had problem with his room. He was in a room with another man. It was clean as far as I remember, but it was very small. Also the food they served was not good. I didn't feel that it was a good facility.

My mother was a resident there recently. She was cared for by wonderful staff. Someone visited everyday to assure she received the care she deserved. Not that we went in trying to find something wrong everyday. But we wanted to be apart of making this a pleasant experience for her. We truly took a team approach. No facility will ever be perfect or feel like home. I got to know the staff the Nurse Practitioner and MD. The Nursing staff was very efficient as well and we gave them all hugs when we took Mom home. I talked to other family members and some said they have been there four and five times. They told us to just make sure you report all concerns to the Supervisors and any issues would be resolved. I think it works and things work out when people don't go in being critical. It takes special people to perform the work they do and they save lives everyday. Just go in with positive and you will come out with positive.

I had a parent in there just recently they they almost let die. I witnessed on several occasions the care givers letting my lights stay on and ignore them. They let my family member sit I. Urine until we found someone, which I must say is hard to get someone's attention there, the 30 something days they were there, we were there daily and never seen a doctor. They let my family member gain 50 lbs if fluid in a little over 30 days. Never would listen when we asked for help, just kept running lab work. He was in there only for rehab, after demanding him transported to hospital, he received to bags if blood and had 53 lbs of fluid removed in three days. After leaving the facility we have heard a lot more horror stories. My advice to anyone before is do homework, talk to social workers and check the web. This was I first experience so we didn't know, but we learned the hard way. It was all about money to them. The physical therapy part is wonderful, but the caregivers and staff are only there for pay!

The facility was very clean, organized and well managed. The staff was very caring and competent in their jobs. If you needed anything, they were right there and constantly checked in throughout your visit. The staff and facility planned a variety of activities to keep the residents happy and healthy. The physical therapy department was very compassionate and firm but effective in the rehabilitation. If the residents weren't mobile, activities were brought to them. Medication and care were given on time. The staff seemed very knowledgeable and seemed to genuinely care about the residents and their jobs. The family was given a report every day on the improvement of my grandmother, and was told about any problems or concerns as they arose. The visiting hours were very acceptable. You were even able to join your friend or family member for meals. I would definitely recommend this facility to others in need of rehabilitation.

My mom's been there since June. She's in for a stroke and had a hip replacement. I think she's been well taken care of. She had urinary tract infection 4 times, so we really haven't gotten to do the rehab program that we wanted to because they isolate her during those times. So in fairness to them, I think they have an excellent rehab program, but she hasn't been able to participate in it because she's been isolated.

The facility is generally pretty but not clean in appearance, and with some slight odors in the air. I saw an inadequate number of staff during our visit and did not see any who were addressing residents in a more personal way, especially the nurses. The staff were "doing their job" and not as if they were enjoyed their job. Not giving many options to those residents who might prefer a different schedule for therapy and activities. Little or no interaction with management and staff. Overall I just was not very impressed with this facility. I cannot pinpoint anything that was particularly wrong with it, but it just lacked that home like feel that I would want someone to have if they were moving in long term. I'm more neutral on whether or not I would recommend this one and rather, would suggest that someone visit the facility for themselves and see if the more "personal touch" that some other facilities might offer.
