I recently had a short stay at Liberty Commons for rehab due to a fractured hip. The majority of the staff was helpful, some more than others; the PT was very good. Also I want to commend Paula; she was outstanding in helping me! I wish to thank the staff for the care I received especially Paula!

Liberty Commons Nursing & Rehabilitation Center of Alamance County is perfect for my mom. We chose them because of the physical therapy services that they offered. The staff were very compassionate. They have done a great job. My mom enjoyed the food. They had activities every day. It was more physical therapy for her; she had intense physical therapy every day. The structure itself is clean. It looks like a normal facility, but it could use some updating. They have a family room, and they have the main dining hall facility. For the physical therapy portion of it, they have their own room down there. They assisted her with helping her learn how to get back into a car so that she could drive again and those things.

Liberty Commons Nursing and Rehabilitation Center of Alamance County went out of their way to help my husband. He falls a lot so they put rubber mats beside the bed and got him a large chair on wheels so he can sit in it and we could push him around. It's a skilled nursing facility and I've had no trouble with the staff. They've been very helpful. I think the place is good and it has a nice layout. He can't do anything now but lay in his bed or the chair and watch TV or play on his tablet. He can't communicate and he doesn't do activities so I don't know what they are. At the other place that he was at, they would leave him lying on the floor if he falls. I have yet to find him lying on the floor when I go and visit him at Liberty Commons.

My brother will be moving into Liberty Commons Nursing & Rehabilitation of Alamance County. It's an excellent community. The place is very clean, and when I walked in the front door, I didn't smell anything. I also toured the rooms. They were very clean, and very well kept. I liked the staff there; everybody was so friendly and helpful. The dining area was beautiful. I haven't seen the food, but they have a large dining area, and it is beautiful. They said, if he'd prefer to have his meals in his room, he could have his meals in there if he didn't want to go to the dining room. The staff was very helpful, very kind, very knowledgeable, and very patient. She explained everything to us. She was really nice, courteous, and everything.

Mom was at Liberty Commons N & R Alamance for six weeks and enjoyed it there. We received very good service there because the staff was great, friendly, and always willing to assist her with occupational therapy and rehab therapy. They did a wonderful job. It was a very good facility, where everything looked fine and clean. Mom enjoyed the food; it was really great. They had activities every day, like games and music.

I agree w/the comment about ahc and bed sores! best thing about ahc are the hospice cnas and nurses from hospice of alamance/caswell. liberty commons is one stept ahead. rehab staff is wonderful and professional! there seems to be couple of great lpn, one on day shift, 2 I met at night! most of the cna staff seem to be mouthy, more of a "look at me, I am wonderful" attitude. one cna seemed to operate the entire facility, and in charge of the entire facility. the facility staff seem to lack respectful boundaries, lack of professionlism, poor verbal skills.

First the good points: 1) the facility seems neat, orderly and on the surface clean 2) the majority of the nursing staff seems to be very patient focused and caring 3) the PT and OT staff are top notch! Wonderful, focused on rehabilitation, patient connectivity, and partnering with family to ensure well being and progress. The not-so-good news:1) in my opinion a few members of the nursing staff and most of the administrative staff (except reception) need to take a refresher course on customer service. 2) the administration needs to re-fresh the entire staff with a sense of "OWNERSHIP" and a few customer service skills. This is specific to a 5 day quarantine that was a result of a dysentary like virus. It took at least 5 days before the facility was "deep cleaned" vs. a simple isolation. Lots of interest and concern about the spread of illness, but thwarted engagement to resolve the issue at hand. Additional issues: 1) there seems to be little coordination of cares services (PT with RN/Meds with Doctor visits with Social work with Exit/Transition coordination) 2) My parent NEVER saw an actual doctor! I was told one visited on Tuesday and Thursday, possibly to review charts, but never actually met with the patient/parent. I can't wait to see the itemized bill for clarity. 3) Like other reviews - a great number of staff members seem to be focused on eating (in the front conference room), exchanging stories with other staff members, and socializing. In my opinion they seemed to be caught up in the IDEA of working vs. actually delivering their services. The exception (meaning the staff delivering quality services) were the CNA's, the PT and OT staff, and few select nurses. In short: WHO IS RUNNING THIS PLACE?! I witnessed two RN's from different wings in an argument about sticking a piece of paper in a file. They were speaking AT each other vs. to each other. This place could benefit from LEADERSHIP to coordinate good efforts into great efforts. In closing - I would pick this facility for my parent and loved one ONLY if I (the family member) had a free schedule to "run shotgun" over all the efforts to be coordinated and delivered. Also - I can't forget this one: The food is horrible - I saw one meal that could not have cost the facility more than $2.50 tops! Yes it was a balanced meal (one meat, one over-cooked green dish, one - starch...and ..wait for it - the slice of white bread that was on every tray, every meal). What are they charging for food vs. what are they delivering.

Love the staff glad to have my Mom back to a facility I can trust instead of a facility like Alamance Healthcare which had given my Mom pneumonia and bed sores from neglect. The nurses and the staff at liberty Commons go out of there way for my Mom and my Uncle and I feel so much better and so much more at ease.

I will make sure that myself, and my loved ones, never set foot in this facility!!!!! I have never dealt with such non-professionals in my entire life! I had no idea people such as the staff there, "exist" in the real world. I met one nurse, LPN, that was professional, polite, compassionate, she is only part time! The rest of the staff is questionable in my opinion. Place is not all that old, but structurally, it appears old and falling apart. Nothing good to say about the place!

Lots of smells, urine, bm. Smell better spray isn't used due to lung issues I was told. Makes sense, yet, wet, urine and bm soaked towels and sheets along with diapers could be removed from building at least hourly. I noticed the trash and linen, very soiled stayed on floor, possibly until shift change? DON was very nice, polite, appeared well-educated. Social Worker appeared to know her job and did it. Lots of activity at the Nurses Station, what appeared to be CNAs eating, drinking, reading paper, gossiping about others. Everyone needs a "break", but so unprofessional these were. Loud as well. Appears to be an old building. I suppose this place is just as good or worst than any others. There is no place like home. Love one was eventually able to leave.

I did not get much out of this facility. This receptionist was nice but that was about it and some of the guests I met were also. It took about 20 minutes for me to meet the management because of an "emergency" which can happen so I will let that one go but I got no tour and no extra time. I was just sat down for a few minutes and asked a few questions and vice-versa. It almost seemed like she was in a hurry. We finished our little talk and I was sent on my way but not before I met an elderly gentleman sitting in a chair looking out the window patiently out the window as if he were waiting for someone. I said hello and he replied back. He did not look all that happy. I can only speculate as to why. The grounds needed some work and the building also need some repairs here and there from what I saw. I would not recommend it unless you saw the entire building.
