Achieve Rehab and Nursing Facility was fine. The food wasn't so good, but everything else was good. The building was fine. My father had his own room. They get a lot of sandwiches for dinner, like a ham sandwich, and to me, that didn't seem very reasonable. The staff was very good and very helpful, though. They're very polite. The rehab professionals were very good.

i like Achieve. I was living in a different facility and was very unhappy. i moved here and i am a lot happier. the staff and bosses are nice to me, and the food is really good. i enjoy the weekly activities and visiting with the friends I've made here.

My grandfather stayed here for rehab for a couple months. During that time, he grew weaker and weaker because he was being fed pureed food even though he could chew fine. When we finally got him out of there, he ate for nearly 12 hours straight and still wanted more food. When we visited, they made us see him in groups of 5 in a large outdoor area which we all could have easily fit in while social distancing. After just a couple weeks of actually being fed he had the strength to walk again. He asked us not to complain to anyone out of fear that they would retaliate against him. To top it off, as we were picking him up the clueless administrator had the audacity to ask if we would leave a good review. The greed of this place is insane.

In the two months our father was a patient at Achieve, he was not treated with the respect he deserved. He was given pureed food, every day, for all his meals and was going to bed hungry every night. Our father can chew food, we didn’t ask for him to have pureed food nor did he need pureed food. The day we rescued him, he ate for 12 hours straight! Ate real food, a sandwich, pretzels, cookies etc. Our father was given oxygen, every day, all day, no idea why, he doesn’t need oxygen and hasn’t used oxygen since being rescued from Achieve. Achieve tried to charge us $300 for oxygen he didn’t need. Our father was supposedly given Physical Therapy. I say supposedly because when we picked him up his leg was, and still is, contracted. This means if we try to move his leg it might break because its so stiff. This happened because he was put in a chair in the morning and didn’t move until it was time to be put in bed at night. He has a bad sore on his back from sitting in a chair all day. The business office was called multiple times for 3 weeks and didn’t return our calls until the day the insurance ran out. The phone call from the business office went like this “You need to give us $4,000 for the next week and $12,000 for the month of August. We need that ASAP”. Lastly, when we mentioned complaining to our father, he said, and I quote “Don’t complain, they’ll take it out on me.” We were shocked and distressed to hear these words come out of our father. Our father fought for his country in the Korean war, he deserved much better!!

because of the care i received at Achieve i am here today. the staff were wonderful and the room was nice, everyone was caring and answered questions when asked. my only issue is resolving the bill. when i ask for clarification of who paid what and how much the bill was i just get sent another bill.

I would not leave my Dog their it is a nightmare, the bathroom sink was falling off the wall the bed was from the 40s that you cranck up. the night staff was awful. like they didn care what so ever. stay away

Over crowded and smelly.Staff talks down to "patient" as though they were idiot children . Lord- keep me away from HAVING to go here.

Besides for the great therapy department, there were so many things to keep busy with. Social events, Planting, baking. Food was plenty and varied. The specialty kitchen that recreation runs for special events was a surprise. I'm so glad I found Achieve.

My friend has returned to achieve 3 times over the past 5 years and cannot speak highly enough about the place. Thank you for what you do for the community by having such high standards. We used to have to go to the city for such good Rehab.

Horrible care. While the place looks pretty nice, my grandmother received horrible care here. She went in for rehab after a hip replacement and I was just apauled by the care or lack there of she received. When she asked for help walking to the bathroom they made her wait over 25 minutes and the proceeded to just give her a bed pan. The nurse then left her to sit in her urine soaked bed for an hour because "she was very busy". Due to their negligent staff she could have gotten an infection or worse. They are way understaffed and the patients suffer as a result. Please don't send anyone you care about here.

Very satisfied with the care. Leida and Travis take pride in their short term floor. Was surprised that food was tasty and offered variety. Highly recommend.

My relative was a rehab resident at this facility. At first, she was placed in a room with another patient who was loud and demanding. We finally were able to get her moved to a more suitable room, but had to create quite a fuss to do so. The employees on the floor were not very helpful. Many times when a request was made, even for something as simple as an extra blanket, the employee would announce, "That's not my job," and refuse to even offer to direct us to the appropriate employee. We spent a lot of time trying to get basic care for my relative. Then, my relative, who seemed to be getting sicker and sicker, thought she was being given the wrong medication, so she did not take the pills and gave them to my wife when we visited. My wife looked them up on the internet, and sure enough, they were pills that my relative had had a bad reaction to years earlier! By then it was too late and my relative began to have renal failure. We took her out of the facility under the pretense of taking her to the doctor, but took her to the emergency room. She died a few days later. A lawyer told us that due to her age, a court was unlikely to find fault with the rehab center. TO this day, we regret choosing Achieve for our relative's care.
